Septic System Design
Dear On -Site Services Engineer:
The holding tank on the subject lot is nearing the end of its useful life and must be replaced. We
are submitting this permit application for the construction of a new septic system. The existing
holding tank will be decommissioned in accordance with the Uniform Plumbing Code. The
attached site plan identifies the location of the home and the existing well and septic system along
with the proposed location for the new system. No conflicts exist between this proposed system
and any other well or septic system on this lot or adjacent lots.
The ground surface on the lot is flat in the area of the proposed trench. There are no slopes greater
than 25% within 50 feet down slope of the trenches. Ground surface contours are shown on the
site plan indicating the grade and direction of flow. Storm water drainage will not impact this
septic system. The new trench cannot be constructed parallel to the slope and will have a 2'
difference in elevation from the south to the north end. The trench is designed so the effective
absorption area is entirely in the receiving soils.
The percolation test completed on subsurface soils in 1983 indicated an absorption rate of 24
minutes per inch. A new test hole was recently placed and indicated similar soils, but with a slower
percolation rate of 60 minutes per inch. The absorption trench has been designed for the slower
percolation rate, but with a conservative application rate of 1.5 gpd/sf. Seeps were encountered in
the test hole at a depth of 6'. No accumulation of water was noted in the test hole during the
monitoring period and the seeps are probably surface water from recent rainfall filtering through
the area.
The proposed absorption trench has been moved out of the area of the old absorption pit. If the pit
is encountered during construction it will be removed as noted on the Site Plan.
The new system will be a minimum of 50' from all wells and surface water, 10' fiom any water
service and more than 5' away from the septic tank.
Please refer to the attached test hole log and plan and profile sheets for the septic design. If this
design is followed, there will be no adverse impacts to adjacent properties.

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and
entered into as of this Cn " Day of Qjpf L1a4�,f__ of 20A% by and between

Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
'1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),
described as Advantex
located at (legal description)

2. Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $600). "
Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement. .

repairs from the manufacturer's representative or maintenance provider,

assessed in accordance with AMC 14.60.030.

AWWTS. The Municipality will give at least 24-hour notice.

by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
—Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results. (Certificates may be reissued for a period of up to one year with valid water samples.)

The attached information explains some options (not many) that
HUD must consider. Alaska Environmental Control Services
cannot do any further work until a decision has been made and

Since we are being transferred to another area this will be
the end of my work on attempting to get this DEC approval. I
have spent over four months just getting this far with the

Attached .is a copy of the submittal to Alaska Department of Environmental
Conservation (ADEC) requesting approval of the subject lots Class "C" well which
includes a waiver request for the distance from the well to a neighbors sewer
system. Also attached is ADEC's denial of the waiver request. This means no
approvals will be given for the well. The waiver request. letter (Attachment '.1
of 10) explains the situation of the neighbors sewer system being installed too
close to the subject well. This makes the neighbor in violation of maintaining
required separation distances. The installation of this system was inspected
and approved by a Municipality of Anchorage inspector.
I discussed this with an engineer at ADEC and asked what suggestions they might
have to remedy the situation. They have no suggestions and indicated that they
will not make any due to not having the time to consider it. I explained what
the MOA does in a situation like this, of not penalizing the owner of b"
subject lot, but approaching the neighbor in violation and forcing them to solve
the problem they have created. ADEC's response was that they don't .have time to
do things like that, and also, because this neighbor is a single family house,
ADEC has no jurisdiction over private systems.
They say any solution must be presented by the engineer for review and approval.
I then discussed this problem with the }ICA, Department of Health & Human
.Services. They said if an offical compliant is filed with them against the
neighboring lot explaining that the violation of separation distances will not
allow for the necessary approval to sell the subject property, they will attempt
to force this neighbor to bring their sewer system into compliance,. This could
be a very time consuming process.
One possible solution would be to convert the duplex to a single family house.
The only other solution I can think of would be to drill a second well on the
subject lot, making two private wells, each serving one of the two units of the
duplex. Either of these would reduce the required separation distances to 100
feet and the existing well would need a waiver to the neighbor's septic tank.
This waiver would be requested through the MOA, and I believe the chances are
excellent.

