Dear On -Site Engineer:
Forge was hired to obtain a permit for a tank replacement at the subject address last year. One of
the provisions of the permit mentions that a waiver may be required for reduced separation between
the existing field and the garage foundation before IR approval. We were able to confirm at
construction that the field is indeed about 5.5' from the garage foundation. Therefore, we would
like to request a waiver for 5.5' separation from the existing trench to the garage foundation.
The existing trench has been in continued use since 1978 and is still operational today. The garage
was built in 1979 according to property records and has not been influenced in any way by the
drain field thus far. Since there has been no adverse effect on the foundation or drain field since
they were both constructed, we expect no adverse impacts in the near future.

Subject: Timberline, Lot 1 – 10801 Trails End Road
Septic System Design
Dear On-Site Services Engineer:
The septic tank on the subject lot has failed and must be replaced. The owner has requested the
system be upgraded to service a four bedroom home. We are submitting this design and permit
application for the construction and placement of a new 1,250-gallon septic tank and an extension
to the existing absorption trench. The attached site plan identifies the location of the home and the
septic system along with the new tank location and absorption trench extension. The existing well
on the lot is also shown. No conflicts exist between this proposed tank and any other well or septic
system on this or adjacent lots.
The new tank and absorption trench will be constructed a minimum of 100’ from any existing
wells. It will also be 100’ from surface water and more than 5’ from the existing absorption trench.
The new tank will be 10’ from the house foundation and more than 5’ from any deck or stair
supports.
The absorption trench extension was designed with test hole and percolation data from the original
construction. We request a condition to the permit requiring a test hole and percolation test prior
to construction to verify the design parameters.
Please refer to the attached plan sheets for the septic design. If this design is followed, there will
be no adverse impacts to adjacent properties.

Dear Mr. Buchholz:
Please find attached the completed Soils L0g and Percolation
Test form for testing performed on June 5, 1978. DrilliFg
was done by Denali Drilling Co. using a six inch auger.
Soil samples were obtained from the test hole to a'depth of
20 feet and classified by visual examination every 5 feet.
A percolation test was then conducted as defined in the
Manual of Septic Tank Practices (Pub. No. 526, U.S. Dept.
H.E.W.) page 6.
The percolation rate was found to be 20 minutes per inch.
Therefore, an absorbtion area of 210 square feet per bed-
room is recommended.

Dear Mr. Bucholtz;
This letter is to confirm our phone conversation of October 20, 1978. AS
stated to you, Ted Forsi and Associates had earlier that day, investigated
the alleged ground water problems which delayed the installation of the
septic system at Lot 1, Timberline Subdivision.
After evaluating the water which was observed during the initial excava-
tion of the trenches and considering the conditions, we feel that the
original soils log and percolation test is adequate and that the water
present was surface runoff originating from the recent precipitation.
This surface runoff should not pose a detriment to the operation of the
proposed septic system[
