2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)

2. WRO10051 waived tank to well separation. This waiver is renewed with this permit approval and assigned
OSV251043 as a new number. This waiver has been in place now for 24+ years and water quality has not
been affected as seen by the 2025 water samples provided with this permit.

Tank Replacement
To whom it may concern,
The owner of the aforementioned lot has a 3-bedroom home on the property with a septic
system that is nearing the end of its life. The current system consists of a steel 1,000-gallon
tank (installed in 1997, an undocumented system), a 700-gallon steel lift station, and a
pressurized 90’ deep trench (uphill of the tank with all monitoring tubes and cleanouts verified
during the September 2025 COSA) being utilized. Due to the age and uncertainty surrounding
the installation of the tank, the owner has elected to pursue a tank replacement. The
replacement tank shall be a 1,500-gallon polyethylene MOA-approved tank with a lift station
incorporated, sizing the system to support a 4-bedroom home. During the COSA, the following
deficiencies were noted, and waivers are being requested.
1. Lift Station to Well Waiver: Similar to the septic tank waiver in the 2001 Permit, the new
septic tank requires a waiver for being approximately 88’ from the well. We are
requesting that a similar logical application for the septic tank to well be applied to the lift
station as well. As stated in that application, effluent is treated well within the native
substrate in a short distance, and the homeowners daylight basement would serve as an
obstacle for the path of flow.
2. Lift Station to Surface Water (Fire Lake) The new septic tank is approximately 96’ from
the surface water (Fire Lake). However, the current residential structure is obstructing
the path of overland flow. This makes the shortest path approximately 110’ of surface
travel, going around the northeast portion of the home.
3. Tank to Foundation: Please see the second sheet of the replacement plans for the tank
for a diagram of the proposed new tank placement. The tank is expected to be shifted to
the north, allowing for more distance between the bottom of the home’s foundation and
the tank. If installed to these specifications, the tank should be entirely outside a 1:1 load
distribution prism from the foundation.
4. Drainfield under Driveway Waiver: Using measured values collected from the 2025
COSA, the following depths for the monitoring tubes are as follows:

The effective depth of the trench is listed at 8.0 feet, per the 2001 inspection report. Both
MT2 and MT3, the closest monitoring tubes to the driveway, had one foot of stick-up.

This would put the depth of the pressurized distribution pipe at approximately 7’ below
the ground surface at MT2 and approximately 8.3’ below the ground surface near MT3.
Additionally, there is insulation installed over the drainfield. Given a minimum of six feet
of cover and insulation board placed over the drainfield, the load distribution of the traffic
activity dissipates prior to reaching the PVC pipe. This makes it a minimal concern for
the system.
Granting a new tank replacement and these waivers should not cause any eminent health
hazard concerns or adverse effects to the homeowners or neighboring lots. Additionally, the
home is currently not occupied and is not anticipated to be occupied before the replacement of
the septic tank.

