DATE: 72-008 (Rev. 4/~) ON-SITE WASTEWATER DISPOSAL SYSTEM CONSTRUCTION PRACTICES and MATERIAL SPECIFICATIONS REFERENCE: South Hills Subdivision, Block 1, Lot 8 GENERAL: 1. Se The scope of this project includes the installation of a 1250 gallon wastewater S.T.E.P. system (septic tank) and a pressurized absorption bed to serve the three bedroom residence located on the referenced property. The existing septic tank is to be excavated, pumped, crushed, and abandoned in place. Also the existing leachfield is to be abandoned in place. Construction shall be in accordance with the approved site plan and design drawings; Municipal permit with any special provisions or conditions; and all applicable State and Municipal Wastewater Disposal Regulations. The contractor shall be responsible for obtaining any necessary underground utility locates. Unless specifically agreed otherwise, the property owner shall be responsible for final grading areas subsequently depressed from soil settling. On all leachfield mound systems, the property owner shall be responsible for ensuring a satisfactory vegetation growth over the mounded area. Contractors installing wastewater disposal systems must be certified by the Municipal Health Department for system installations. Owners installing their own systems must also receive prior approval from the Municipal Health Department. SEPTIC TANK INSTALLATION: A septic tank is to be constructed by a certified septic tank manufacturer. Construction shall include two 4" cleanouts for pumping access. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. 3. Ail standpipes on the septic tank shall extend a minimum of 12 inches above final grade. Page two South Hills Subdivision, Block 1, Lot 8 Construction Practices Septic tanks installed with less than 4 ft. of cover shall be insulated. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shall be two adjacent cleanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shall be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in line, shall be to clean toward the leachfield. The second cleanout shall be to clean toward the septic tank. Final grading over the septic tank shall be such that a positive slope exists away from the septic tank. PRESSURIZED MOUND SYSTEM INSTALLATION: 1. Any peat or organic matter must be removed from the elevated mound site· 2. The bottom of the basal bed area as well as the top of the sand filter is to be within two inches of level· The distribution piping is to be of PVC (ASTM D3034 or equal). All joints are to be solvent cemented. The side slopes of the top layer of the mound system must not be steeper than 33% (3:1). The top of the mound shall be covered with a minimum of 6 inches of topsoil and vegetated sufficiently to prevent erosion. The distribution pipes are to be embedded in sewer rock. Care should be taken to backfill in such a way as to prevent damage to the piping system. Silt barrier material must be installed between the final gravel layer and the native soil backfill. Ensure the silt barrier covers the entire gravel surface before placing backfill. Backfill over the final gravel layer must not be less than twenty-four (24) inches. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. Page three South Hills Subdivision, Block 1, Lot 8 Construction Practices MINIMUM MATERIAL SPECIFICATIONS: 1. Any septic tank proposed for installation must be constructed by a Municipally approved septic tank manufacturer. Heaffh & Human Servi~~...~,~ . ~ Ref: Health Authority Approval for Lot 8, Bk 1, South Hills S/D. To whom it may concern: GENERAL: The subject lot is served by a private well and septic system (upgraded 7/93). On 5/20/96 adequacy tests were performed on both. The results are sununarized as follows: WELL: The static water level was determined to be at 45 feet below the top of the casing. Water was pumped at an average rate of 4.68 gpm for 120 minutes (561 gallons). The water level dropped continuously throughout the test and reached 204 feet (BTC) at the end of the pumping period. The recovery was monitored for 65 nfinutes during which time the water level rose to 105 feet, a recovery of 148.5 gallons. This corresponds to a recharging rate of 2.28 gpm. With a static water level of 45 feet, and the pump set at 250 feet, the storage capacity in the casing is approximately 305 gallons. Based upon this data, the well is deemed to be adequate for a 3 bedroom house. SEPTIC: The existing system was installed in July of 1993. Prior to starting the adequacy test it was noted that there was no liquid in any of the monitoring tubes. Water was introduced into the STEP tank at a rate of 4.68 gpm for 120 minutes (561 gallons). It was estimated that 80 gallons had been introduced into the STEP tank since the last pumping cycle ended, and prior to starting the test. I stopped introducing water at the start ora pumping cycle to ensure that all of the water metered was introduced into the drainfield. It is estimated that the total amount of water introduced into the drainfield was 640 gallons. The water level in the drainfield monitoring tubes varied from .75 inches to 1.5 inches. Sixty-five minutes later all the monitoring tubes were dry. I visually inspected the base of the mound and found no sign of daylighting. Based upon this data, the septic system was deemed to be adequate for a 3 bedroom house. If you have any questions, please 1-800-481-1162. Thank you. contact me at 337-6179, or on my digital pager at Sincerely, Je . trness, P.E., M.S. Owner/Consultant NOTE: The adequacy of a septic system is influenced by numerous factors, including, but not limited to, seasonal surface water infiltration, groundwater variations, septic system maintenance (frequency of septic tank pumping, usage of biological additives), condition of drain pipe and pipe joints (which can be damaged by seismic activi(y and deteriorate with age), (ype of substances deposited in septic system (cigarette butts, sanitary napkins, misc. objects), and the amount of water being introduced on a continual basis. Consequently, the results of this adequacy test are only valid for the specific day of the test. Furthermore, because of the limited nature of this investigation, it is possible that there are hidden defects which may not have been detected. No warrantee is made regarding the future performance of this well or septic ,system. c.c Paul and Rosemarie Crowe Paul Crowel.wps M-W DRILLING, Inc. P.O.
