1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions:
• A bedrock confirmation and groundwater monitoring testhole is to be placed near the southeast end of the
proposed field prior to installation. Construction may proceed atyour own risk prior to 7 -day groundwater
monitoring. Please submit stamped and signed results with the As -built Inspection Report. If the results
require a design change, construction of the system will stop pending On -Site review and approval.

RE: Proposed Septic System for BROADWATER HEIGHTS TRACT D BLOCK 1 LOT 19
Dear Reviewer,
The leachfield at the above referenced property appears to have failed. We are requesting approval to
construct the alternate site drain field.
There is a drainage swale more than 50’ from the proposed field location.
Groundwater was found as shallow as 6’ on the site. The bottom of field will be at a max depth of 4’. Note
that fill has also been added since the original field was constructed.
There will be more than 3’ separation between the bottom of the field and the bedrock previously
identified and more than 2’ separation to seasonal high groundwater. See attached soils log which shows
additional fill placed at the leachfield location when compared to the ERES testhole.
Note that the ERES testhole was excavated prior to the placement of several feet of fill at the reserve site.
There has also been significant earthwork and drainage work on the property. An October groundwater
reading revealed no groundwater at the leachfield location. This justifies not applying the 6’ December (4’
seasonal high) groundwater reading to the current design. The profile drawing for the original system
shows the original leachfield compared to original grade. It appears that there was approximately 1’ of
topsoil at that time as opposed to the 4’ of fill at the reserve site.
The existing Advantex unit will remain and a diverter valve will be installed.
The Advantex and tank is under or near a deck. The deck foundation does not impact the septic tank. The
deck supports are not directly above the tank.
A waiver is requested between the proposed field and a steep slope. The justification for the waiver is
provided on the profile drawing.
Our review of historic soils logs and soils logs on neighboring lots show that the native soils have a
percolation consistently below 10 minutes per inch. Based on this the use of a 5 gpd/sqft application rate
is justified, subject to visual confirmation of soil conditions during construction. If soil conditions change
materially, an additional perc test will be performed.
A gutter drain system appears to be a contributing factor in the failure of this field. The contractor
must relocate the gutter drain so that it daylights more than 50’ from the fields.

Lot Line Waiver Application
Dear Mr. Roth:
After receiving the certified as -built survey for the above referenced property, it was noticed that
the septic tank and leachfield were installed too close to the lot line. Prior to the installation of
all septic system components, the lot line was clearly marked, staked, and flagged by a licensed
land surveyor. It appears that these markings were in error, causing the septic system
components to be installed 2' closer to the lot line than intended.
The purpose of this letter is to request a waiver for the separation distance between the installed
Advantex tank and leachfield to the lot line. The distance we are requesting to be waived is for
3' lot line to septic tank, and 8' for the leachfield to lot line. The proposed site for the septic
reserve will not have to be waived, as a distance in excess of 10' can be maintained. The
proposed lot line waiver will have very limited impact on adjacent properties for the following
reasons:
1. Several of the surrounding lots are on a public water system, thereby eliminating
some 100' protective well radii.
2. Immediate neighboring septic systems are all +30' distance, or undeveloped.
3. Drainage will not be affected by this lot line waiver request.
4. The wastewatewater system is treated with Advantex technology, a MOA
Category III treatment system. Any potential for contamination of drinking water
sources or to the environment is extremely small while utilizing a properly
maintained Category III system.

For the reasons listed above, we believe that granting this lot line separation distance waiver of

Narrative & Permit Application, Vertical Separation Distance Waiver request.
Dear Mr. Roth:
Eagle River Engineering Services (ERES) was contacted by the owner of the property noted
above to design a septic system to serve a new 4 bedroom house. A test hole indicated free -
draining soils from 1 to 7 feet below ground surface, with bedrock at 7 feet. The water table was
monitored for one week and was measured to be 6' below ground surface.
The homeowner wishes to utilize Advantex technology to minimize the size of the leachfield,
and also wishes to construct the alternate site at the time of installing the primary system.
We are requesting a waiver from bottom of leachfield to bedrock of 5 feet. We feel the fractured
nature of the bedrock in addition to the quality of the effluent provided by the Advantex
technology will be protective of human health. In addition, we are requesting a waiver for 4 feet
from the bottom of the leachfield to ground water. ERES believes that positioning the sewer
rock just below the topsoil layer, with the sides and bottom of the sewer rock surrounded by
native free -draining soil is preferable to being elevated with a sand layer, with the sides of the
drain rock confined by visqueen or topsoil. We believe there to be a lesser chance of the effluent
saturating the topsoil layer, or potentially daylighting if this vertical separation waiver is allowed.
The topography at the leachfield site is a consistent 15%-20% toward the southwest.
The lot will be connected to the public water supply. A keybox already exists in the southern
corner of the lot.
The proposed 4 bedroom septic system and well will have very limited impact on adjacent
properties for the following reasons:
1. The surrounding lots are well laid out, so there is room for wells, septic systems
and alternate sites.
2. Immediate neighboring septic systems are all +20' distance, and no private (or
community) wells within 100' of the proposed septic system.
3. Drainage will not be affected and is not a major consideration in our design.

