Septic System Upgrade Design Change
Permit No. SW050412
Attention: Joe Goodall
I request a change to the design for the subject septic system, to increase the maximum
depth from 6 ft to 6.6 ft. This design change was verbally approved by Jeff Poet on May
12, 2006, with the condition of deepening the test hole.
The test hole was dug on September 22, 2005 and evaluated by Jeff Garness, P.E. It
was dug to 12 ft and encountered no water. Further groundwater monitoring showed no
water through October 10, 2005. 1 monitored the groundwater in this test hole at 11.0 ft
below original grade on May 10, 2006.
On May 13, 2006, the test hole was deepened to 13 ft. The soil was moist, and at the
bottom, we were beginning to encounter chunks of bedrock. Groundwater monitoring
was performed on May 15, 2006, with a water level at 11.4.ft below grade.
The bottom of the easterly trench is 6.6 ft below high original grade. Based upon the
deepened test hole, this is 6.4 ft above the bedrock and 4.4 ft above the high water level
measured on May 10. 1 believe that this design change is within regulations and is
protective of human health and the environment.

Ref: Proposed Septic Upgrade for Storck Subdivision #2; Lot 3,
To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The septic system is in a
state of failure and needs to be upgraded. At the homeowner's request, we are proposing to install a new
1250 gallon septic tank and a dual 5 -wide trench type drainfield sized for a four bedroom house. One test
hole was excavated on the property. The drainfield will be designed around the 30 foot radius of this test
hole. Comments regarding the design are summarized as follows:
1. SOILS: Sec the attached logs which shows the soil classifications, groundwater monitoring, and the
percolation test results.
2. DRAT\FIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There arc no surface waters within 100 feet of the proposed septic system.
4 TOPOGRAPHY: As can be seen on the attached design drawing the average topography below the
drainfield is a 10-15 percent slope running approximately east to west. In short there are no slope
concerns.We are unaware of any adverse impacts this installation would have on adjacent wells or septic
systems.

Lot 3, Storok Subdivision, Add. 2, NE4, Section 35, T12N, R3W, S.M.,

Dear Mr. Sutton:
We are submitting herewith the test boring results and our comments
regarding soil conditions encountered at the subject site. This in-
vestigation was performed in accordance with your request of March 16,
1977, and those procedures outlined in a letter dated July 15, 1975,
by Mr. Rolf Strickland of the Municipality of Anchorage, Department
of Environmental Quality.
A single test hole was put down within the subject site area for the
purpose of defining general subsurface soil conditions for the proposed
sanitary system. Excavation was accomplished with an auger type drilling
rig and the test hole was extended to a total depth of 19 feet below
ground surface. The final log prepared for the test hole has been
included in Drawing A-01.
Groundwater was not encountered in the test hole while drilling.
A percolation test was performed from a depth indicated and reflects
average infiltration from that depth to the bottom of the hole.

In conducting this evaluation, GEG, UD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during tha year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that

Certificate of On -Site Systems Approval # 121373
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block , Lot
3 of Stork #2 subdivision. This inspection revealed a nitrate concentration of
6.27 milligrams per liter (mg/L) was reported for the property's well water
sample. The Environmental Protection Agency (EPA) has established a
maximum contaminant level (MCL) of 10.0 mg/L for public drinking water
systems. While private wells are not subject to this regulation, EPA
standards are based on existing health information and can therefore be used
to gauge the relative quality of water from private wells. Please see the
attached "Nitrate Fact Sheet" for important information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.
