Subject: Upgrade Sewer Permit — Birch Hills Estates Block A, Lot 2
It has been determined that the absorption crib of the subject property has failed.
Subsequently, the owner has requested we proceed forward to obtain a septic
permit to upgrade the septic system.
The general slope of this lot is from south to north at a grade of approximately 8-
13% over the septic area. On May 28, 2014 a testhole was performed to
investigate soils and groundwater. The results of this test are attached for your
review. The proposed upgrade will serve the existing 3 -bedroom house. We
propose to install one 32' long deep trench. Groundwater was not encountered
at excavation or at monitoring.
The property is served by private water and adjacent lots are served by public
and private water. There is no surface water within 100' of the proposed system
and there are no known curtain drains within 50'. We do not expect there to be
any adverse effect on adjacent lots by the development of this system. If you

of the well and septic applies only to the conditions as of the day tested. The flow and absorption rates may change due to subsurface
conditions that may not be observed from the surface, changes inland use, local soil characteristics, groundwater levels that may fluctuate
during the year and the water usage of the family being served by the system. The operational life of all well and septic systems are
subject to these various and dynamic characteristics and are outside the control of the

Nitrate Advisory
Certificate of On -Site Systems Approval # 141427
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block A, Lot 2 of
Birch Hills Estates subdivision. This inspection revealed a nitrate
concentration of 6.51 milligrams per liter (mg/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
has established a maximum contaminant level (MCL) of 10.0 mg/L for
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

In regards to Birch Hills Estates Block A Lot 2. On 8/20/14 We ran a camera to a depth of 40',
the casing was in tacked, the pitiless was not leaking & there were no perforations. All looked
fine.

Reference: Lot 2; Block A; Birch Hills Estate Subdivision;
Raymond Viau property
A sewer system adequacy test was performed on the system
located on the referenced property per your request.
The septic tank had to be uncovered and a cleanout
pipe installed. While the tank was uncovered it was
verified to have a capacity of 1000 gallons. The seepage
pit #1 located nearest the septic tank was dry. The
seepage pit #2 was also dry at the beginning of the test.
Approximately 1750 gallons of water were added to these
cribs and after a period of 24 hours all the water added
had percolated out of the cribs.
It can be concluded from this test that the waste water
disposal system serving the three bedroom residence located
on this property is currently functioning adequately.
However, the system.cannot be guaranteed against subsequent
failures.