A. GENERAL
1. The septic plan is for a 4 bedroom single family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. All materials and workmanship shall meet the Anchorage Department of Health and State Department
of Environmental Conservation requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the field by the engineer.
5. All excavations and depths are advisory and are to be verified in the field by the contractor to meet
Municipality of Anchorage, Department of Environmental Conservation requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate any
adjacent multi -family wells. A licensed surveyor should locate any utility easements and exact 100'
radius from the existing and neighboring on-site wells.
7. The excavation is to be exactly in the area shown on the site plan, any deviation requires engineer
approval.
8. Any remaining open test hole excavations shall be filled and monitor tube removed.
13. ADVENTEX SYSTEM AND SEPTIC TANK
1. Septic tank shall be a fiberglass or steel 1500 -gallon tank capacity of MOA approved construction for
use with Advantex system. Install as per current manufacturers recommendations for conditions in
Alaska.
2. Install one AX -20 filter pod over septic tank capable of treating 450 gallons per day of waste flow.
3. If gravity feed can not be achieved, Install Orenco 24" pump basin P13-2496 outside septic tank,
with OS1 pump model P30-05 and controls to allow 23 gallon dosing of septic leachfield. Set pump
float to 13.5" above pump, timer for 1 minute dose time. Controls to be installed to MOA code by
licensed electrician familiar with OSI applications. Controls to include a dedicated phone line and web
based monitoring system.
4. A municipal inspection report or receipt from a licensed electrician shall be provided to the
engineer verifying lift station wiring to all applicable codes.
C. DRAINFIELD
1. The drainfield is to be located as shown on the site plan.
2. The total depth of the initial drainfield excavation is to be at a maximum 2' deep, relative to existing
ground elevation and end in native sandy GM -type soil. The excavation shall remove the topsoil layer.
The bottom of the excavation shall be level, plus or minus 1.5".
3. An eighteen -inch layer of sewer rock is to be placed over the sand layer (fifteen inches if lift station is
utilized). The 4" effluent piping (1' if lift station is required) is to be buried within the gravel layer
with 2" of gravel cover over the piping.
4. The completed drainfield gravel and piping is to be covered with typar fabric material.
5. Soil or combination of soil and extruded board insulation to a depth of 3' or equivalent is to be placed
over the bed. Mounded side slopes on uphill side and ends are not to exceed 3:1.
6. The septic tank and leachfield must not be closer than 100' to any existing private well, 150' to any
Class "C" well, or 200 feet to any community well.

REPAIR AGREEMENT
MEMORANDUM OF UNDERSTANDING
BETWEEN MUNICIPALITY OF ANCHORAGE
AND

"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
Treatment System (AWWTS), described as Ad V Q 4 " CX located

2. Definitions.

A. Alteration. Any change to the design or function of an AWWTS that
Includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65.
B. Certificate of On -Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15.65.
The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On -Site approval is issued to
the new owner or transferee of the property.
5. Alterations, Installation and Removal of Additional Equipment. Owner agrees
not to make any alterations, removal of parts or additions to the AWWTS without a
Construction Permit from the Municipality.
6. Maintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further, Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWWTS
Operating Permit is:

AWWTS are codified in AMC 14.60.

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of
Understanding shall be brought in the Superior Court for the Third Judicial District of the
State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severability. Any provisions of this Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions
of the Memorandum of Understanding.

The subject property had a new wastewater system constructed 6 years ago. This was an
advanced wastewater system. The system was previously approved by the MOA and a COSA
issued. The property is being sold and now requires a new COSA from the MOA. During the
COSA inspection it was discovered that surface water is present at a horizontal distance of 60'
from the "Advantex" wastewater tank and drain field.
After consultation with the owner and MOA, we will request a waiver according to the State of
Alaska (SOA) Memorandum on waivers. We expect that the general score for the waiver will not
be a high number, but the SOA Memorandum allows for mitigating factors which may allow a
waiver. In this case the mitigating factor is the advanced wastewater effluent from the
"Advantex" wastewater system.
Geological: The MOA approved the wastewater system at 4' to groundwater and 5' to bedrock
from the bottom of the leach field. This was done due to the mitigating factor of the advanced
wastewater system. We are also making the same argument for the horizontal separation.
1. Water Table: The distance from the bottom of the system to the highest water table is 4'.
This only gives a point total of 0.5.
2. Soil Sorbtion: The soil tests from the MOA file indicate the soil is GM, silty gravel. The
average of gravel and silt equals a point total of 1.75.
3. Permeability: There is not a point value for silty gravel, so a combination of silt and sandy
gravel will be used. This yields a point total of 1.5.

4. Water Table Gradient: The water table gradient is at -5%. The ground slopes toward the
surface water. This yields a value of 2.0.
5. Horizontal Separation: The horizontal separation is 60'. This yields a value of 1.4.
The total point total = 0.5 + 1.75 + 1.5 + 2.0 + 1.4 = 7.15
This is not a point total that would normally allow the waiver. However the SOA Memorandum
places special emphasis on "mitigating" factors. The essentially clean effluent from the
"Advantex" advanced wastewater system will not pollute the surface water. Anchorage Tank
continually monitors the system from their office via telemetry.
We request the surface water waiver due to the advanced wastewater system, as was previously
approved by the MOA for groundwater/bedrock separation.
