2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)

shall be either:

The property has recently passed a septic adequacy test on the existing bed and the owner has
requested we obtain a septic permit to upgrade the existing aged steel septic tank on the above
referenced lot. We propose to remove and decommission the existing septic tank and install a
new 1250-gallon HDPE tank in the same location to serve the existing 4-bedroom residence.
With this upgrade, we request to maintain the existing waivers to the subject and adjacent wells.
The proposed upgrade with a HDPE tank will improve the protection of these wells. Also, the
documented nitrate levels of both these properties are shown to be non detect (ND). The design
will not impact any of the neighboring properties. Please contact us if you have any questions.

depth of a trench or pit is the distance between the surface of the ground and
the bottom of the excavation(in feet). There is no set width for trenches.
The gravel depth is the minim~ depth of gravel between the outfall pipe and
the bottom of the excavation(in feet).

Minim~ distance between a well and any on-site sewage disposal system is 100 feet~

is 25 feet and to a co--unity sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.

available to insure proper installation.

Comments: This investigation was completed in compliance with MOA guidelines, regulations,
and best industry practices / methods. The assessment of the condition of the well and septic
applies only to the conditions as of the day tested. The flow and absorption rates may change
due to subsurface conditions that may not be observed from the surface, changes in land use,
local soil characteristics, groundwater levels that may fluctuate during the year, quality of
construction (workmanship & materials), the water usage of the family being served by the