Re: Lot 6, Thunderbrush Subdivision
The subject lot contains a 4 bedroom duplex served with a Class "C" well and a
4,000 gallon holding tank. The well was drilled in 1971 and is 199 feet deep
with a static level of 134.7 feet. In this particular area there is no known
bedrock at 199 foot depth, therefore, the well would be cased to the bottom.
A 4 hour well flow test was performed on 6/9/88. See attached data sheet: 5.3
GPM ws maintained with a total drawdown of 5.2 feet.
Storage Calculations:
Instantaneous Demand 8 x 2 x 20 = 320 gal.
Pump Yield 5.3 x 20 = 106 gal.
Storage Required 320 - 106 = 214 gal.
Existing Storage Tank- 40 + casing 82 = 122 gal.
Additional Storage Needed 214 - 122 = 92 gal.
With a total drawdown of 5.2 feet, the well is capable of producing a higher
instantaneous demand. The system has been working adequately since 1971,
therefore, we request you approve the existing storage until such time that
demand exceeds production.
From the attached site plan diagram, we find the separation distance from the
subject well to lot .6 septic tank cleanout pipe is 97 feet and to absorption
field standpipe is 113 feet. This system was installed on 9/15/80, after the
subject well was operating as a Class "C". To the north, lot 40B septic tank
cleanout pipe is 141 fee from the well. It ws installed 11/7/74, also after the
subject well was operating as a Class "C". At that time the required distance
was still 80 feet to the septic tank. We are requesting a waiver of the
separation distance to the lot.$ system. There is no well log for the subject
Drell. Neighboring well logs show various areas of hardpan, silt and clay from
the surface to 180 feet depth.
Water samples taken 6/9/88 show no sign of contamination. There are no other
sources of contamination within 100 feet of the well.
We request that you issue a Certificate to Operate for the subject well. If you
have any questions, please call.

Dear Mr. Savina:
We have completed an evaluation of the septic system and associated problems on
Lot 6, Thunderbrush Subdivision. The evaluation was undertaken at your request.
The findings of the investigation follow.
on-site sewer upgrade permit 830017, issued 1-06-83 by the Municipality of Anchorage,
specified that 720 square feet of absorption area (60' of 6' deep trenQh)._must..be
added to the existing system. This area requirement was based on a septic system
adequacy test performed by Alaska Environmental Control Services, Inc. on 11-17-82.
That test resulted in a daily absorption capacity of 200 gallons per_day.,_Qr.,400
gallons per day less than Municipality of Anchorage requirements for a 4 -bedroom
dwelling.
The excavator who was hired to install the upgrade recognized that the small lot size
and well locations at the site would make the upgrade very difficult to install
legally. He then contacted Arctic Engineers, Inc. (AEI) to perform -.a soil
analysis to Municipality of Anchorage specifications, with the hope of attaining
a lower soil rating and consequent smaller upgrade. The soil analysis and
pert test performed by AEI resulted in a soil rat;na f.230 squarefeet per
bedroom, in c ose agreement with t e previously established 240 square ft. per
bedroom. Additionally, AEI encountered groundwat 9feet below surface level,
which remained near that level during the observation period, 7/20 - 97_2T9_!_.__
The results of the soils testdidnot yieldany:significant reduction in the
size of the required upgrade.
Subsequent to the soil tests, all available information concerning the existing
property and septic system was assimilated and plotted on the attached site
plan, page Al. That drawing shows the surveyed -in house and lot corners,
septic system standpipes and well, from a 3-20-74 "As -Built." Additionally,
the drawing shows the approximate locations of the underground septic system
components as found in the records at the Municipality of Anchorage, Department
of Health and locations of adjacent wells determined by AEI.

AEI was-alsorequested to perform -a septic system adequacy test to clarify the
results of the previous adequacy test. In that test, the surge capacity was
given as 10 allons, and the daily a orltion c 't deter be
gallons per ay. No information was included in that test report which
indicated ow the test was performed or how the results were determined. The
surge capacity was suspected to be innaccurate by AEI because the standpipe
at the crib.had just a very.. small hole into the crib. If water was input at that
point, it would require 10 gallons to fill the pipe- giving misleading surge
capacity data. AEI chiselled out the opening into the crib prior to performing
the test and achieved a substantially different surge capacity of 350 gallons.
The dailv absorption capacitv determined by AEI was 260 qallons per day. The
adequacy test report is included with this report. ��
The attached site plan along with the attached excerpt from the Munici a ity
of Anchorage, Dpeartment of Health and Environmental Protection on-site
wastewater system design manual, page A2, shows that with the existing
requirements for clearances to wells, no upgrade is possible. However, if
variances could be obtained onlv_. on the distance from ,well B, a class C semi-
public well, and distance from a driveway, the cross -hatched "possible upgrade

The necessary variances would be 100 ft. to well B and the upgrade would need to
go under driveways on the south portions of the property. Those parts of the
system would require insulation.
'• The only other alternative which exists, as defined by Dr. Robert Robinson.of
Municipality of Anchorage, Department of Health and Environmental. Protection,
'would be to abandon the existing absorption system and install a 4000 gallon
holding tank. This would need to be emptied periodically and would/result in /

