STATEMENT OF INSPECTION BY ENGINEER As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at the time of installation. In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed above. Pannone, Principal Registered Professional Engineer E-mail: steve@panengak.com April 12, 2021 Subject: Lake Ridge Terrace S/D Block 2 Lots 13 Drainfield to Foundation Waiver Request I am writing to request a Separation Distance Waiver be issued for the existing wastewater disposal field on Lot 13 and the existing detached garage. The foundation wall of the garage is located 13 inches from the cleanout (center of a four foot diameter concrete rings). The drainfield was constructed in 1971. It is a crib, with concrete rings and concrete top. The concrete rings are surrounded by sewer gravel. The outside dimensions are 12 foot wide by 27 foot long, with six feet of effective depth. The top of the crib is 82 inches below ground. A clean-out/Monitor Tubes was found when the snow receded. The Clean-out is approximately 15 feet from the Crib's clean-out. It appears to be a monitor tube that was installed at the eastern edge of the drain field. It was found to be d ry. The garage was constructed in approximately 1980. It is a single story garage with a truss roof and standard spread footing. The bottom of the footings are 48 inches below ground level. Based on the dimensions, the spread footer is sitting directly on top of the concrete rings and lid. The side that is over the crib is the gable end. The loads were analyzed and found to be negligent, or about the same as the soil load. The drainfield portion is under the concrete slab and no settlement has occurred over the 30 plus years it has been in operation. In my opinion, this waiver request meets the above criteria for approval and does not constitute risk to health, and a waiver to the minimum separation distance should be granted. The existing septic system was installed in 1971 and consists of a 1000 gallon concrete septic tank & a concrete crib style absorption field. A surveyed property as -built located the crib monitors approximately 5 feet from the southern property line. The property to the south is developed with the existing septic system approximately 10+ feet from the property line. The system has operated in this configuration for 43 year with no adverse impacts to either property. The current configuration of this septic system will not prevent wells and septic systems from being installed on the adjacent lots and no adverse impacts are amiciapated. In 1985 a detached garage was built. It appears that there may only be 4 feet of separation between the edge of the septic tank and the garage foundation. The septic tank is a cylindrical concrete tank is not impacted by the garage. The septic system has function satisfactorily in this configuration for over 30 years with no adverse impacts. The absorption field cleanouts are approximately 90' from the edge of the lake. In 1971 the minimum required distance between and absorption field and a surface water was 25'.