SPECIFICATIONS
FOR
ON SITE WASTEWATER DISPOSAL SYSTEM

LAKE RIDGE TERRACE
Section A — General
1. Site plans and cross section in Section 1 are included in this Specification.
2. All materials and workmanship shall be of good quality and in accordance with
minimum requirements of AMC 15.65, Wastewater Disposal.
3. Soil conditions shall be verified by the contractor at the time of construction. The
engineer shall be notified of any changes.
4. The existing cribbing seepage pit is to be abandoned in place.
Section B — Materials
B.l Septic Tank and Lift Station
1. The septic tank, lift station and piping shall be in accordance with the Standard
Specifications for Component Parts and Material Used in Construction of On Site
Waste Water Disposal Systems dated February, 1999 by the Municipality of
Anchorage, Department of Health and Human Services.
2. The septic tank and t; ft station shall have a rnirAirt to combined storage capacity
of 1250 gallons for a 4 bedroom home, plus 250 gallons for the lift station.
3. The downstream end of the existing 1000 gallon septic tank shall be excavated
and inspected for integrity, and replaced only if necessary,
4. If the existing septic tank is adequate, a 500 gallon lift station shall be installed at
the location shown in Figure 2.
5. All new components shall be Anchorage Tank& Welding Inc. or approved equal.
6. All lift station components including switches, alarms, and wiring shall be in
accordance with manufacturer's specifications.
B.2 Piping
1. Effluent distribution piping shall be in accordance with lift station manufacturer's
recommendations.
2. Monitoring standpipes shall be L5 inch (minimum nominal diameter) ASTM
D3034 PVC. End caps shall be included and the bottom 7 feet shall be slotted or
perforated.
Lot 7, Block 8, Lake Ridge Terrace
B.3 Backfdl, Insulation and Geotextile
1. Drain gravel shall be 0.5 inch to 2.5 inch, screened, with a maximum of 3%
passing the #200 sieve.
2. Insulation shall be in accordance with the Standard Specifications for Component
Parts and Material Used in Construction of On Site Waste Water Disposal
Systems dated February, 1999 by the Municipality of Anchorage, Department of
Health and Human Services.
3. Geotextile shall be a woven, polyester fabric, Amoco 2004, supplied by owner.
Section C — Execution
CA Septic Tank and Lift Station
l . All excavations for subsurface features are to be in the exact areas shown on the
plans. Any deviations must be approved in advance by the engineer.
2. All loose soil shall be removed from the excavation and the septic tank and lift
station shall be placed on undisturbed soil.
3. The bottom of the excavation shall be prepared to properly bed the tanks and
graded in accordance with manufacturer's recommendations.
C.2 Piping
1. Effluent distribution pipe installed within the trench shall be installed level,
perforated in accordance with the lift station manufacturer's
recommendations, and installed with the pPrfnratinnc tintx u.
2. Effluent distribution piping between the lift station and the trench shall be non
perforated.
3. Pipe connecting the existing septic tank to the lift station add -on shall be
sealed and installed in accordance with lift station manufacturer's
recommendations.
4. All effluent distribution piping shall be properly bedded to prevent excessive
deflection or damage upon backfill.
5. One monitoring standpipe shall be installed vertical at each end of the trench
and at the bend, for a total of three monitoring standpipes. The standpipe shall
be installed to the bottom of the trench and in such a manner as to limit
deflection upon backfill.
6. At least 18 inches of stickup shall be provided above finish grade for all
monitoring tubes or cleanouts.
7. A 4 inch minimum nominal diameter steel protective casing shall be installed
over each monitoring tube
Lot 7, Block 8, Lake Ridge Terrace
C.3 Trench Excavation
i. The total surface area of the trench under the effluent line shall be
approximately 1.333 square feet.
2. The bottom of the trench shall be level, plus or minus 1.3 inches. The design
elevation of the trench bottom is 75.8 feet based on the site datum described
on Figure 1. The lowest elevation of the trench shall not be below 75.5 feet.
3. The total depth of the trench shall range from approximately 9.9 feet to 17.2
feet.
4. The sidewalls of the trench shall be inspected for slick or compacted surfaces,
and scarified with a steel rake if necessary.
C.4 Backfill, Geotextile, and Insulation
1. Drain gravel shall be installed to the minimum depth necessary to obtain the
required surface area specified in the previous section. An additional 2 inches
of drain gravel shall be placed after the effluent pipe is installed.
2. Drain gravel shall be installed in such a manner as to prevent contamination
with excavated material.
3. Drain gravel shall be installed in 12 to 18 inch lifts and tamped with the
excavator bucket to reduce settlement.
4. Owner supplied, woven geotextile fabric shall be placed over the entire length
and width of the drain gravel.
5. A minimum of 24 inches of native material will be backfilled over the drain
gravel. Four inches of insulation shall be placed in all areas with less than 36
inches of cover.
6. The area over the trenches is to be slightly mounded and finish graded to
prevent ponding of surface water.
Section D — Quality Assurance
1. The on site disposal system construction shall be inspected and documented in
accordance with AMC 15,65.150.
2. A minimum of two inspections shall be conducted.
3. At least on inspection shall occur after the trench is excavated and prior to the
placement of any backfill. If the trench cannot be excavated in its entirety
prior to backfill, then additional inspections will be conducted as necessary.
No trench backfill shall be placed unless the trench has been inspected by the
engineer.
4. An additional inspection shall occur after the placement of the effluent piping,
2 inch gravel cover, and geotextile and prior to the placement of any
insulation or soil cover.
5. An additional inspection shall occur as necessary to observe the condition of
the existing septic tank and to inspect the installation of the lift station add on.
6. Electric components shall be inspected by a licensed electrician.

