Permit #870052, On-site Sewer Permit
A permit issued by this Department for an individual well
and/or on-site sewer system has expired as of December 31,
1987.
Permits are issued on a calendar year basis by authority of
Municipal Ordinance. A new permit must be obtained from this
Department for any well and/or on-site sewer system not
installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this Department for documentation of the installation and to
close the permit.
If a private engineer inspected the installation of the on-site
sewer system, the original as-built inspection report (three-part
form) must be sent to this office for review and approval, and
for documentation.
Effective January 1, 1988, a new fee schedule is in effect.
When re-applying for a new permit, the new fees are; $90.00 for
an on-site sewer permit; $50.00 for a well permit; $140.00 for
a combined sewer and well permit.

THE TYPE OF ABSORPTION SYSTEM IS A CRIB WITH AN AREA OF 540 SQFT.
THE SYSTEM IS CAPABLE OF ACCEPTING 7§0 GALLONS OF WATER PER DAY.
THE SURGE CAPACITY OF THE SYSTEM IS 8000 GALLONS.
BASED UPON THE TEST DATA THE SYSTEM IS ACCEPTABLE FOR A
5 BEDROOM HOME.
SEPTIC TANK ADEQUACY
THE EXISTING SEPTIC TANK VOLUME OF 1250 GALLONS IS INADEQUATE BY
250 GALLONS FOR THIS HOUSE OF 5 BEDROOMS.
THE SEPTIC TANK/PACKAGE PLANT HAS NOT BEEN PUMPED WITHIN THE PAST YEAR.
THIS REPORT DOES NOT VERIFY THE INTEGRITY OF THE PIPING FOR THE WATER
SUPPLY OR WASTEWATER SYSTEM.

Attached is a site plan of lot 27; inspection report from GAAB; signed by Mr.
John Lee, November 18, 1970; and a soil test from National Testing Service Inc.
dated November 16, 1970. Please refer to these items as you read this letter.
It appears that on November 16, 1970 a soil test was done on the lot, probably
in the area where the seepage pit was to be built. The depth of the pit was
noted at 12 foot and the soils were recorded as 125 square foot by Mr. R. E.
Lyle. The soils are noted as a gray, gravelly sand (SW). The inspection by
John Lee on November 18th has some discrepancies in that the well is located
closer than 97 feet from the lake. We measured 32 feet. The dimensions from
the well to the septic tank checks with information that we have been able to
develop on site. Apparently the house was later enlarged and the septic tank is
now under the driveway, right at the entrance to the garage. The crib is not 60
feet from the septic tank but is nearly 97 feet wh{ch would correspond to the
lake distance shown by Lee. The enclosed plan figure #1 shows the
approximately locations as could best be determined in the field by taking
references from the lot survey.
It had been my understanding the Mr. Fernandez has talked to Steve Morris and
Steve has informed him that in order for him to keep his five bedroom house the
septic tank size must be increased. Since the Espanoza tank is a one
compartment tank an additional 500 gallon tank after the existing septic tank
would improve the treatment.
On A~ril 3, 1987 I visited the site, and checked the septic tank and crib.
Although surface water from snow melt is running across the top of the crib, the
crib is dry. This indicates that ground and surface water ~s not getting into
the crib area. The crib has not been used in several years and it probably has
recovered much of its original permeability. However, at this time it is not
possible to determine if it is adequate for a five bedroom house. In the event
that this system does not perform properly there is a slight amount of area
between the driveway and the barn which could be used for upgrading an on-site
system. It is entirely possible that the barn may have to be moved to get
sufficient area for on-site system. This is a pole barn w~th d{rt floors so
removal of the barn, although expensive, would not be a major detriment for
upgrading the system.
There is a cross hatched area between the 10 foot utility easement and beyond
the 100 foot rad{us to the well that this system could be upgraded in case the
crib is deemed to be inadequate for the house. If the soils are as good as that
indicated by NTS then it is apparent that th~s system could be upgraded and kept
within the cross hatched area. It may require placing it closer than five foot
to the barn.
Please issue the permit to upgrade the system by installing the 500 gallon
septic tank. The location of the crib is to be resolved between Public Works
and the Owner.
