Record Drawing / Final Installation of System
Ladies and Gentlemen:
I am writing in regards to the above referenced property and questions that arose during the Record Drawing
review of the system that was installed in August of 2015 and had the record drawings for the installation
submitted in February of 2016.
The previously existing system had a waiver for separation of the original holding tanks to the well on the subject
property of 56'. The 2015 submittal for upgrading that system included separation waiver requests for both the
tank and the absorption fields to the existing wells on the subject property along with the wells on L11 and L13.
Due to conditions on the property and improvements that had been made, placing the new Advantex AWWTS in
the proposed location proved unfeasible. As such the tank was installed in a location near the previous holding
tanks but outside the distance of the original separation waiver ('"56'). The original drawing had reflected a
"Future Addition" for the dwelling, but the footprint on that plan was erroneous and should have shown a five-
foot separation from the vicinity of the now decommissioned holding tanks.
Also of note is the system that was installed for this permit is an Advantex AWWTS. The new code only requires a
50' separation distance for both tank and absorption fields to a well provided the installed system is an AWWTS.
As previously stated, the installed system is an AWWTS, and as reflected on the included Record Drawing is outside
of the 50' separation distance for all wells in the area. I request a waiver to allow this installation to be allowed
under the proposed code.

Septic System Permit Request
Design Narrative
This is a design narrative for a permit to install a new well and septic system to be issued for this property.
The proposed systems will serve an existing four (4) bedroom house. Currently the lot is developed,, and
the house is served by a holding tank. The proposed system will utilize a 12508 S.T.E.P. tank, that pumps
into a 500g lift station with an AX20 pod where recirculation and treatment will occur, with a drain field
based on CAT III. This lot and the surrounding developed lots are served by private wells. The well on this
lot (Lot 12) is located 68.4' from the proposed drain field and 74.8' from the proposed 12508 S.T.E.P. tank.
the well on Lot 13 is located 85.1' from the proposed drain field and over 100' from the proposed tank
and lift station. See waiver request. The remaining Surrounding wells are located over 100' from the
proposed septic system.
1. Soils. One test hole was performed in the vicinity of this system by Pannone Engineering Services in
May of 2015, and groundwater was monitored for at least seven days. Ground water was not
observed to a depth of 13.0' below the surface in the test hole monitor tube in April of 2015. Bedrock
was not encountered in the test hole. It is my opinion, based on the results of the percolation tests
and overall soils appearance; an application rate of 6 gallons/day/square feet should be used for a
CATIII wastewater system in the area of these test holes.
2. Soil Absorption System Design.

3. Surface Water: There is no surface water within 100 feet of the proposed septic tank and drain field.
The proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage
ditches.
4. Topography: The existing topography slopes from the northwest to the southeast at
approximately 3%-5% in the area of the septic system. There are no steep slopes within 50' of the
proposed drain field. The proposed drain field will maintain 50' separation all steep slopes.
5. Drawing Markings: The Drawings are marked "For MoA Review Only". When written notification that
the review is complete and that there are no further comments is received from MoA On -Site
Department, the note will be removed and "Issued for Construction" drawings will be issued.
The proposed installation will not affect the future development of this or the surrounding lots.

Waiver Request
Ladies and Gentlemen:
I am writing to request a separation distance waiver between the existing Private well on Lake Ridge
Terrace, Block 2, Lot 12 and the proposed septic tank, lift station, and drain field on that lot from 100' to
68'. 1 am also requesting a waiver from the well on Lake Ridge Terrace, Block 2, Lot 13 to the drain field
on Lot 12 from 100' to 85'.
PES was unable to locate a well log for this property or Lot 13. The static water level of the well on lot 13
was reported as 59 feet below ground level in August of 2014, and for reference the well on lot 11 was
found to have a static water level of 36 feet, both at approximately the same elevation as the surface of
the lake. The ground water in this area typically flows from the west to the east. The well is located 68.4
feet southeast of the proposed drain field and 74.8 feet south of the proposed S.T.E.P. tank. The
proposed tank will be further from the well than the existing holding tanks, and the CAT III wastewater
system will improve the quality of the effluent leaving the drainfield.
As outlined under 18 AAC 80.020 and 18 AAC 72.021(a), I have calculated the following points:

THIS MAINTENANCE AND REPAIR AGREEMENT made and entered into as of this
1 Day of JULY of 2015 by and between Joseph E. Stancil Jp,. and
Doralee A. Stancil
herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Maintenance and Repair Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. Municipality grants permission to
Owner to utilize and operate an Advanced Wastewater Treatment System
(AW WTS), described as

located at (legal description).

