ArcTerra Consulting, Inc. 212 E. 51st Ave. Anchorage, Alaska 99503 Sean Parnell, GOVERNOR 555 CORDOVA ANCHORAGE, AIC 99501 Phone: (907) 269-7519 Fax: (907) 269-3487 http://www.state.ak.usklec/ September 23, 2010 manta pi 170 Re: Stephans Subdivision, Tract A-4, Chugiak-Conditional Approval to Construct a Wastewater Collection System-ADEC Plan Tracking Number 8323 Dear Mr. Duffus: On September 9, 2010 the Department received the engineering plan submittal for the replacement of the onlot wastewater treatment and disposal system using a new Orenco, Advanced Wastewater Treatment System (nitrate test results were found to be 4.13 mg/1 on September 7, 2010), and a new deep trench type soil absorption system for the 7 bedroom duplex at the above location. In addition to the decommissioning of the existing wastewater system, new Drinking Water Service lines will be added. This allows that the facilities on both Lot A-4 & A-3 be connected to the existing Public Water System and that the old Class C source well to not be used as a public water source, but be used for only agricultural purposes. Should this old Class C source well be found to be no longer required, the well would need to be properly decommissioned per 18 AAC 80.015 [Drinking Water Regulations]. The 7 bedroom duplex has a design requirement of 1,050 gallons per day flow. The soils were field tested and determined to have a 27 min/inch percolation value with an application rate of 0.45 gallons /ft2 being selected for sizing. Therefore a field with a minimum area of 2,333 ft2 is required. Your design selected the use of a deep trench type soil absorption system with 4 parallel trenches, 5 foot effective depth and length of 59 feet for each trench or an area of 2,360 ft2. The Orenco unit is as supplied by Anchorage Tank for a flow in excess of the 1,050 gallon required flow by utilizing a 1,500 gallon septic tank and a 1,000 gallon STEP system in tandem. Along with the commercial Orenco systems alarms and controls as supplied by Anchorage Tank and approved per standard drawings, for the flow of 1,050 gallons per day. The Department has reviewed the engineering plans submitted for the onsite wastewater disposal and treatment system and per the Wastewater Regulations 18 AAC 72.225 Conditional Approval to Construct the proposed system is hereby issued. A "Certificate to Construct" for the community wastewater system is enclosed. Please use the referenced plan number in correspondence regarding this system. This approval is contingent upon compliance with the following conditions: 1) The current copy of the maintenance agreement, with Anchorage Tank for operation and oversight to assure proper operation of the Advanced Wastewater Treatment System must be included with the record drawings. Should the maintenance agreement be changed with new or different personnel providing the needed oversight, this office must be kept current on these changes by the owner. 2) If the applicant fails to construct, alter, install, or modify the system within two (2) years after the date that the department issues an approval to construct, the approval is void, and plans must be resubmitted, [along with associated fees], for department review and approval. 3) Deviations from approved plans which affect capacity, flow, operation, major design of units, point of discharge, materials of major system components (such as pipe, lagoon liners, etc.), or separation distances, must be approved by DEC in writing prior to their implementation. 4) This approval is contingent upon compliance with the conditions of Wastewater Disposal Regulations, 18 AAC 72.235, Construction Certification. The noted section of the regulations requires that a "Certification of Construction" be completed and submitted to the Department within ninety (90) days of completion of construction. As - built plans or record drawings should indicate any changes or deviations from the approved plans to facilitate final review. A "Certification of Construction" form is enclosed for your use. 5) This approval is contingent upon your receipt of any other state, federal or local authorizations which are required for your project. You are required to obtain all other necessary authorizations before proceeding with your project. 6) You are advised that if this development will require placing fill in wetlands or working in a stream, river, or lake, permits from the U.S. Army Corps of Engineers and the Alaska Department of Natural Resources may be required. A Coastal Projects Questionnaire will help you identify other permits and approvals that may be required for your proj ect. This approval does not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185.
