Ref: Proposed Septic System for T1 5N, R1 W, SEC9, LOT 62 N2;
To whom it may concern:
The existing 3 bedroom house is served by a private well and septic system. The
drainfield is in a state of failure and needs to be upgraded. We are proposing to install a
5 -wide trench type drainfield and a new 1000 gallon septic tank. The two (2) existing
tanks and cribs on the property will be decommissioned per UPC. Comments regarding
the design are summarized as follows:
1. SOILS: See the attached soil logs which show the soil classifications, groundwater
monitoring, and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: As can be seen on the attached topography drawing, the average
topography around the proposed drainfield is relatively flat.
We are unaware of any adverse impacts this installation would have on adjacent wells

The length dimension is the length(in feet) of the trench or drainfield. The
depth of a trench or pit is the distance between the surface of the ground and
the bottom of the excavation(in feet). There is no set width for trenches.
The gravel depth is the minimum depth of gravel between the outfall pipe and

Permit applicant has the responsibility to inform this department during the
installation inspections of any wells adjacent to this property and the number
of residences that the well will serve.

Backfilling of any system without final inspection and approval by this department
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. well logs are required
and must be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.

be granted until the following items have been completed:
° The well casinry needs to be extended twelve (12) inches
above ground level.

leacliing area. This test will determine if the system is
adequate according to National Standards. A listing of
private firms performing the test is enclosed. This report
needs to be submitted to this office for our review.
° An outside water tap was not available. please call this
office to make arrangements to have an inspector to meet
you at the site.
Please notify this Denartment for a reinspection v✓hen the
noted discrepancies have been corrected. If there are any

An adequacy test was performed on the cesspools serving
the two three bedroom trailer housed located on this
property„ Both cess pools were charged with 1000 gallons
of fresh water_ and after- a period of 24 hours all the
water iah:ich had been added to the systems had percolated
away.
[t can be concluded from this test that these cess pools
are currently functioning adequately to be used as seepage
pits for the two three bedroom trailer houses. Tt will
be necessary however_, that you install a 1-000 gallon
septic tank between the trailer house and each of these
seepage areas.

In conducting this evaluation, GEG, LfD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD, can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.
