If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

Dear Mr. Smith:
On August 13, 1993 we submitted a plan for a mounded leachfield to be installed in difficult soil
conditions on the above referenced property. The submittal was returned with the requirements
that we monitor test hole #1 through October, that the drain be located 20' distance from the bed
and that the bed profile be dimensioned.
Tiffs work has been accomplished and is attached. We request your re-evaluation of our submittal.

Narrative
Dear Mr. Smith:
We are proposing to replace the holding tank serving the above referenced facility with a
leachfield that utilizes a lift station and built-up leachfield. The water table has been monitored
at 2' below a natural silty gravel soil and we believe an on-site system would be feasible for this
lot with the proper design precautions.
The proposed septic upgrade will have very limited impact on adjacent properties for the
following reasons:
The area has large lots allowing sufficient room for septic sites where soil is
acceptable. Most homes have holding tank facilities.
Immediate neighboring septic systems are all +30' distance.
Reserve space is adequate, due to absorption capacity and combined lot size.
Seasonal drainage will be channeled away from the leachbed by a shallow cut-off
drain. We feel this is necessary due to the potential for spring run-off entering
the sand bed and flooding the system. The cut-off drain is not to intercept ground
water.

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM
LEGAL: LOT 3 & 8A, BLK A, LAKE HILL ACRES #6
A. GENERAL
1. The septic plan is for a single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of Health and State
Department of Environmental Conservation requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the field by the
engineer.
5. All excavations and depths are advisory and are to be verified in the field by the contractor to
meet Municipality of Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any
adjacent multi-family wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer
approval.
8. It is always recommended that a surveyor locate the nearest lot line position and the location of
any easements.
B. SEPTIC TANK
1. Sewer line is to be re-routed, exiting on the west side of house with 10' separation to water line
maintained.
2. Tank is to be Orenco Systems with lift station and 20 OSI 05-HHF pump wired to code by a
licensed electrician, receipt to be provided to engineer.
C. LEACHFIELD
1. The leachfield is to be located adjacent to TH#1 as shown on the site plan.
2. The total depth of the drainfield excavation is to be approximately 7.5', removing only the topsoil
layer and underlying clay layer to natural gravel soil surface shown as GP soil on attached soil
log. A clean, scarified surface is to be provided at GP level.
3. An imported clean sand fill is to be placed to ground surface without mechanical compaction and
leveled to _+0. I'. Sand source is to be approved by engineer prior to construction.
4. The drainfield gravel to a depth of 1' is placed on sand layer and is to be covered with typar
fabric material.
5. Pressure effluent pipe is to be buried at +4' and covered with 2" burial foam from tank to field
location. 1-1/4" PVC effluent leachpipe is to be installed 2" below top of gravel bed layer and
is to contain 1/8" holes spaced at 1.4' facing up with orifice shields installed. Pipe ends are to
contain cleanouts buried just below ground level with rebar markers.
6. A combination of soil and extruded board insulation with a minimum soil depth of 2' is to be
placed over the leachfield and sloped 3:1 beyond leachfield at edges.
7.A cut-off drain is to be constructed to direct seasonal surface run-off away from the leaehbed area.
8. The area over leachfields is to be finish graded to prevent pending of surface water runoff. The
entire leachfield mound is to have 4" topsoil and seeded.
9. The septic tank and leachfield must not be closer than I00' to any existing private well, 150' to
any Class "C" well, or 200 feet to any community well.

GRAVEL ELEVATION = at grade (mounded)
DRA1NFIELD LENGTH = 36'
SOIL RATING= 0.7 GPD/ft2 (filter)
SEPTIC TANK = 1,250 with lift
GRAVEL DEPTH = 1'
DRAINFIELD WIDTH = 18'
BEDROOM CAPACITY = 3
