Box 240773 Anchorage, Alaska 99524-0773 Re: T 15 N, R 1 W, Section 9, Lot 1 -Chugiak-Final Operational Approval for two Domestic Wastewater Treatment and On lot Disposal Systems-ADEC Plan Tracking Number 5182 Dear Mr. Anderson: On December 7, 2007 the Department received the final documentation and record drawings for the development of two domestic wastewater treatment and disposal systems for the Liquor Store and the Grocery Store at the above location. The Grocery Store also has apartment units above the store (a total of 7 bedrooms), and an estimated use of 1,640 gallons per day. A 3,500 gallon two compartment septic tank and a deep trench type soil absorption system has been installed using an application rate of 0.8 gallons per day per ft2 determined from an in field percolation test. The use of 2-60 foot long by 9 foot effective depth trenches are providing 2,160 ft2 which is slightly larger than the 1, 640 gallons per day / 0.8 gallons per day per ft2 = 2,050 ft2. This development has the soil absorption system installed on Tract B, Stephens Subdivision, Addition # 1; this required a corrective easement to be established that allows the soil absorption system to continue to use the adjacent lot. The Liquor Store has.a new domestic wastewater treatment and disposal system installed at the above location. The amount of domestic wastewater used at the store "is estimated to be 850 gallons per day. Two 1,500 gallon septic tanks are installed. The soils are rated as 0.8 gallons per day per ft2 the absorption fields location, thus an area over 1,062.5 ft2 is required. The engineer's plans provided 2 - 50 feet long by 8 foot effective depth or 1,600 ft2 deep trench type soil absorption units. f } The Department has reviewed the documentation and record drawings for the subject project and per the Wastewater Regi_,,hAtioizs 18 AAC 72.240 final operational approval for the disposal and treatment systems are hereby issued. A "Certificate to Construct" for the wastewater systems is enclosed. Please use the above plan tracking number with any correspondence regarding these systems. This approval does not imply the granting of additional authorizations nor obligate any state, federal or local regulatory body to grant required authorizations. Any person who disagrees with this decision may request an adjudicatory hearing in accordance with 18 AAC 15.195- 18 AAC 15.340 or an informal review by the Division Director in accordance with 18 AAC 15.185. Informal review requests must be delivered to the Division Director, 555 Cordova Street, Anchorage, Alaska 99501, within 15 days of receiving the decision. Adjudicatory hearings requests must be delivered to the Commissioner of the Department of Environmental Conservation, 555 Cordova Street, Anchorage, Alaska 99501, within 30 days of the decision. The depth of a trench or pit is the distance between the surface of the ground and the bottom of the excavation(in feet). There is no set width for trenches. The gravel depth is the minimum depth of gravel between the outfall pipe and the bottom of the excavation(in feet). ~ ~ C.~)'~'~ * * REQUIRED SEPTIC(HOLDING) TANK SIZE = /~ GALLONS * * Permit applicant has the responsibility to inform this department during the installation inspections of any wells adjacent to this property and the number of residences that the well will serve. * * * TWO(2) INSPECTIONS ARE REQUIRED * * * Backfilling of any system without final inspection and approval by this department will be subject to prosecution. Minimum distance between a well and any on-site sewage disposal system is 100 feet for a private well or 150 to 200 feet from a public well depending upon the type of public well. Minimum distance from a private well to a private sewer line is 25 feet and to a community sewer line is 75 feet. Well logs are required and must be returned to this department within 30 days of the well completion. Other requirements may apply.
