CERTIFICATION & STATEMENT OF INSPECTION BY ENGINEER As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines, indicates that the on-site water supply and/or wastewater disposal system appears to comply with applicable Municipal and State codes, ordinances, and regulations in effect at the time of installation, unless noted otherwise. Name of Firm FIRST WATER CONSULTING Phone 907-350-9566 Engineer’s Printed Name CURTIS HUFFMAN, PE Date 07/16/2026 Comments: This investigation was completed in compliance with MOA guidelines, regulations, and best industry practices / methods. The assessment of the condition of the well and septic applies only to the conditions as of the day tested. The flow and absorption rates may change due to subsurface conditions that may not be observed from the surface, changes in land use, local soil characteristics, groundwater levels that may fluctuate during the year, quality of construction (workmanship & materials), the water usage of the family being served by the system and maintenance. The operational life of all well and septic systems are subject to these various and dynamic characteristics and are outside the control of the evaluator of the well and septic system. Therefore, any or NO estimate of how long a system will function satisfactory for current or future occupants or guarantee that no unseen encroachments, deficiencies or discrepancies exist can be given by First Water Consulting & 07/16/26 13030 Sues Way, Anchorage, AK 99516 907-350-9566 / FirstWaterAK@gmail.com July 16, 2026 Municipalities of Anchorage On-Site Water & Wastewater Program 4700 Elmore Road Anchorage, AK 99507 RE: WAIVERS –WELL TO SEWER LINE & MANHOLE (MH) LEGAL: TURNAGAIN BLOCK 3, LOT 6 - FORTY-NINE TOWNHOMES PHYSICAL: 1118 BYRA COURT, ANCHORAGE, AK 99515 The subject private certified well was drilled on January 12, 1964 with subsequent Alaska Department of Conservation (DEC) approval along with a sewer line installed in 1988 by Anchorage Water & Wastewater Utility (AWWU). No DEC waiver could be found by AWWU…and this well is now considered or designated as a private well serving 4 total units of the Forty-Nine Townhomes. The existing manholes (MH) of the sewer line installed in 1988 have been located with the sewer line approximately shown between these two MHs and combined with the recent as- built survey by Buku Saliz, LS. Based on these findings, it was determined that the private well to MH center and sewer line center are approximately 122.3’ & 68.2’ respectively. Therefore, we are respectfully requesting MOA waiver approval of the private well to the MH of 120’ and of 66’ to the sewer line be granted at this time. Granting of these separation waivers is warranted since there have been no known issues for the past several decades nor anticipated, the recent colli, arsenic & nitrate (CAN) water samples are non-detect (ND) / clean and the interior well is protected from any outside potential effluent interaction. Per these justifications, MOA waiver approval of the private well to the MH of 120’’ & the sewer line of 66’ are respectfully requested to be granted at this time. Please contact us if you have any questions. Sincerely, Curtis Huffman, P.E. Municipality of Anchorage P.O. Based on review of the submitted data it has been determined that the proposed subject water system is classified as a NON-PUBLIC WATER SYSTEM. SYSTEM CLASSIFICATION: NON-PUBLIC SERVICE & POPULATION INFORMATION USE Apartment — 2 Buildings W/ 4 units each W 3 bedrooms NUMBER OF SERVICE CONNECTIONS 1 existing, +1 proposed NUMBER OF DAYS PER YEAR OF OPERATION Year-round Residents RESIDENTS (PRIMARY PLACE OF ABODE) 24 NON -TRANSIENT 0 TRANSIENT 0 The potable water well is located on Tumagain Blk. 3 Lt. 6 and serves an existing structure will serve an additional proposed structure. Under 40 CFR 141 and 18 AAC 80, based on the limited population served by the subject water system, it is classified as a Non -Public Water System. You are required to inform the Drinking Water Program if the population served by this water system and appurtenances increases to >_25 persons per day for >-60 days a year or if the number of services connections increases to >_15. The State's DWP does not regulate Non -Public Water Systems. More information concerning Non -Public Water Systems can be found at: htti2://dec.alaska.gov/eh/dw/index.htm. Records pertaining to this system will be filled for future reference and shared with other agencies or jurisdictions which may regulate your system. While not required to be submitted to the DWP, it is recommended your potable water system be routinely sampled for bacteriological contamination, nitrate, arsenic, and other chemical contamination. Appeal Process Any person who disagrees with this decision