Dear Mr. Robinson,
At your request, we are providing the following data on the
above referenced septic permit submittal.
The home by your records was a duplex dwelling, but has
recently been converted to single family usage. This was verified
by having a professional appraiser evaluate the home with letter
attached verifying single family residence status.
The length of the proposed leach area is set at 50' total
length. A test hole, however, was dug to a depth of 15' down slope
of our proposed leachfield site. This was dug to verify depth to
groundwater below the existing system in June prior to our recent
submittal. A monitor tube was installed with no ground water noted.
A perc test of the soil at 6' depth had a result of 155 S.E. per
bedroom rating.
Neighboring septic system measurements were added to the site
plan in relation to on lot well. The neighboring Lot 5 septic tank
as it turns out is 951, to cleanout, from our well. We wish to
apply our previous 80' waiver to this tank with the additional
information that the tank is located down slope from the well with
a vertical distance of 10-151. The neighbors home is located
directly between the tank and well location.

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM
LEGAL- LAKE RIDGE TERRACE, LOT 6A. BLOCK 0
A. GENERAL
1. The well and septic plan are for a single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of
Health and state Department Of Environmental Conservation
requirements.
4. All sail tests are advisory to the design and are to be verified or
modified in the field by the engineer.
5. All excavations and depths are advisory and are to be verified or
modified in the field by the contractor to meet Municipality of
Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits
or easements and to locate any adjacent multi -family wells.
7. The excavation is to be exactly in the area shown on the site plan.
any deviation requires engineer approval.
Es. It is always recommended that a surveyor locate the nearest lot line
position and the location of any easements.
B. TRENCH
1. The trench is to follow the natural land contour to maintain uniform
total depth of the trench bottom.
2. The bottom of the trench shall be level, plus or minus 1.5".
3. The total depth of the trench excavation is not to exceed 12' at anv
point.
4. The trench gravel is to be covered with typar fabric material.
5. soil or combination of soil and extruded board insulation to a depth
of 4' or equivalent is to be placed over the leachfield.
6. The area over the trench is to be finish graded to prevent ponding
of surface water runoff.
7. The septic tank and leachfield must not be closer than 100' to any
existing private well, 150' to any Class "C" well, or 200 feet to
any community well..

Dear Mr. Butera:
On October 16, 1989 this office conducted a routine field
inspection of the new on-site wastewater disposal system which
serves the subject property. Based on our inspection, it was
visually apparent that the structure on the property is a
duplex. It was noted that the structure on adjacent Lot 5 is
also a duplex.
Municipal property appraisal records indicate that Lot 6A and
Lot 5 are zoned R2A (duplex) and that the structures which
occupy these lots are presently functioning as duplexes. We
also understand that the subject property is currently being
marketed as a duplex.
As you are aware this office does not have authority to issue
permits or waivers for on-site water and wastewater systems
which serve more than a single family residence. Therefore,
please be advised that the separation distance waiver, dated
August 28, 1989 and the on-site sewer permit, dated August 24,
1989 are rescinded and no longer valid. The waiver and permit
were both issued based on information you prepared and submitted
to this office.

Given the obvious visual appearance of both structures being
duplexes, we question how you concluded that the structures on
Lot 6A and Lot 5 were strictly single family residences. We
assume that this mistake was not intentional. However, we would
like to remind you that this office takes exception to any
misrepresentation for the purposes of obtaining waivers, permits
or Health Authority Approvals for properties which are not
strictly single family residences.
If it is intended that the referenced properties remain as
duplexes, we suggest that you obtain any required permits or
waivers from the Alaska Department of Environmental Conservation.

regarding the status of our septic permit on Lot 6A, Block 8,
Lake Ridge Terrace. We were very upset by your letter and your
implication that we were trying to misrepresent our intentions.
We inherited the residence last year when my father passed away.
My family has spent over $10,000 trying to bring the home"up to
current municipal code. We proceeded in good faith on the permit
issued by the Municipality and invested considerable time and
money on the septic system replacement. We hired Eagle River
Engineering to put in the new system for a single family
residence and Lou Butera did a very good job for us. ,
As you mentioned, this lot is zoned R2A and was_a duplex unit in

As requested, I have conducted an inspection of the above
referenced property to determine if the improvements situated
thereon constitute a single or two-family residence. The date of
inspection is November 9, 1989. An exterior and interior
inspection was conducted. I am aware that the sewer system was
recently upgraded.
The improvements consist of two structures, the primary building
and detached garage. The main structure is a complete residence,
including two bedrooms, two baths, kitchen and dining facilities,
etc. It is my understanding that at one time there was a small
"studio -type" dwelling over the garage occupied by the former
owner. As of the date of my inspection, there are no kitchen
appliances installed, and this area is considered to be a separate
bedroom and bathroom that would not be capable of being rented out
as a separate unit.
Therefore, it is my opinion that the Subject property consists of
a three bedroom, three bath single family residence with detached
garage.

