SUBJECT: On-site Sewer: Lot 14 Block 3 Brookwood Subdivision
This letter is written to summarize the status of the on-site
sewer system located on the above mentioned property.
The system was installed and inspected in June of 1975. Approval
was given to the system on June 6, 1975 by Robert C. Pratt,
Sanitarian (approval form attached). The system that was installed
was a Multi-Flo waste water treatment plant.
In October, 1976 a survey of all Multi-Flo units installed in the
Municipality of Anchorage was initiated.
On November 17, 1976, Mr. J. Pace, Jr., owner of the above
described property at the time, was contacted and asked to complete
a questionaire (copy attached).
On November 22, 1976, a meeting was held by Health and Environmental
Protection as a result of homeowners complaints and the findings
of this department while accomplishing the survey. A resume of
that meeting as well as a memo to the file concerning a homeowners
meeting held that same night, at Dave Downs residence in Talus
West is attached. A copy of the notes made by one of the home-
owners that attended the meeting is also attached.
On December 2, 1976, Rolf Strickland, Environmental Engineering
Manager, wrote a letter to Mr. Dick Schoph, Aerobic Sewage Puri-
fication, Inc., 5909 Old Seward Highway, Anchorage, Alaska 99502,
that discussed the meetings held in November 22, 1976 and to
request response by the distributor, Aerobic Sewage Purification,
Inc., and maintenance contractor, Arctic Chain Plumbing, in making
the necessary corrections agreed upon at the meetings referred
to above.
On January 4, 1977, Nick Feldman of this department again surveyed
the above referenced Multi-Flo system on Lot 14 Block 3 Brookwood
Subdivision, and in a report of the same date noted maintenance
problems previously described at the homeowners meeting

still existed (report attached). Maintenance problems existing,
at this time, with system would have precluded approval
by this department.
On January 18, 1977, photos of the system were taken by Nick
Feldman of this department. These photo's visibly show the
problems existing with the unit (see attached memo).
As a result of the survey conducted by this department, a letter
dated January 20, 1977, by Joseph S. Blair, Environmental Services
Manager, was sent to Mr. Dick Schoph, Aerobic Sewage Purification,
Inc., 5909 Old Seward Highway, Anchorage, Alaska 99502, to
inform him that the Multi-Flo waste water treatment plant had
been removed from the list of package treatment plants approved
for installation within the Municipality of Anchorage. This
letter referenced, in part, the problems mentioned above and
requested correction within thirty (30) days of receipt of the
letter.
Additional photo's were taken January 31, 1977, and the same
problems were evident.
Another field survey was conducted by Nick Feldman on February
2, 1977. The findings of this survey reflect the re-ocurring
problems previously noted. No improvement was evident. Report
attached.
On February 28, 1977 a request for approval of J. ndividual sewer
and water facilities was received from Alaska Statebank listing
Larry D. and Cecily Wright as owners of the above property.
Letters were written on March 4, 1977, and March 18, 1977 con-
cerning the above referenced on-site sewer system by Robert C.
Pratt. Both of these letters are in error and should be disregarded
as they do not reflect the current status of this system.
The system cannot be approved by the department at this time, unless
two (2) conditions are met:
Maintenance problems previously noted with this system must
be corrected.
(2)
A written agreement to insure that proper maintenance will
be continued as long as required.
Brookwood Subdivision has been officialy declared to be a health
hazard because of problems generated by on-site sewer systems.
A lateral improvement district has been proposed for this
subdivision. It is anticipated by this department that this L.I.D.
will be accomplished in 1977.

Lot 14 Block 3 Brookwood Subdivision
Dear Mr. Matthews:
We are submitting herewith the test boring results and our comments
regarding soil conditions encountered at the subject site. This
investigation was performed in accordance with your request of April,
1975, and those procedures outlined in a letter dated December 19, 197~
by Mr. Rolf Strickland of the Greater Anchorage Area Borough Department
of Environmental Quality.
A single test hole was put down within the Lot 14 area for the purpose
of defining general subsurface soil conditions for the proposed sanitary
system. Excavation was accomplished with an auger type drilling rig
~nd the test hole was extended to a total depth of 20.0 feet below
ground surface. One sample was recovered from the bottom of the test
hole and retained for grain size analysis. The results of this sample
are enclosed. The final log prepared for the test hole has been included
in Drawing A-0I.
Ground water was encountered in the test hole at a depth of 10.5 feet.

MULTI-FLO WASTEWATER TREATMENT PLANT

by and between the user of a multi-rio wastewater plant and _ /c_ ff S
the servicer,
WITNESS:
THIS AGREEMENT is executed concurrently by the user and the dealer service manager
at the time of installation. The dealer covenants and agrees to furnish twenty-four (24) months
service on the user's waste treatment plant from date of installation. The purchaser-user
covenants and agrees by the presents to, and does hereby contract with
to service .the user's waste treatment plant upon expiration of the dealer's aforesaid free service
period of twenty-four (24) months. The user understands by signing this agreement that the user
accepts respon, sibility for the service after the dealer free service period, and also understands
the necessity and responsibility for the service requirements that are advised by the dealer.

This service guarantees to make two (2) service and inspection calls per year at regular intervals
and to respond promptly to any emergency calls for repair service. Unscheduled emergency or
service calls not due to malfunction of or defective sewage treatment units shall be paid for by
the sewage 'treatment plant owner. Although the regular service and inspection calls during the
first twenty-four (24) months of installation will be free to the sewage treatment plant owners,
the sewage treatment plant owner shall be required to pay the Service Contractor's road time for
travel on an average scale computed by averaging road time costs for the number of plant
customers in the area of the service call.
Inspection service Calls will include the following:
1. An effluent quality inspection consisting of a visual check for color and examination
for odors.
2. Adiustment and servicirig of any mechanical and electrical components that are out of
order.
3. Backwashing of filter media or replacement of filter media with previously washed
filters,
4. Periodic sampling, twice a year if necessary, of the effluent quality, and of the settled
solids in the aeration tank.
Any additional work necessary will be done upon written authority from the homeowner and
at additional charge.
