We have addressed the comments and are submitting the additional information requested. 1. We spoke with Tom Varney, Anchorage Tank and per his direction the VeriComm system remote alarm will be mounted on a wall inside the garage. Per his recommendation we added a note to mount the control panel 5' above finished grade. These revisions are shown on Figure 4. 2. VEI performed an adequacy test on the existing field on June 14, 2008. We put 450 gallons of clean water into the system downstream of the existing tank. The system returned to its original level within 24 hours. We further determined that the spill over system was not performing properly and the total flow was routed into the south leg, and the other two legs received no flow. The spillover system is being revised with these improvements. There was no evidence of any overflow or water surfacing in the adjacent road ditch or any other low areas in the vicinity. 3. The title blocks of Figures 1 thru 5 have been revised to include the legal description for the lot. (see attached) 4. The correct Parcel ID number for the newly replatted lot is 050-081-8500. Per our telephone conversation, please make this change to the previously submitted permit application. Per your requirements we have designed an Advanced Wastewater Treatment System (AWTS) to replace the existing septic tank and we are utilizing a portion of the existing absorption field. We request that you approve this design so that we may move forward with construction and complete the installation this summer. VEI has reviewed all available records for the existing well and septic system on the property. We have completed additional field investigations including surveys and absorption field mapping and TV inspection with Denali Sewer and Drain's camera and location equipment. The results of our investigations are presented in the attached drawings. The Overall Site Plan presented in Figure 1 shows the adjacent properties including the 100 -ft protective well radii at a 1:50 scale. The Existing Septic System Plan and Profile presented in Figures 2 & 3 show the location of the existing system including the test hole dug by Tobben Spurkland during a previous investigation. The proposed design, presented in Figures 4 & 5, incorporates an AdvanTex System and includes abandoning the current septic tank in place. It also includes abandoning the northern most leg of the existing absorption field. Both the abandoned tank and abandoned leg are within the 100 -foot well radius, so this design eliminates the need for a well separation distance waiver. The proposed design uses approximately 50 LF of the existing absorption bed. Calculations for this system indicate a required length of 18 LF of a 5 foot width absorption bed. On -Site Sewer Permit Application far Broadwater Heights Subdivision, Block 3, Lot 16A; SG'/>, Sec 1, T14NR2W, S.M.; Grid NW0253 Page 2 of 3 Field investigations and records indicate that the existing absorption field (constructed in 1978) lies inside the current regulatory slope setback requirement of 50 feet. However this system was constructed prior to the slope setback requirement being adopted in 1986. Additionally, field investigations indicate that the system does not daylight on the slope below the sump. A slope set back waiver is therefore not required for this system. As shown on Figure 3, the gravel interface at the bottom of the existing absorption field is less than 4 feet above bedrock which is the generally required minimum separation distance for a Category III AWTS. We request a waiver of this requirement with the following justifications: 1. The closest downgradient lots are Lots 3 and 4, Block 1. These are located to the west across Broadwater Drive, as shown in Figure 1. They are currently undeveloped, narrow and steep; and it is possible they will not be developed. The nearest property line is more than 80 feet from the subject absorption field. Only a small portion of these 2 lots are within 100 feet of the subject absorption field. The proposed septic system would not impede the development of private wells on these lots as there is sufficient area beyond the limits of the required 100 foot separation distance for installation of private wells and septic systems on these lots. 2. The density of this residential area is currently 0.8 homes/acre and it will likely remain low. Replatting of Lots 16, 17 and 18 that split Lot 17 and increased the size of the 2 remaining Lots 16A and 18A was completed in April 2008. This replatting action served to further reduce the lot density of this residential area. This area is fairly well built out with more than 60% of the lots currently developed. Many of the currently undeveloped lots are relatively narrow with steep topography and would therefore be difficult to develop. 3. Testing of a 2006 drinking water sample indicated nitrate levels of 2.78 mg/L, well below the Maximum Contaminant Level (MCL) of 10 mg/L. Coliform bacteria results in this drinking water sample were "Not Detected". 4. The AdvanTex System installed in the Mode 1 a configuration as called for in this design is classified under Municipal Code as a Category III AWTS Nitrogen Reducing System. 5. The AdvanTex System will be installed with the standard VeriComm Monitoring Control Panel. Any potential problems with the system detected by the monitoring system will be corrected in accordance with the AWTS Maintenance and Repair Memorandum of Understanding between the Municipality of Anchorage and the Property Owners, Dick and Bonnie Rohr. Advanced Wastewater Treatment Svstems. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS), described as located at (legal description). r'gAlS ?�IQ6 (rtoA 2. Definitions. Alteration. Any change to the design or function of an AW WTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Certificate o3 On -Site Stistems .Approval (COSA). An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with Anchorase Municipal Code (hereinafter, -AMC-) 15.65. These appro,,als ccrtih that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On -Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. J. Term. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations, Installation and Removal of Additional Equipment. Prior to performing any alterations to an AWWTS, the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality_ The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AWWTS for the entire term of the AWWTS In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AWWTS. C. Upon request by the Municipality, the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor, the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be assessed in accordance with AMC 14.60 for improper discharge. E. Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. H. Owner agrees that the AWS TS installation and maintenance requirements as provided by the AWWTS vendor installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS, 1. Owner agrees to provide and maintain a telephone connection to the AWWTS as required by the AWWTS approval. 6. Nonwaiver. The failure of either parry at any time to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. This Maintenance and Repair Agreement shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality that this Maintenance and Repair Agreement was executed with, and such writing shall be attached to this Maintenance and Repair Agreement as an atnendment. B. For the purposes of any amendment modification or change to the terms and conditions of this agreement, the only authorized representatives of the parties are: u. Owner: Gr f X- i tJ b. The laws of the State of Alaska shall govern the rights and obligations of the parties under this Maintenance and Repair Agreement. 9. Severability.