2. Definitions.
Alteration. Any change to the design or function of an AW WTS that
includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design.
Certificate of On -Site Systems Approval (COSA). An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with Anchorage
Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that
the systems are adequate for the homes that they support and meet the
codes that were in place at the time of system construction.
Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AW WTS that were included
in the original design which would allow the AW WTS to continue to
perform as designed.
Permit. An On -Site Wastewater Disposal Permit as required by AMC
15.65 to construct and operate an AWWTS.
3. Term. The term of this Maintenance and Repair Agreement shall begin on the
daze of approval by the Municipality to operate the installed system or issuance of a
COSA, and shall continue while the AW WTS is in use or is operational or until the
property is sold or title is transferred by the owner and a new COSA is issued to the
new owner or transferee of the property.
4. Alterations. Installation and Removal of Additional Equipment. Prior to
performing any alterations to an AWWTS, the owner agrees to obtain -an On-site
Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65.
5. Maintenance and Repairs.
A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall
maintain their AW WTS in a satisfactory condition capable of producing treated
septic effluent in accordance with the equipment's approval for operation in the
Municipality. The owner shall enter into a service agreement with an AW WTS
service and maintenance provider =roved by the municipality and the
manufacturer of the AW WTS for the entire term of the AW WTS. In addition, it
shall be the responsibility of the Owner during the term of this Maintenance and
Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay
for any and all: (1) repair(s}, (2) maintenance, (3) adjustment(s), (4) replacement
costs, and (5) inspection costs.
B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules
and orders for the AW WTS.
C. Upon request by the Municipality, the owner agrees to provide the Municipality a
written schedule of routine maintenance and repairs which have been performed
on the system. When a record of maintenance is documented and maintained by
the system vendor, the owner agrees to allow the Municipality access to this
information.
D. Owner acknowledges that the fine for failing to maintain and repair an AW WTS
may be assessed in accordance with AMC 14.60 for improper discharge.
E. Owner agrees that only maintenance and repair personnel approved by the
Municipality will inspect and make any necessary maintenance, repairs or
permitted alterations to the system.
F. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
G. Owner agrees that any sale or transfer of title of the property will act occur
without a new Certificate of On -Site Systems Approval.

8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance
and Repair Agreement shall be brought in the Superior Court for the Third Judicial
District of the State of Alaska at Anchorage. The laws of the State of Alaska shall
govern the rights and obligations of the parties under this Maintenance and Repair
Agreement.
9. Severability. Any provisions of this Maintenance and Repair Agreement decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining
provisions of the Maintenance and Repair Agreement.

Dear Mr, Merrill:
On May 13, 1983 Larry Montgomery and I inspected the sewer system
located on lot 12, block 2, Lake Ridge Terrace, Eagle River for the
purpose of determining where a soils test could be made for a
replacement system.
We determined that even if the well were moved, it would be impossible
to get an on-site system on this lot.
Therefore, we recommend that a holding tank be installed on this lot.

A sewer system adequacy test was performed on the system located on the
referenced property, as you requested. The septic tank was pumped and verified
to have a capacity of approximately 1,000 gallons. The seepage pit was charged
with 900 gallons of fresh water, and after a period of 24 hours, approximately
300 gallons had percolated out of the crib.
It can be concluded from this test that the waste -water disposal system serving
the residence located on this property is currently functioning adequately for
approximately two bedrooms. The septic tank appears to be adequate for three
bedrooms; however, the .seepage pit did not provide sufficient percolation for
more than two bedrooms. This system, though appearing to be functioning
adequately, cannot be guaranteed against further deterioration.
