PROPERTY OWNER AGREEMENT
FOR THE MAINTENANCE OF AN
ON-SITE WASTEWATER DISPOSAL
SYSTEM

Anchorage Department of Health and Human Services (DHHS) and the property
owner(s) of:
This agreement is made for the purpose of maintaining an on-site wastewater disposal
system on the subject property.
The property owners agree to the following:
Allow the Municipality of Anchorage the perpetual right of entry to the property during
normal working hours, to allow for effluent sampling or evaluating the general state of
repair or function of the system.
Submit to the Municipality of Anchorage, on an annual basis, an inspection and
operation statement from a registered professional engineer. This inspection and
operation statement shall verify that the engineer has inspected all effluent and air
pumps, timers, and alarms, and that any deficiencies have been repa/red and that the
system is functioning as designed.

Dear Homeowner/Prospective Buyer:
The on-site wastewater disposal system you are purchasing/installing is an "alternative" wastewater
disposal system. This system, known as a "Recirculating/Upflow Filter Septic System", is undergoing
testing within the Municipality of Anchorage under the Alternative System section of the Wastewater
Disposal Regulations (AMC 15.65). There are certain risks involved with the ownership of one of these
systems:
1. The technology used in this system has been shown to be effective in other areas. The system is
currently undergoing a two year testing period in Anchorage under the guidance of the Department of
Health and Human Services (DHHS) and the State of Alaska Department of Environmental Conservation
(ADEC) to determine its effectiveness in a subarctic environment.
2. Changes to the Wastewater Disposal Regulations (AMC 15.65) through Municipal Assembly action
will be necessary prior to this system being approved as a standard system.
3. The system for this property received a vertical separation distance waiver from both State of Alaska
and Anchorage Municipal Codes to ground water. This waiver was granted due to the system's expected
performance within the site conditions on this property.

Well and Septic System Permit
I have revised the shallow bed system proposed for Lot 5, Block H, Timberlux
Subdivision to an elevated mound system in accordance with our conversation of April 11.
The groundwater level in Testhole No. 1 has recently risen to within 5' of the surface as
opposed to 7.5' below the surface as determined on our last measurement of March 25.
The system is now designed for a maximum water level of 2' below the existing ground
surface.
The owner of the lot has also agreed to install a 240' long X 24" diameter culvert in the
ditch fronting the lot to assure a minimum of 100' separation from any surface water. The
ditch is now dry and apparently remains so much of the year. It is, however, a drainage
course and, as such, considered surface water. The culvert will be placed during the
installation of the septic system and will follow the contours of the existing ditch.
All other aspects of the design package remain the same. The total dynamic head will
increase by 4' as a result of raising the elevation of the distribution piping. The specified
pump, however, will be able to deliver the effluent through the distribution piping at nearly
5' of head even with the additional elevation. No adjustment is required.
Please review the attached system profile and advise if additional information is required
before a permit can be issued for the construction of a well and septic system on Lot 5,
Block H, Timberlux Subdivision.

Impacts to Adjacent Properties
Dear On Site Services Engineer:
We hereby apply for a permit to construct a well and onsite septic system
on Lot 5, Block H, Timberlux Subdivision. The attached site plan and
backup documentation identify the size and location of the new system as
well as the proposed location of the well to serve the house to be
constructed on the lot. Groundwater was located at a depth of 7.5' below
the surface so a pressure system has been designed to assure a separation
distance of 4' from groundwater.
A percolation test completed on the soils in the area of the new
absorption beds indicated a rate of between 15 and 30 minutes per inch.
We propose to place a 15' wide absorption bed with a total length of 100'..
Total depth of the system will be 2' below the surface. A 1,500 gallon
S.T.E.P. tank and pressure distribution system will be installed. The
drainage patterns on the lot will not be affected by the construction of
the new system. The ground surface slopes at a gradual 0 to 2% slope
from the northwest to the southeast.
If the system is constructed as designed the following statements can be
made:
The system, if constructed as designed, will have no adverse impact
on the wells in the area or those to be constructed in the future.
The system, if constructed as designed, will have no adverse impact
on existing septic systems in the area or those to be constructed in
the future.

The system, if constructed as designed, will have no adverse
impact on reserved space, either surface or subsurface, on any lots
located in the area.
The system, if constructed as designed, will have no adverse impact
on drainage patterns in the area. The current drainage pattern will
be maintained.
Some surface grading may be required to assure a minimum of 100'
separation between surface water and all components of the septic
system. Grading and filling will not impact the current drainage patterns.

CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: Lot 5, Block H, Timberlux Subdivision
GENERAL:
The scope of this project includes the procurement and
installation of a 1,500 gallon S.T.E.P. Tank with a lift
station and the construction of a total of 100 Linear Feet of
15' Wide absorption bed. Surface grading may be required to
assure a minimum of 100' separation between surface
water and any component of the septic system.
Construction shall be in accordance with the approved site
plan, design drawings, Municipal Permit with any special
provisions or conditions, and all applicable State and
Municipal Wastewater Disposal Regulations.

be responsible for final grading areas subsequently
depressed from soil settling. The property owner shall be
responsible for revegetation of affected areas unless
specifically agreed otherwise.
Contractors installing wastewater disposal systems must
be certified by the Municipal Department of Health and
Human Services for system installations. Owners installing
their own systems must receive prior approval from D.H.H.S.
before beginning system installation.
LIFT STATION INSTALLATION
1. The lift station is to be constructed by a certified tank
manufacturer. Construction shall include an 18" manhole
for access to the lift station.

The lift station shall be sufficiently bedded to prevent
settling or shifting of the tank.
All standpipes on the lift station shall extend a minimum of
12 inches above final grade.
4. Lift stations installed without 4' of cover shall have a
minimum of 2" of direct burial insulation.
A foundation cleanout shall be installed one to four feet
from the building foundation. No cleanouts are required
between the lift station and the drainfield in a pressure
distribution system.
6. Final grading over the lift station shall be such that a
positive slope exists away from the septic tank.
ABSORPTION BED CONSTRUCTION:
The absorption bed shall be constructed to the dimensions
shown on the design. The bottom of the bed shall be within
2" of level.
Distribution piping must be placed level with perforations
down atop a level bed of drainfield rock. Rock should then
be placed over the pipe to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between
the drainfield rock and the natural soil backfill.
Monitor tubes must be 4" in diameter and installed at the
locations shown on the design. The portion in the drainfield
rock must be perforated.
5. Direct bury insulation must be placed over the distribution
system when less than 3' of backfill depth is available.

Finish grade over the bed must be mounded to prevent
settlement or depressions.
MATERIAL SPECIFICATIONS:
1. The lift station must be constructed by a Municipally
approved septic tank manufacturer. An Orenco 20 OSl 05
HHF - 5 is recommended.
2. The following pipe materials are approved for use in septic
system installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C.
(perforated and solid), ASTM F810 or H.D.P.E. (perforated, but
not solid) and ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial
polystyrene (Dow Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with
watertight couplings (Caulder, Fernco, or equal).
A permeable geotextile fabric (Typar, Mirafi or equal) must
be installed between the final drain rock layer and the
native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than
3% passing the #200 sieve.
INSPECTIONS:
A minimum of two inspections are required by Municipal
Ordinance. These inspections must be conducted under the
supervision of a professional engineer registered in the State
of Alaska. The first inspection must be conducted after the
excavation of trenches, beds or pits and before the installation

of any gravel. A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement
of the geotextile fabric, gravel, distribution piping,
standpipes, cleanouts and insulation. No backfill should be in
place at the time of the second inspection.

We have not received final word from ADEC conceming the issuance of a waiver between
the absorption field and the ditch fronting Lot 5, Block H, Timberlux Subdivision and are
assuming it will not be approved. We have therefore decided to install a mcirculating lift
station septic tank with trickling filter and upflow filter and are requesting a revision to the
permit originally issued for this property. This system as manufactured and supplied by
An6horage Tank has been approved for use in new construction. In addition, the use of
this system requires a much smaller absorption bed and allows a minimum separation of 2'
to groundwater.
The absorption bed designed for use with this new system will be 15' wide and 20' long.
The accepting soil percolates at less than 30 minutes per inch, but we are designing the
system for the slower percolation rate to provide additional absorptive area. The
distribution piping will be placed at 1' above original ground which will provide nearly 5'
of separation from the highest measured groundwater on the lot.
The alternate site is shown on the attached site plan and is within 100' of the ditch. If it
becomes necessary to use the alternate site the ditch will be encased in culvert to provide the
necessary separation. Sufficient area is available between Testholes No. 1 and 2 to provide
more than 80' separation from the ditch.
Please review the attached information and revise the permit accordingly. The owner is
anxious to move on with construction of the septic system.
