agreement is enclosed.
A test hole was excavated and a percolation test performed. The approximate location of

the test hole is located on the attached site plan. At the time of excavation 6/16/06, ground
water was not found. After monitoring, ground water was found at 16.5 feet.
We do not anticipate any adverse effects on neighboring wells, septic systems, reserve

construction of this system will not prevent any future development on any of the adjacent

GENERAL:
1. The scope of this project includes the installation of a new 1300 gallon HDPE

2. Construction shall be in accordance with the approved site plan and design

Municipal Development Services Department or ADEC if required, for

3. All standpipes on the septic tank shall extend a minimum of 12 inches above final grade.
4. Septic tanks installed with less than 411. of cover shall be insulated.
5. A foundation cleanout shall be installed one to four feet from the building foundation. In
the line between the tank and the leachfield there shall be two adjacent cleanouts (unless
an effluent pumping system exists within the septic tank). These cleanouts shall be
located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in line,
shall be to clean toward the leachfield. The second cleanout shall be to clean toward the
septic tank.
6. Final grading over the septic tank shall be such that a positive slope exists away from the
septic tank.
ABSORPTION TRENCRMRAINFIELD INSTALLATION:
1. Excavate the proposed trench to the dimensions shown on the design. The bottom of the
excavation shall be within 2 inches of level. If the sidewalis of the excavation become
smeared, they must be raked or scratched (ruffed -up) before gravel (sewer rock)
placement.
2. Once the gravel is installed, the distribution pipe is to be installed level with the
perforations faced downward. Gravel is then to be placed over the distribution pipe to
provide a minimum of 2 inches of cover over the pipe.
3. A silt barrier must be installed between the final gravel layer and the native soil backfill.
Ensure the silt barrier covers the entire gravel surface before placing backfill.
4. Monitor tubes shall be of four (4) inch diameter, installed approximately in the locations
shown on the design, and extend a minimum of 12 inches above final grade. The portion
of the monitoring tube extending through the gravel shall be perforated from the bottom of
the trench to the invert of the distribution pipe. This is equivalent to the effective depth of
the gravel as noted on the design.
5. Backfill over the final gravel layer must not be less than twenty-four (24) inches.
Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The
finish grade over the trench must be mounded to prevent the formation of a depression
after settling.

septic tank manufacturer.

Use of a type of pipe other than listed above must be approved by the inspecting engineer.
3. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical
Company Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Femco, or
equal).
5. A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed
between the final leachfield gravel layer and the native soil backfill.
6. All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3%
passing the #200 sieve.
7. When sand is being used as a filter material, its gradation specifications must conform to
current M.O.A. or D.E.C. requirements, which ever requirement applies.

INSPECTIONS:
Typically there will be a minimum of three (3) inspections required during the installation of the
wastewater disposal system. These inspections will occur as follows:
I. The first inspection must be conducted after the excavation of ditches, pits,
trenches, or beds and before the installation of any gravel. A septic tank may be
set in place, but may not be backfilled before this inspection.
2. The second inspection must be conducted after the placement of the silt barrier,
gravel, distribution lines, standpipes, cleanouts, and insulation, but before the
placement of any other backfill.
3. The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required. Especially with the installation of
multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer
is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre -
construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or
control in any way the contractors activities.
The owner shall contract with the contractor to perform the work outlined in these specifications
and plans and in accordance with the attached M.O.A. permit. There will be no contractual
arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be
the owner's representative and will inspect the work as stated above to document the contractors
activities. Final acceptance of the contractors work rests with the owner and the M.O.A.
S & S Engineering shall have no liability to the owner or to others for acts or omissions of the
contractor or any other persons performing work on this project or the failure of the contractor to
carry out the work in accordance with these construction documents. S & S Engineering's
inspecting engineer will not be responsible for the construction means, methods, techniques,
sequence, procedures or the safety precautions incident to this project.

EASEMENT AGREEMENT C C
The undersigned, in considcratio of mutual a reement set forth below, grant, convey and affirm
the following easement, effective this day of Q ,u P 2006, and agree to the conditions and
restrictions as follows:
I, Velva Fowler, hereinafter referred to as grantor, owners of certain real property herein after
referred to as "Lot 4; Lake Hill Acres Subdivision NI". According to the official plat thereof filed in the
records of ANCHORAGE RECORDING DISTRICT, Third Judicial District, State of Alaska, do hereby
grant to Velva Fowler, hereinafter referred to as the grantee, as owner of certain real property hereinafter
referred to as "Lot 5; Lake Hill Acres Subdivision 01" according to the official plat thereof filed in the
records of ANCHORAGE RECORDING DISTRICT, Third Judicial District, State of Alaska.
An easement for an on-site wastewater disposal system to be located on Lot 4; Lake hill Acres Subdivision
N I, together with right to maintain the on-site wastewater disposal system to include reasonable egress and
ingress for pumping the septic tank and maintenance of the absorption area. The easement to be effective
upon the signing of this agreement by the parties.
I. It is understood and agreed that sewer service to Lot 5; Lake Hill Acres Subdivision HI is to be
provided by connection beginning on Lot 5 and extending onto Lot 4. It is further agreed that the
cost of maintenance of the in-place sewer lines shall be born by the grantee.
2. There shall be no charge for the use of the on-site wastewater disposal facilities so long as this
easement is in affect unless a charge is arranged between the parties of their heirs or assigns or
successors in interest by separate written agreement.
3. Grantor makes no warranty as to the adequacy of the wastewater disposal system and shall not be
liable for any damages caused by the system failing to work adequately.
4. The grantor further agrees that the grantee shall have full, free and uninterrupted use of said
easement for reasonable domestic use as long as such use docs not create an unreasonable, adverse
impact upon the grantor.
5. Failure to make use of the on-site wastewater disposal system herein described shall not be deemed
abandonment of the said casement.
6. The grantee further agrees to bear all cost, if any, for the operation, maintenance and/or
abandonment of the on-site wastewater disposal system and/or easement.
7. This easement from grantor to grantee shall be binding upon and inure to the benefit of the parties
hereto, their heirs, legal representative, successors and assigns, and shall run with the land.

Water Well Advisory
Certificate of On -Site Systems Approval (COSA) # 131310
During a recent COSA on-site inspection and test of the potable water
supply well on Block , Lot 5 of Lake Hill Acres #1 subdivision, the
well's productivity was determined to be 0.5 gallons per minute. The
minimum well productivity required by this Department (AMC 15.55) for a
4 -bedroom residence is 0.42 gallons per minute. Although the subject well
currently exceeds this minimum requirement, all parties concerned are
advised that the production capacity of the well may fluctuate. Restriction
of non-critical water uses such as washing cars and watering lawns and
gardens may be required.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.