Dear Mr. Robinson,
We had previously submitted a wavier application for your
review regarding the above referenced lot. After your review, you
requested additional data on well logs, soil strata and water
levels in neighboring wells to determine hydraulic gradient.
I have searched files at the M.O.A. and also at the State
D.G.G.S. to provide as much information as is available. The well
logs are plotted to vertical scale on the attached drawing. Wells
are positioned vertically as determined from a 4' Municipal Contour
Interval Map of the area, adjusted to U.S.C. $ G.S., M.S.L. 1972.
Horizontal distance, where applicable are shown between specific
wells. The well position map is included for your reference.
The well logs, being from different drillers, may contain
different. descriptions of the same or similar materials. The area
is located adjacent to a fault zone which results in dissimilar
soil or rock strata.
Static water levels are as reported on well logs or as
reported by the D.G.G.S. in field survey. We have taken static
levels where possible.
I believe our best data would come from the two well logs #5
and #7 on drawing, that are drilled by the same driller (Sullivan
Water Wells). These logs show a possible ground water table
gradient toward the lake or East at 5%. The gradient of the shale
layers is indicated as possibly being East at appropriate 15%. It
can be assumed that any near surface water table will be located
on top of the relatively impermeable sandstone or shale layer and
follow the gradient of this layer in the direction of slope.
Dealing with the limited data available, our best reasonable
assumption of the hydraulic gradient would be somewhere between 5-
10% allowing a minimum point value of 5.
I have attached available soil logs in the area to depths of
18' with no ground water table noted.

It should also be noted that placing a new leachfield and tank
Will create a situation where the septic effluent will be more
effectively treated in known soil conditions. This is an
improvement over the existing undocumented system that is presently
in use.

Dear Mr. Roth,
At your request of our client, Mr. James Krafft, we are
submitting a request for !wavier of separation, distance well to
septic leach,field/tank and trench .to lot line separation distances.
The existing system was installed and approved for duplex
dwelling with the well drilled 9/21/79. The home is being converted
into a single family dwelling for resale and the existing seepage
pit has failed adequacy testing. The only available septic area
that is located 100' from the lake is as shown on the site plan
requiring a waiver to allow lesser separation to the well. This
is the only option for onsite disposal. A letter showing prior
approval by Kyle Cherry dated 8/18/79 is attached.
The existing well is drilled to a total depth of 260' (log
attached), with 81'-8" casing, thereafter penetrating into shale
and rock ledge layers until reaching the indicated water bearing
strata at 246'-255' elevations in a sand stone aquifer. Our
measured static water level is at 53' below ground surface. The
ground surface slopes from the well towards the septic system and
natural drainage patterns would flow away from the well toward the
lake at a +25% average gradient. The subject home is physically
located directly between the well and septic system. A test hole
was excavated 15' down gradient from the existing seepage pit in
preparation for a pit to lake wavier, with no leachate present.
Soil conditions are GM type soil approximate silt content 10-15%
with perc rating of 150 S.F. per bedroom. No ground water table was
encountered to a depth of 11'. Neighbor soil + well info is
attached.
The proposed 38' leachfield is based on 3 bedroom, 150 soil,
8' total depth, with 6' of gravel. A new test hole will be
excavated for permit submittal.

Engineer's Comments: This investigation was completed in compliance with ADEC and MOA regulations. The
assessment of the condition of the well and septic applies only to the conditions as of the day tested. The flow
and absorption rates may change due to subsurface conditions that may not be observed from the surface,
changes inland use, local soil characteristics, groundwater levels that may fluctuate during the year and the
water usage of the family being served by the system. The operational life of all well and are
subject to these various and dynamic characteristics and are outside the control of the
evaluator of the well and septic system.

On 10/5/11 the subject water well was inspected in coordination with Jeff Poet ~ MOA
Development Services. The water well is located directly in front of the garage door opening-
this createS a hazard. The water well was constructed below grade in sealed manhole. The casing
extends above the well-pit bottom. A sanitary seal is properly installed and a daylight drain runs
from the well pit to a lower elevation down-slope.
This water well should function successfully in the present location and will not be damaged by
vehicle traffic. The water well can be accessed by a pry bar to raise the sealed manhole lid.
