cannot be granted until the following items have been
completed:
(i)
The water analysis report needs to be submitted to
th~s office from the Chem Lab, 5633 B Street, for
our review.
A well log submitted to this office for our review
and files.
Exposed electrical wires to the well head are in
violation of the Municipality of Anchorage codes and
must be encased in conduit and buried. This
will need to be reinspected by this office.
(4) The septic tank pumped with a receipt submitted to
this office.

Comments: This investigation was completed in compliance with MOA guidelines, regulations,
and best industry practices / methods. The assessment of the condition of the well and septic
applies only to the conditions as of the day tested. The flow and absorption rates may change
due to subsurface conditions that may not be observed from the surface, changes in land use,
local soil characteristics, groundwater levels that may fluctuate during the year, quality of