Section 4 Narrative and Request for Waiver
The proposed design consists of a deep trench subsurface disposal field with a 1000
gallon septic tank and a 500 gallon lift station added on to the existing tank. The
proposed system is being designed for a 4 bedroom home and replaces the cribbing
seepage pit previously installed in accordance with municipal regulations current at that
time.
The Lake Ridge Terrace subdivision is located in Eagle River, southeast of the Knik Arm
near the transition between the Upper Cook Inlet-Susitna Lowlands and the Matanuska
Valley. Lower Fire Lake, which adjoins the site, is located in a glacial trough cut into
bedrock. The slopes east of the lake at the lower elevations are lateral moraine from the
Naptowne Glaciation period'. The area west of the lake near the site is believed to be
dominated by relatively shallow deposits of glacial outwash and till overlying bedrock.
Boring logs for water wells on Lot 5, Lot 6A, Lot 7 and Lot 8 were reviewed, as well as a
test pit log from the Lot 6A septic system design. The site water well located on the west
edge of the site below improvements shows shale bedrock near 60 feet below ground
surface (bgs) overlain by interbedded gravels and "clay". Soils within 30 feet of the
surface are described as "medium gravel and some clay" with a static water level 24 feet
bgs. The total depth of the well is 210 feet bgs. The water well log on adjacent Lot 8 is
within 35 feet at a similar elevation and is consistent. The water well for Lot 6A is
located approximately 90 feet from the Lot 7 line and at least 115 feet from the proposed
deep trench drain field. The well log shows clay and gravel to 18 feet bgs and
interbedded sandstone, shale and "rock ledges" to a depth of 260 feet bgs. The water
well log for Lot 5 is over 110 feet from the Lot 7 line and shows "clay sand & gravel" to
a total depth of 196 feet bgs. The Lot 6A test pit located near the north corner of Lot 7
shows "dense, dry, sandy gravel with silt (GM)" to a depth of 16 feet, with a percolation
rate of 32 minutes per inch. Locations of Lot 5, 6A, 7 and 8 water wells, and the Lot 6A
test pit are shown on Figure 1.
This information combined with the on site geotechnical investigation confirms that the
area is dominated by glacial till, which can be expected to exhibit low permeabilities.
Because of this characteristic and because minimum separation distances for the deep
trench, subsurface disposal field are achieved between all water wells and subsurface
disposal fields on adjacent lots, no adverse impacts from the proposed replacement
system are likely.
Groundwater encountered in MW-1 is most likely perched within the silty sand layer
interbedded with glacial till shown in the deeper water well logs, and is not an aquifer
used for water supply by any of the adjacent lots. Nevertheless, the proposed design
maintains the 4 foot minimum separation distance, requiring that a lift station be installed
in order to provide gravity service to the basement level of the improvements.

Lot 7, Block 8, Lake Ridge Terrace
Because of the relatively steep topography on the Lower Fire Lake side of the lot ( as
well as the 100 ft clearance required for surface water), a limited area is available for the
proposed subsurface disposal field and the alternate field outside of the 100 foot
clearances to the Lot 7 & 8 water wells. Consequently, the Lot 7 septic tank and lift
station located between the house and the primary deep trench requires a waiver to the
Lot 7 water well only. The revised offset distance is approximately 90 feet. No adverse
impacts are likely because of the low permeabilities on site, because the water well is
cased to bedrock and is very deep, and because the septic tank and lift station are sealed
systems.
An alternative deep trench disposal field location is shown on Figure 2 and 3.
A check in the amount of $625.00 for the proposed waiver is attached.
The proposed design described herein is certified to be in general compliance with the
requirements of AMC 15.65.

Justification
To whom it may concern,
The owner of the aforementioned lot has a 3-bedroom home on the property with a septic
system that is nearing the end of its life. The current system consists of a steel 1,000-gallon
tank (installed in 1997, a currently undocumented system), a 700-gallon steel lift station
(installed in under the 2001 permit). This letter is to serve the purpose of address the current
1,000-gallon tank’s age and integrity.
During the COSA on September 6th, 2025, the operating water level for the septic tank remained
at a fluid level of approximately 4.1 feet for the duration of the test. After this test, the home
became vacant and has remained vacant to current day. On January 9th, 2026, the water level
in the tank was measured to be approximately 4.0 feet, which is consistent with the water level
during the COSA.
Additionally, the owner provided a document stating that the tank was placed in 1997. Given the
findings of the COSA and this document, it appears that the tank is approximately 28 years old,
rather than the previously recorded tank installed in 1973.
MOA’s Policy W.05 states that a steel septic tank with a conventional coating that is more than
20 years old, but less than 30 years old, will be issued an Advisory Notice explaining that the
tank may be near the end of its life. In this situation, it is appropriate to issue that advisory and
we are seeking an Unconditional Cosa. In the future, if the tank were to need replacement, the
owners may also utilize the current tank replacement permit after re-issuance of the permit (if
more than a year has elapsed from October of 2025.

