Ref: Proposed Septic Upgrade for Lilac Park Subdivision; Lot 4A, Block 1,
To whom it may concern:
The existing 4 bedroom house is served by a private well and septic system. The drainfield is currently
in the state of failure and needs to be upgraded. We are proposing that a dual deep trench type drainfield
be installed . The integrity of the existing 1250 gallon septic tank will be verified during construction. If
the integrity is found to be poor, we propose that a new 1250 gallon septic tank be installed. Three test
holes where excavated on the property by Micheal E. Anderson on 9/27/1992. The new drainfield will
be designed around the 30 foot radius of Micheal E. Andersons test hole #2. Comments regarding the
design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic system.
4. TOPOGRAPHY: As can be seen on the attached topography drawing the average topography in the
area of the proposed drainfield is a relatively flat. In short there are no slope concerns.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

Dear Mr. Bolles:
The installer of the subject septic system, Harten Construction, placed the
drainfield lines using a lazer. They further state the lines were placed
level with no variation from end to encl. As built elevations were
determined by placing a grade rod in the cteanout at each end of the field
and measuring the elevation difference using a level. The accuracy of the
as built measurement is hampered because of the difficulty of determining
whether the grade rod is actually at the invert of the drain line.
The contractor was notified of the elevation difference and told that
according to Municipal ordinance all systems must be installed level. He
again reiterated his position the system was in fact installed level.
Based upon past experience with absorption systems I do not feel a .36'
variation will adversely affect the operation of the system. The contractor
has advised that all future installations will be installed level for engineer
and MOA approval.

Installation of an on site wastewater system and well for this lot
should have little if any impact on the surrounding properties due to:
1. The subdivision has large lots and there is plenty of room
for water and wastewater systems plus reserve systems on
the lots.
2. Installation of on site systems will not effect the lot drainage
or drainage in the area.
3. When this subdivision was developed excess materials from
road cuts, etc. were placed on this lot and all materials are
well compacted at this time.

SUBJECT: Imported fill.
A site visit by D.N. Bolles and S.E. Oswalt, on this date,
revealed that almost all the available area on both the subject
lots had been covered with imported fill material. The
thickness of the fill varies, however, the eastern extremity
would seem to exceed 15' in depth. This fill material contains
trees and other organic materials, thus rendering it
unacceptable for use by on-site waste water disposal systems.
Another concern is that the native organic mat may not have been
removed and thus act as a conduit for surface and ground water.
Also the storm drain platted along the back of Lots 1 and 2
along the side of Lot 3, discharges through a CMP to the north.
This pipe crosses Lot 3 and may cross over portions of Lots 4
and 5 as well.
Until these items can be taken care of the subject lots cannot
receive approval for on-site well or septic. The storm drain
must be platted and an easement granted,also no septic system
may be allowed within 100' up slope or 30' down slope of the
storm drain. The fill area must be probed and the content and
depth determined.

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being seared by the system.

Ref: Field Investigation of Septic System @ Lilac Park; Lot 4A, Block 1,
Mr. Weddell:
It is our understanding that Mike Blakeslee with A+ Home Services recently pumped your septic tank and
turned the Bull Run valve from the 1992 drainfield to the 2005 drainfield. In the day's prior to this,
Anchorage was experiencing abnormally warm temperatures, which resulted in rapid snowmelt/runoff
throughout the city. Per your comments, at the time of extreme weather event, there was approximately 3'
of water submerging the north end of the 2005 northwest drainfield. It is to our understanding that once
A+ Home Services switched trenches, the water backflowed from the 2005 drainfield, filled the recently
pumped septic tank and backed -up into the house. A+ Home Services was then called back to switch flow
back over to the 1992 drainfield, which immediately stopped the backflow into the house. In fact, water in
the house then flowed to the 1992 drainfield.
Per your request, on 1/28/09 we performed a site investigation. A surveying instrument was used to verify
relative elevation of all three trenches and the liquid levels. We found that the bottoms of the all three
trenches are all within a foot of each other. This leads us to believe that it was not groundwater flowing
from the 2005 trenches back to the house. If it was in fact groundwater, all of the trenches would have
been fall to essentially the same level, and switching the Bull Run valve would not have relieved the
situation. It is our assertion that it was not groundwater encroaching into the drainfields but rather surface
water infiltrating into it, via septic pipes @ the ground surface.
It is our belief that, given the fact that the clean-out and monitoring tube located at the north end of the
northwest trench installed in 2005 were submerged in 3 feet of water, that surface water entered the 2005
drainfield, causing the resultant back-up.

The weather events which occurred to cause the extreme surface runoff and flooding event are rare, but
the impacts are severe. In order to prevent your system from being adversely impacted by surface
infiltration, we recommend the following;
• The monitoring tube @ the north end of the 2005 northwest trench should be filled with silty soil to
within several feet of the ground surface. The upper 2 feet should be 'filled with ebonite chips. The
standpipe should then be sealed with a glued -on cap, and buried below grade.
• The clean-out @ the north end of the 2005 northwest trench should be extended above grade.
• Import and place fill as necessary to prevent future ponding over the drainfields.
• Don't operate on the 2005 trench system if the area appears to be flooded.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given In paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also Issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions In the professional engineer's work.

investigation, based on procedures outlined In the Health Authority Approval Guidelines for this application,

information obtained from the Municipality of Anchorage riles and from my investigation and Inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations In effect at the time of Installation.

Engineer's Comments:
In conducting this evaluation, GEG, Ltd. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines 6 Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily Identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.'
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, Ltd. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.