According to the Municipality of Anchorage, Water & Wastewater Utility, there
are -no plans to provide that area with public sewer service.
The possible upgrade area is about 55 feet long, determined graphically from
page Al. From this determination, the calculations on page A3, were computed
to determine the maximum system size which the subject lot could support. Two
types of systems were evaluated, these being drainfield and trench designs, to
meet the Municipality of Anchorage requirements for clearance to ground water
as highlighted on page A2.
The resultwas that a maximum of 3.64 bedrooms could be provided for with a 4 foot!
deep trench upgrade. This system. would require a lift station and insulation
as shown on .page A4. The drain field (shallow trench) design resulted in a
maximum of 3.44 bedrooms, which would be constructed as also shown on page A4.
Both designs would probably require relocation of existing underground utilities
(i.e. natural gas service lines).
It is realized by AEI that the proposed system does not meet the full Municipality( -
of Anchorage standards, and that all figures used assume optimum conditions -
soil ratings and distance requirementsarepushed to the limits. However, when
the proposed system is compared to a holding tank system, it seems justifiable.
A well depth of 199 ft., previous satisfactory fecal coliform analyses, and tYeF�,
soil conditions on the property indicate that the proposed upgrade should hav
no adverse effects on the well serving the property, nor on neighboring wells,
AEI was also requested to make comments on a --letter addressed to Colonial
Mortgage Service Company Associates, Inc., dated August 26, 1983, from John
Gustason, Brian Walimaa and Joanne Walimaa, which is attached to this report.
Referring to item 3, "Raw Sewage is Seeping...", this is unproven - it is our
opinion that that water is scummy surface water, because at the time we examined
the crib the water level in the crib was several feet lower than the water
level on the surface. Proper landscaping so that surface waterwill be diverted
from the crib would eliminate that problem, and may improve performance of the
absorption unit. Item 6 is similarly unproven.
We were also requested to comment on any alleged damage to the absorption unit
caused by Mr. Savina operating a backhoe near the soil absorption system during
the spring of 1983. We could .find no evidence that any damage other than
aesthetic damage to the ground surface occurred.

DESIGN OF ON-SITE
WASTEWATER DISPOSAL SYSTEMS
1. General
The options for a standard on-site wastewater disposal system
include deep trench, drainfield (shallow -wide trench) and
bed. The option selection will be governed by site con-
ditions, available material, and economics. Of these, site
conditions will usually be the most critical, and certain
guidelines and limitations must be standardized and applied
to all designs. These standardizations are defined in the
following paragraphs.
2. Location: The configuration of the lot must be such that at
east two replacement systems may be installed. The minimum
distances required from a drainfield are as follows: (a)
individual single family well = 100'; multi -service well -
Class C - under 25 service of less than 15 connects = 1501;
multi -service well - Class A/B - over 25 service or more than
15 connects = 200'; (b) lake, river, stream or major surface
drainage course = 100'; (c) any other existing or anticipated
drainfield = 301; (d) lot or property line = 10'; (e)
building or structure = 10'; (f) driveway or vehicle storage
or parking area = 10'; (g) water main or service line = 101;
($) septic tank = 5'. The drainfield must be installed
within 10' of the soils test site.
3. Slope:� The slope of the area in which a drainfield is
installed may not exceed 25% (14 degrees). Seepage bed
designs must be limited to areas having 2% or less slope.
These slope limitations must also be applied to the areas(s)
set aside for the two replacement systems. Trench or drain -
field systems must be designed and installed so as to conform
to the natural slope or contour of the area, and across
slope. The grade of distribution lines within a trench,
drainfield or bed system must not exceed 6" per 100' of
length. If distribution lines are not used, the same grade
limitation applies to the trench or bed bottom.
4. Ground Water and Bedrock: In all system designs, the minimum
depth from the bottom of the trench or bed to ground water
or seep shall be 41. Should there be seasonal fluctuations in
ground water depth, the shallowest depth to ground water
shall apply. The minimum depth from the bottom of the trench
or bed to bedrock or fractured 'bedrock must be 6'..,,

PROCEDURE: The occupants were instructed to minimize water use prior to the test.
Testing was begun at 1:00p.m. on 9-7-83. 350 gallons of water was added
to the standpipe at the crib when itbegan backing up into the septic
tank. Water was input in 100 gallon increments, withliquid rise
measured between successive inputs.
When the crib became full, addition of water was halted, and the septic
tank was pumped to isolate the absorption system. Absorption monitoring
was then carried out over a 3 -hour period. Data from monitoring was
plotted on the attached plot of absorption vs. time.

The seater analysis report needs to be submitted to this
office from the Chem Lab, 5633 B Street, for our review.
° The septic tank pumped with a receipt submitted to this
department.
° An adequacy test needs to be performed on the existing
leaching area. This test will determine if the system is
adequate according to National Standards. A listing of
private firms performing the test is enclosed. This report
needs to be submitted to this office for our review.
Please notify this Department for a reinspection when the
noted discrepancies have been corrected. If there are any
further questions, please call this office at 264--4720.
