We propose to install a 1250-gallon HDPE tank to serve the existing 4-bedroom residence. The lot and area are served by private and public water as shown on the design. The existing tank will removed, decommissioned and the new tank installed in the same location to maintain the 87+ separation to the referenced propertys well and in accordance with WR90042 as previously issued by the MOA. Continuance of this waiver is justified with a new improved tank (HDPE) and as outlined previously with the original waiver. The design will not impact any of the neighboring properties. We request you issue a Health Authority Approval and grant a 98 feet separation distance waiver from the well serving the neighbor's house (lot 9, block 4) to the drainfield. The well was drilled on 12/11/85. The encroaching septic system was installed on 10/23/90, with no waiver to the subject well. We shot the separation distance from the center of the well to the center of the closest clean-out (not monitoring tube) using a Topcon GPT -1003 total station. The measurement is 99.9 feet. Given that the trench is 3 feet wide the separation distance is approximately 98.1 feet (edge of well to edge of trench). The following items are justification for the waiver: • Surface Migration and Contamination: As can be seen on the site plan, the well is uphill from the drainfield, consequently, contamination from surfacing effluent is not possible. • A waiver was granted in 1990 for 87 feet separation distance from the well on Lot 8 to the septic tank on Lot 8, and 92 feet from the well to the drainfield. • Subsurface Migration and Contamination: As can be seen on the attached well log, the confined aquifer (well 161 feet deep, static water level at 87 feet) is relatively deep, with over 49 feet of hardpan, which has served to inhibit the migration of untreated wastewater into the aquifer. Water samples taken from both Lot 8 and Lot 9 on 8/22/2000 had no coliform and showed nitrate levels of 0.500 mg/L.
