The subject permit #SW990o¢~,issued by this office for a single family well and/or on-
site wastewater system, is duc to expire 365 days after it's issuance on May 7, 1999.
If this is a well permit and you have drilled the well, a well log must be sent to this office
for documentation of the installation and to close the permit.
If this is an on-site wastewater system and a licensed Professional Engineer has inspected
the installation, the original as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports must be submitted within 30
days of construction completion.
A new permit must be obtained fi.om this office for a well and/or on-site wastewater
system NOT installed by the expiration date. However a new permit can be issued free of
charge for a second year if the application for the renewal is received on or before the
date of expiration of the original permit for which a fee was paid.
When applying for a new permit after the original permit has expired or for more than a
second year, the fees are: $320.00 for an on-site wastewater permit and $120.00 for a well
permit.

Lot 12, Block 3 Broadwater Heights Subdivision, Eagle River, Alaska
Dear Sirs:
We have prepared and subnfit herewith an application for a permit to construct a new
well and septic system on the above-referenced lot.
A well and septic layout plan "LCMF-7-10-98-F' at 1"=20' scale and a site plan
"LCMF-7-10-9&2" at 1"=50' scale and a detail drawing "LCMF-4-27-99-1" were
prepared and are attached herewith to show the new system's design and its relation to
the existing wells and septic systems in the area.
The original site is generally described as an approximate 1/2 acre lot originally
heavily treed with birch, alder, and mature spruce trees. The lot slopes upward toward
the east at an approximate natural slope that varies from 11% to 42% at the back of
the lot fm'thest a~vay from Broadwater Drive.
Three soil percolation tests were conducted on July 9 and July 10, 1998 (TH 1, 2, and
3). Three 4 inch diameter perforated monitor tubes were installed to the bottom of the
testholes on July 9, 1999 and were monitored until April 27, 1999. Groundwater was
found in TH-1 only and water depths were recorded on 4/10/98, 8/10/98 and 4/26/99.
See attached Detail 1. Ground~vater was not found in TH-2 and TH-3 through the
monitoring period of 7/10/98 and 4/26/99. Although groundwater was encountered in
TH-l, the depth is greater than the 4 foot minimum required by regulation. Bedrock
was not encountered.
Soils encountered in the desired percolation zone between a depth of about 4 feet and
8.5 feet are described as a uniform, homogeneous layer of well-graded to poorly
graded sandy gravels (GP/GW). The calculated percolation rates were 6 minutes per
inch for TH-l, 1.3 minutes per inch for TH-2, and 0.2 minutes per inch for TH-3.
This lot has available space for the required and an alternate absorption system. The
The proposed absorption system is located downslope of the proposed residence and
will operate by gravity. Based on the need for four (4) bedrooms, a five foot wide
trench design, a trench width of 5 feet and a required application rate of 0.80 GPD/SF
the required trench length is calculated to be:
Length = (4 BR)(150 GPD)(.58 R.F.) = 87 feet

The probable impacts to adjacent properties is as follows.
Wells. Construction of the septic system on this lot will reduce the available
space for new wells on Lots 10 and 11 of Block 3. See the attached site plan.
V/astewater Systems. Construction of the well on this lot will reduce the
available space for new septic systems on Lots 11 (vacant)and will have a
minor impact on available space for septic systems on Lot 11 Block 1 (vacant)
and Lot and 12 of Block l(not vacant). See the attached site plan.
Reserved Space. The proposed septic and well systems will have minor
impacts to reserved spaces on Lot 12, Block 1; Lot 11 Block 1, and Lot 11
Block 3.
Drainage. The probable impact to adjacent propm~y's drainage will not be
affected by the proposed development. The proposed septic system will not
change natural drainage patterns.
