Box 196650 Anchorage, Alaska 99519 Subject: Lot 13, Block I Spring Forest Subdivision Request for Reissuance of Pemfit SW900 155 Gentlemen: I am writing to request that your deparm~ent re-issue pennit SW900155 for this subject lot. There has been changes to the original submittal. The testhole shown on the original submittal had the test hole in the wrong location, I have showu tile con'ect location on the altached design and site plan. I also conducted a percolation test of the soils found in tile test hole. The attached soils log and percolation test results are attached. In addition, Pemfit SW900155 was issued for a four bedromn system. The new owners are consn'ucting a three bedroom house. So I request that the new pemfit be issued for only tlu'ee bedrooms. 'File lot is approximately I. t acres in size. Lot 13 slopes to the east at a rate of approximately t percent where the new drain field is being placed. The proposed installation will be located on the eastern pmlion of the lot. The proposed location is gq'eater titan 200 feet away fi'om ally wells (lhis subdivision is served by a comnmnity well) and 25 feet from the water service lines. The sun'ounding wells are located greater than 200 feet from the proposed installation. The proposed installation will not affect the futm'e development of the sun'ounding or existiug lots. A new permit must be obtained from this office for a well and/or on-site wastewater system not installed by the expiration date. If you have drilled the well, a well log needs to be sent to this office for documentation of the installation and to close the permit. If a private engineer inspected the installation of the on-site wastewater system, the original as-built.inspection report (three-part form) must be sent to this office for review, approval and documentation. All inspection reports must be submitted within 30 days of construction completion. When applying for a new permit, the fees are: $90.00 for an on-site wastewater permit; $50.00 for a well permit; $140.00 for a combined on-site wastewater and well permit.
