To Whom it may concern:
This is a request for a septic system permit on the above referenced lot. The owner is planning for as
small kitchen addition and needs to move the septic system. A test hole was excavated and found silty
gravel for the full 18 foot depth. The perc was 24 minutes per inch with no water observed during
excavation or after the 7 day monitoring period. The lot slopes down (south) toward the back at about 1
to 2 percent almost flat.
None of the neighboring lots will be impacted by this installation. No slopes greater than 25 percent
existing within 200 feet of the proposed system.

the bottom of the excavation(in feet). There is no set width for trenches.
The gravel depth is the minimum depth of gravel between the outfall pipe and
the bottom of the excavation(in feet).

installation inspections of any wells adjacent to this property and the number
of residences that the well will serve.

Backfilling of any system without final inspection and approva~ by this department.

Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days Of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.

SUBJECT: Claim on Lot 9 Lake Hill Acres Subdivision - Darlene Street
On two(2) occasions our staff made reference to an inspection
on Lot 9; both times only the legal description was used and
no site location was checked. The filed inspection, done by
Jim Roberts in 1981, was the only Lot 9 we showed. Apparently
the listing agent had not seen the lot in question (on Darlene Street)
and was unaware that it was vacant.
This office never wrote a health authority approval based on the
1981 final inspection. However, a copy of the final was made for
the agent. Again, no one noticed that it was located on the Old
Glenn Highway. The listing agent also never requested a formal
health authority approval.
In May, 1983, a final inspection was performed on Lot 9 of the
original subdivision by Robert Shafer, P.E. He has since verified
the location on Darlene Street. In August a health authority
approval was done on that lot.
Although the original information on file did not show a complete
legal description, it was based on information supplied by the
soil tester and the owner. In addition, the first inspection
clearly shows the site location as the Glenn Highway that should
have been checked against the legal by the listing agent, buyer
and seller. I recommend the Municipality not pay this claim.
