A permit issued by this Department for an individual well
and/or on-site sewer system has expired as of December 31,
1986.
Permits are issued on a calendar year basis by authority
of Municipal Ordinance. A new permit must be obtained from
this Department for any well and/or on-site sewer system not
installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this Department for documentation of the installation and to
close the permit.
If a private engineer inspected the installation of the on-site
sewer system the original as-built inspection report (three part
form) must be sent to this office for review and approval, and
for documentation.

Wastewater Disposal Permit for Lot 13 Lake Hill Acres Subdivision
Terms and Conditions of Permit:
This permit shall be considered an operating permit for an innovative
single family household wastewater permit. This initial "evaluation phase"
permit is valid for one year and is subject to the testing schedules and
stipulations contained in the attached on-site sewer application. At the
end of this one year testing period (a shorter period may be allowed upon
the discretion of the department) the performance of the system will be
evaluated. If the department finds that the system has operated
substantially in conformance with the standards outlined in the permit,
a new or "extended phase" operating permit will be issued by this department.
The terms of this extended phase operating permit will, under most
circumstances, prescribe a testing and maintenance schedule that is less
rigorous than prescribed in the evaluation phase. The terms of the extended
phase permit will be established by this department at a future date.
The following conditions pertain to the initial "evaluation phase" operating
permit:
The effluent testing schedules and procedures described in the letter
dated August 5, 1986 and signed by Thomas S. Graham shall be adhered
to (see letter attached).
The treatment plant shall be converted to a holding tank if in the
opinion of this department, the t.esting goals described in the
aforementioned letter are not met. If the treatment plant fails to
meet the conditions of the permit during the experimental phase (one
year or less) the cost of converting the system to a holding tank shall
be covered by a $1,000.00 bond deposited in an interest bearing account
in the First Interstate Bank, O'Malley Branch.
The old well located southwest of the house shall be properly abandoned
in accordance with procedures described in AMC 15.55.060, Water Well
Standards.
A permanent shallow groundwater monitoring well shall be installed
approximately 15 feet to the east of the disposal field. This
monitoring well shall extend to a minimum depth of 8 feet and be
constructed in accordance with standard specifications. The minimum
diameter of the slotted monitor tube shall be 2". The tube should be
capped and provisions to prevent surface water infiltration into the
well should be made.

The separation distance between the base of the absorption bed and
the measured water table shall be a minimum of two feet. Suitable
fill material must be imported if two feet of acceptabl~ soil above the
water table does not exist at the site. This fill material must meet
specifications outlined in AMC 15.65.060.
The 100 foot separation distances required between septic system
component and wells and surface waters have been waived. This waiver

is valid only as long as the conditions of this operating permit
and subsequent operating permits have been met to the satisfaction
of this department. These distances has been waived as described
below:

There is no well log available for the well located on this property, however,
field inspection indicates that the well is at least 120 feet deep (pump
setting) and it appears that the well casing extends to that depth. The
characteristics of this well appear to be similar to those of the well located
on Lot 16 (copy of well log attached).

Dear Mr Robinson:
For some time now Mr. Bruce Erickson of ADEC, your department, and yourself
have been discussing new discharge staDda~ds for effluent i~to ground water
tables, stzrface discharge a~d w~r discharge. EPA guidelines of 30mg/1
Suspended Solids, 30 rog/1 BOD5,' neutral pH, fecal coliform of 20 colonies per
100., plus nitrates of 10 or less along with an accounting of nitrite,
Kjedabal nitrogen, ammonia nitrogen, and dissolved oxygen ~e involved in the
testing procedures. At your request, the duration of the test and the
procedures for testing are outlined below:
TESTING ~
All samples will be collected by an independent contractor to all parties.
All samples will be tested at a laboratory licensed by the State of Alaska.
All tests will be conducted in a manner prescribed by Standard Methods.

To Whom it Nay Concern,
This letter is to outline the steps and procedures to convert
the Rid-Waste Tank to a "Holding Tank" to meet the Municipal
regulations.
There is a service charge for maintnanee for the Rid-Waste
System in operation. The charge for the conversion will be
considered a maintnance call which is already paid in advance
by the home owner.
STEPS:
1. Power is shut off to compressor house.
2. Power line will be disconnected f~m compressor house.
3. The 2" line coming from the pump in the tank will be cut
and capped at compressor house.
4. The 2" line going from compressor house to drainfield or
discharge will be cut and capped at compressor house.
5. All ether air lines and power cords will then be cut at
ground level.
6. The compressor house will then be removed from location.
NOTE: The high level alarm will let the home owner know when
tank is due for service. Alarm wilt sound approximately 3
days (450 gals.) before holding tank is full.

All Rid-Waste Systems on test in the Nunicipality will be
utilizing ultra-violet disinfection. Ultra-violet disinfection
is an option for the Rid-Waste System.

Lot 13 Lake Hill Acres
At approximately 1:45 p.m., November 11, 1986, an inspection of
the subject property was conducted to determine if a health hazard
existed. The investigation revealed the presence of what appeared
to be a tank of some type located near the southeast portion of
the building with a 4" stand-pipe extending about two (2) feet
above ground level. The tank was probed and found to contain
approximately four (4) feet of water. There was a depression over
the tank near the stand-pipe but no visible evidence that the tank
was damaged.
Further investigation revealed approximately twenty (20) feet of
two (2) inches of flex hose coiled at the southeast corner of the
building. The hose ran inside the residence and assumed to connect
to a dish washer, sink or washing machine.
There is no visible sewage overflow onto the ground surface nor is
there any smell of sewage odor in the immediate area.
Conclusion: In my opinion, the present condition of the subject
property's septic system does not constitute a health hazard.

Lot #13, Lake Hill Acres on Mirror Lake is a health hazard
for the following reasons: 1. 22" Water Table-
2. Broken holding tank ~
3. Gray water discharge into Mirror Lake.
The owner, Mr. Allen Dubin, has purchased a Rid Waste Environmental
System and the permit #860373 has been issued at the pen of
S&S Engineering.
Hr. Dubin has applied for Municipality funding in the amount
of about $4,300 for the field upgrade. According to Stan
Hufford, MOA Rehab Director, the program requires the certification
of this health hazard.
Your immediate attention to this matter will enable us to correct
this problem as soon as possible. Thank you in advance for
your cooperation.

Dear Mr. Graham:
Pursuant to the attached Memorandum from Mr. Kennedy to Mr.
Robinson, Lot 13, Lake Hill Acres is not a health hazard.
The discharge of 'gray water' into Mirror Lake does not
constitute a health hazard, but does place the owner into a
position of violation of the Municipal Ordinance and'a citation
may be issued if this practice does not stop. The high ground
water is the qualifying factor for a holding tank. A properly
functioning holding tank will not contaminate ground water.
Further evidence of a health hazard will have to be revealed to
qualify the owner of this lot for Municipal funding under the
Failed Septic System Assistance Program.
