based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water

Septic System Advisory

During a recent adequacy test on the septic system for Block 3, Lot 23 of
South Park #2 subdivision, 46 inches of standing water was observed in the
absorption field. This indicates that approximately 5% of the absorption
area is inundated. Although this system passed the adequacy test, the
remaining life expectancy may be limited.
This advisory must be attached to all copies of the subject Certificate of On-

Mr. Morrison,
Per the request of your agent,we inspected the septic system which serves the subject property for
the purpose of obtaining an MOA Certificate of Onsite Systems Approval(COSA). Site visits were
performed on 5/2/2017 & 5/7/2017. Our findings are summarized as follows:
The septic system serving the subject property was installed on 5/16/1984 and consists of a trench
type drainfield and a 1,250 gallon steel septic tank. The monitoring tube extends approximately 50
inches below the invert of the adjacent clean-out. Prior to the start of the test on 5/7/2017, the
liquid level in the drainfield was 45 inches deep in the MT (approximately 5 inches below
distribution line). One-hundred-four (104) gallons of water were introduced into the drainfield
during the first 15 minutes of the test, causing the liquid level in the MT to rise approximately 1
inch(4 inches below the distribution line). Over the remaining 75 minutes of the test, six-hundred-
two (602) gallons of water were introduced, resulting in no additional rise in liquid level in the
MT. It should be noted that it is unknown whether the additional 602 gallons of water was absorbed
into the surrounding soil or simply migrated to a section of the drainfield that could not be
monitored. Based upon this limited data, it appears that the drainfield currently meets the required
absorption rate for a four bedroom home within the municipality of Anchorage(600+gallons/day);
however, it is reasonable to believe that the 33 year old drainfield and steel septic tank are
approaching the end of their useful lives. It is our recommendation that a COSA advisory notice
be issued.
