2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing

1. The Record Drawing is to confirm that the 1982 well has been connected to the house.

Dear Mr. Morris:
The enclosed applzca~lon zs resubmztted for issuance of a septic
upgrade permit for a 4 bedroom home usage on the above referenced lot,
in addition 'to an upgrade lpeemit for a 3 bedroom trailer on the same
lot.
The lot currently has. a seepage pit in 'the western section (to be
called section A) that w~s installed with no inspection in 1983. The
owner has agreed to ins'tail a new leaohfield and a new soil log has
been performed. The area idoes not have a ground water problem and
therefore the existing see~page pit will be used as an inline grease
trap. The existing pit was installed to a depth of 14' and it is noted
that the pit contained 62'! o~ standing water demonstrating the
effectiveness of the underlying soil layers as a filter material. The
recommended trench design ils specified versus use of a sand filter
design in 85 material due Itc lack of water table and underlying soil
layer that acts as a filt8r (see well loss). I~a sand filter is a
blanket requirement in ali 85 soils~ the bed designs enclosed are
submitted as alternatives; Each design requires waiving lot line
setback requirements to zero. These lot line setbacks should not be a
problem due to large (100'~) road easem, ents in the area.
The 2nd septic sy'ste~ will serve a 3 bedroom trailer on the
eastern side of the lot (~o 'be ealled section B). We are asking for
permit approval for section B ia advance of the 7 day monitor, with the
condition that the test h61e water monitor be checked at time of actual
installation, 6/30/86. The two trailers located on the original site
plan have been removed. Each well, section A and B~ will serw~ only
one residence, therefore ~hey are private wells. The attached design is
recommended.

OSC211105 Narrative
The existing 3 bedroom house is served by a private well and septic system. The septic system consists
of a 42' x 36" x 72" effective depth trench.
MOA issued permit OSP211001 to replace the septic tank draining this house. The scope of work for this
permit was to replace the septic tank at the house and backfill the seepage pit upstream of the new
septic tank. This work has been accomplished.
In addition to this permitted work, the existing tank draining the old mobile home was removed, and the
field abandoned in place.
There is an additional well located in the southeast corner of the property. This well was found summer
2021 and decommissioned by Anchorage Well and Pump Service in July 2021. A decommissioning log is
included.
The casing of the 1982 well was straightened. This well was determined to have water. The water line
was routed to provide water to the house.
Previously conditional COSA OSC211105 was issued. At this time, we request a full COSA be issued for
this property.

Request for Conditional COSA
The existing 3 bedroom house is served by a private well and septic system. The septic system consists
of a 42' x 36" x 72" effective depth trench.
MOA has issued permit OSP211001 to replace the septic tank draining this house. The scope of work for
this permit is to replace the septic tank at the house and backfill the seepage pit upstream of the new
septic tank.
in addition to this permitted work, the existing tank draining the old mobile home will be removed, and
the field abandoned in place.
There is an additional well located in the southeast corner of the property. It is labeled as 'abandoned'
on an older survey. The well casing was not located in December 2020 as the ground was frozen. When
equipment is on site to replace the septic tank, a further search for this well will be conducted and it will
be verified if it has been decommissioned per MOA water code.
The condition of the 1982 well is not known. The well casing is bent just above the pitless. The well
casing will be straightened and if water is found in this well and the water quality is sufficient, a new
pump will be provided and the water line will be routed to provide water to the house.
It is proposed that this repair work will be completed during the summer months when weather and
ground conditions are more favorable. The work will be complete by June 15, 2021. Money is being held
in escrow to complete this work by First National Bank. See accompanying letter.
At this time, we request a conditional COSA be issued for the sale of this property. Recent water samples
show that nitrates are absent from the 1985 well water on this property.
There is no eminent hazard created by granting this conditional approval. Wastewater is not overflowing
the field and the 1982 and 1985 well heads are intact.

The enclosed applie~ticn is resubmitted for issuance of a septic
upgrade permit for a 4 be~,dr¢om home usage on the above referenced lot,
in addition to an upgradel permit for a 3 bedroom trailer on the same
lot ·
The lol; currently has a seepage pit in the western secti, on {to be
called section A) that was installed with no inspection in 1983. The
owner has agreed to inst~ll a new leachfield and a ne~ soil log has
been performed. The are~ does not have a ground water problem and
therefore the existing seepage pit will be used as an inline grease
trap. The existing pit ~as installed to a depth of 14' and it is noted
that the pit contained 62" of standing water demonstrating the
effectiveness ef the und&rlying soil layers as a filter material. ThD_

The 2nd septic systdm will serve a 3 bedroom trailer on the
eastern side of the lot (~to be called section B). We are asking for

permit approval for section in advance of the 7 day monitor, with the
condition 'that the test hole water monitor be c. hecked at time of actual
installation, 6/30/86. The two trailers located on the original site
plan have been removed. .Each well, section A and B, will serve only
one residence, therefore ithey are private wells. The attached design is
recommended. ]

Request for Conditional COSA
The existing 3 bedroom house is served by a private well and septic system. The septic system consists
of a 42' x 36" x 72" effective depth trench.
MOA has issued permit OSP211001 to replace the septic tank draining this house. The scope of work for
this permit is to replace the septic tank at the house and backfill the seepage pit upstream of the new
septic tank.
In addition to this permitted work, the existing tank draining the old mobile home will be removed, and
the field abandoned in place.
There is an additional well located in the southeast corner of the property. It is labeled as 'abandoned'
on an older survey. The well casing was not located in December 2020 as the ground was frozen. When
equipment is on site to replace the septic tank, a further search for this well will be conducted and it will
be verified if it has been decommissioned per MOA water code.
The condition of the 1982 well is not known. The well casing is bent just above the pitless. The well
casing will be straightened and if water is found in this well and the water quality is sufficient, a new
pump will be provided and the water line will be routed to provide water to the house.
It is proposed that this repair work will be completed during the summer months when weather and
ground conditions are more favorable. The work will be complete by June 15, 2021. Money is being held
in escrow to complete this work by First National Bank. See accompanying letter.
At this time, we request a conditional COSA be issued for the sale of this property. Recent water samples
show that nitrates are absent from the 1985 well water on this property.
There is no eminent hazard created by granting this conditional approval. Wastewater is not overflowing
the field and the 1982 and 1985 well heads are intact.

Thank you for the opportunity to provide you with this estimate.
Changes which increase the specific tasks of machine/labor time spent of
this contract, will be submitted in writing by the owner and accepted by the
contractor and the contract modified accordingly. Payment to the
Contractor for modifications to this contract which increase the work will be
charged. All agreements are contingent upon accidents, design changes,
or delays beyond our control. Northern Excavation & Construction, (NEC)
shall not be held liable for the affects that may occur to the property during
and after the install. Affects such as damage and structural integrity of
decks or deck supports in or close by the work zone, trees left in close
proximity of where machine work takes place or having to stock displaced
excavated material around trees to be retrieved, damage and structural
integrity of retaining walls that may need to be partially removed or traveled
close too, disturbance to driveways, disturbance to any landscaping, etc.
All methods and materials shall be in accordance with MOA regulation.
Payment in full is due upon completion of backfill. Payments received after
30 days of completion shall be charged a 1.5% monthly fee on unpaid
balances.