Affidavit regarding On-Site Permit Number SW010332 and the respective application

Eagle River, Alaska
The permit application and permit number SW010332 describe and include the existing
septic tank and a new lift station and drain field. The existing septic tank was installed in
1997 by Mike Kowaleski, the property owner at the time, and a contractor, a 1000 gallon
steel tank manufactured by Anchorage Tank, as indicated in the On-Site Wastewater
Disposal System Inspection Report dated September 20, 2001. The tank was observed
during construction in accordance with Item 3 of Specification B.1 included in Section 2 of
the permit application and found to be in good condition, as seen in the attached
photographs. The liquid level in the septic tank was observed at 4.1 feet above the bottom
during the recent COSA inspection on September 6, 2025, indicating that the septic tank
continues to perform according to the requirements of the design and the respective
municipal regulations.

THIS AGREEMENT made this 23th day of January 2026, by and between Charles F & Karen
D Cobb and their heirs, administrators, and assigns, hereinafter called "PERMITTEE", and the
Municipality of Anchorage, a municipal corporation organized and existing under its Charter and the
laws of the State of Alaska, hereinafter called the "PERMITTER".
WITNESSETH:
WHEREAS, PERMITTEE is the owner of the following described real property:
LAKE RIDGE TERRACE #1, BLK 8, LT 7, according to the official records thereof,
on file in the office of the District Recorder, Anchorage Recording District, Alaska,
and;
WHEREAS, PERMITTER owns and/or maintains the real property more particularly
described as follows:
The 60 FOOT RIGHT OF WAY OF WEST LAKE RIDGE DRIVE, immediately on
the south side of PERMITTEES property as shown on Plat No. 64-82, on file in the
office of the District Recorder, Anchorage Recording District, Alaska, and;
WHEREAS, PERMITTEE has placed a leach field & standpipe upon the referenced real
property which encroaches 1' upon the PERMITTER'S West Lake Ridge Drive Right of Way.
NOW, THEREFORE, it is mutually agreed between the parties hereto that:
The PERMITTER, acting through the Director of Development Services
Department, hereby grants to the PERMITTEE the privilege of allowing the
leach field & standpipe to encroach 1' upon the PERMITTER'S 60' Right of
Way, as shown on "Attachment A," included herewith.
2. The PERMITTEE agrees forever to indemnify, defend, save and hold harmless,
the Municipality, its officers and employees, from any and all lawsuits, claims or
actions brought to any person for or on account of damage to property or injury,
disease, illness or death of persons, including all costs and expenses incident
thereto, arising wholly or in part from or in connection with the existence of,
alterations, maintenance, repair, renewal, reconstruction, operation, use or
removal of the encroaching leach field & standpipe, as placed upon the
PERMITTER'S 60' Right of Way.
3. The PERMITTEE shall not assign or transfer any of the rights granted herein to
another individual or company without first notifying and securing the approval
of the Director of Development Services Department.
4. This Agreement and Permit grants PERMITTEE no interest in PERMITTER'S
real property whatsoever. except only the encroachment rights described herein.
5. The PERMITTER reserves the right to revoke this permit upon twenty (20) clays
written notice to the PERMITTEE. The PERMITTEE agrees upon such notice
of revocation, to move said encroachment(s) from the 60' Right of Way in which
it is placed. Should the PERMITTEE refuse or fail to comply with said written
notice, the PERMITTER may, without further notice to the PERMITTEE,
remove or cause to be removed the encroachment(s). and the PERMITTEE
hereby agrees to reimburse the PERMITTER for all costs incidental to the
removal thereof.
In addition to the nurtual promises heretofore made, the PERMITTEE has paid the
PERMITTER a one-time permit application fee of $120.00. The PERMITTER hereby Nvaives the
annual fee of $315.00.

The septic tank for this property is 29 years old. The average life of an asphalt
coated steel septic tank is 20 years. Typical replacement costs are $15,000 or more,
not including engineering, surveying, MOA permitting fees or site restoration.
Steel septic tanks utilizing "Coal Tar Pitch" coatings are subject to corrosion as the
steel reacts to water and rusts. These tanks have a finite lifespan buried in the
ground. These tanks can become structurally compromised and pose a health risk.
They could collapse and/or leak untreated wastewater directly into groundwater.
In the interest of protecting public health and the environment, the Municipality of
Anchorage Development Services Department has issued Policy W.05 Standard of
Care Regarding the Inspection of Steel Septic Tanks when Performing COSA
