Dear Mr. Garness:
I have reviewed your October 22, 1998 submittal regarding a domestic wastewater system,
consisting of a 4000 gallon holding tank and sewer service lines for a six-bedroom duplex in the
Lauritz Subdivision. Based on this review, I have the following comments.
Wastewater
From the submitted information and our telephone conversations, it appears that the planned
system will meet State Wastewater Disposal Regulations and guidelines. Based on the soil
condition, from the test results you submitted, the use of a subsurface domestic wastewater
disposal system is precluded. The 4000-gallon capacity of the holding tank will meet the volume
of wastewater produced without modifications. Therefore, the proposed system to serve the six-
bedroom duplex located on the above-referenced property is temporarily approved for the
concerns of this Department. A Construction Certificate, constituting this temporary approval, is
enclosed.
Therefore, subject to the following requirements, the construction plans for the proposed
wastewater disposal system are approved for one year, in accordance with 18 AAC 72,
Wastewater Disposal Regulations:
The holding tank will be used for a maximum of one year, which allows adequate
time for the owner to extend city sewer service to the above referenced property.

2. The holding tank must be regularly serviced.
Complete as-built drawings (plan and profile view) of the wastewater disposal
system will need to be submitted to the Department under the stamp of a P.E. The
submitted as-built drawings will need to verify placement of all components of the
wastewater disposal system. Due to the limited space on the site the as-built
drawings will need to verify horizontal separation distances.
As discussed during an October 27, 1998 telephone conversation with Nicola Metzger of this
department, you stated based on the 60-foot width of Lots 1 and 2, the minimum separation
distance of 75 feet from the private well on Lot 3 to the proposed holding tank should be meet.
Waiver of Separation Distance
You have requested the Department waive the separation distance between the Class 'C' Public
Water System and the private sewer service line from 75 feet to approximately 55 feet. I have
reviewed the information submitted and find the requested waiver is acceptable.
Therefore, in accordance with the provisions of 18 AAC 72.035, the following separation
distance is granted:
The separation between the Class 'C' Public Water System source and the private sewer service
line has been waived from 75 feet to 55 feet.
This waiver is valid if the following requirements are meet at the time of installation:
The existing sewer service line is replaced with cast iron pipe until outside the 75-
foot radius of the Class 'C' Public Water System.
2. All the joints of the private sewer service line are shrink-wrapped.
The private sewer service line is pressure tested after installation and prior to
burial.
Drinking Water
From the submitted information and our telephone conversations, the Class 'C' Public Water
System is not an approved water system. To help expedite the approval of the system, a Plan
Approval Checklist for Class 'C' Public Water System is enclosed. The checklist highlights all
the necessary requirements for approving the water system.
These approvals do not imply the granting of any additional authorizations, nor obligate any

state, federal, or local regulatory body to grant required authorizations.
The State Wastewater Disposal Regulations require fees for plan reviews of domestic wastewater
disposal systems and waivers. The total fee required for this project is $525.00. You have
already paid $295.00 for the plan review portion of the project. The fee invoice for the waiver
separation is enclosed. Please submit fee payment of $230.00 to this office.

Dear Mr. May:
This office received a request for approval of indivdual
sewer and water facilities. The request was submitted as
a three(3) bedroom single family dwelling. After talking
to you and the bank, I have been informed that the residence
on the subject property is a duplex.
Therefore, before we may send an approval to %he bank the
following will need to be completed:

Relocate your sewer system so that the leaching area
is 120 feet fromyour well and the septic tank 80 feet
from your well.

Prior to any construction, obtain a soils 'test and
submit it to this office so that a permit may be issued.
Raise the well casing twelve(12) inches above ground
level.
If there are any further questions, please contact this