may request by mail, e-mail, or facsimile an informal review in accordance with 18 AAC 15.185 or an adjudicatory hearing in accordance with 18 AAC 15.195-15.340. CC.: BDMG Ventures LLC 8921 Toloff St. Anchorage, AK 99507 190307_Tumaga BLK 3 LT 6_Non-Public_CL.docx Broaddus Street 4-Plex COE, ADEC-' February 10, 1987 Page 2 central laundry facility. The building was originally constructed in 1964 as a single family residence, and was subsequently converted to a four-plex. Presently, the units are occupied by single adults. Ms. Yan has indicated that in the five years she has owned the building she has never rented a unit to a couple. She has not received any complaints about water pressure during that time period. In 1983, sewer service was extended up Daryl Avenue and Broaddus Street. Lots five, six and seven connected to city sewer. Lots five and seven border Ms. Yan's lot on the east and west. On -site waste disposal systems were disconnected and abandoned at that time. Well and septic locations were made visually on lots eight, nine, and ten which lie across the alley behind Ms. Yan's property. Septic systems on the lots are located on the Ramona Street side of these lots and do not violate the 150 foot radius required for a Class "C" well on Ms. Yan's lot. The pump in the well is one-half horsepower of unknown make and model. Pumping through 98 feet of 3/4 inch diameter hose; the pump produced 10 gpm. While this is more than adequate to meet the 24-hour demand of 150 gallons per unit, it is far short of the 8 gpm per unit, 20-minute demand requirement. These requirements are general and are formulated for newer, larger units which likely would house at least two people, and have a garbage disposal, dishwasher, and washer. If we make allowance for the fact that there is one resident per unit, and then adjust further for no washer or dishwasher, the 2.5 gpm that is presently available does not seem unreasonable. This assumption is supported by the lack of complaints from the tenants. If you have any further questions, or need additional information, please do not hesitate to call. Sincerely, eA Richard C. E. Attention: Steve Eng Dear Mr. Eng: RECEIVED FEB 11 1987 ANCHORAGE/WESTERN DISTRICT OFFICE I have been retained by Ms. Jushin S. Yan to furnish all information required by you and your department to issue a Certificate to Operate for a well located on her property, legal description: Lot X, Block 3, Turnagain Subdivision. The legal owner of this property is Ms. Jushin Yan, P.O. Box 90719, Anchorage, Alaska 99509. The information furnished with this letter is based on the requirements set forth in your publication "Suggested Practices for Small Water Systems" and a brief conversation I had with you in your office on Wednesday, February 4, 1987. Included with this letter please find the following attachments: 1. As -built, Lot 5, Block 3, Turnagain Subdivision. 2. As -built with adjacent buildings and sewer locations shown. This is a composite of information gathered from several sources listed below. I have drawn the buildings in there approximate location on a copy of the as -built for your information. 3. As -built of Turnagain Sub Sewer - Hanes Street/Daryl Avenue, with existing well radius' superimposed. I have added two wells to this plan based on visual confirmation and M. 0. A. records. These wells are located on lots 9 and 10 across the alley from Ms. Yan' s lot. 4. Copy of original well log. 5. Pump test information. 6. As -built schematic of water system. The well in question serves a four-plex. The units are small with one bedroom, averaging approximately 595 square feet each. They each contain a kitchen sink, lavatory, water closet, and tub/shower combination. None of the units have garbage disposals or dishwashers. The building presently has no provision for a 200 West 34th, #267 • Anchorage, Alaska 99503 • (907) 274-7611 Broaddus Street 4-Plex COE, ADEC February 10, 1987 Page 2 central laundry facility. Yan' s property. Septic systems on the lots are located on the Ramona Street side of these lots and do not violate the 150 foot radius required for a Class "C" well on Ms. Yan' s lot. Sincerely, Richard C. A, 1388C Anchorage, Alaska Subject: Soil Investigation for Sanitary Sewer System, Lot 6, Block 3, Turnagain Subdivision, Anchorage, Alaska Dear Mr. Sleininger: At your request of February 4, 1980, we conducted a snbsurface soils investi- gation at the proposed location of the sanitary sewer system on the subject lot. The investigation complied with those procedures required by the Municipality of Anchorage Department of Health and Environmental Protection. This ~nvestigation, which was accomplished on February 13, 1980, consisted of a test hole drilled to a depth of 20 feet below the existing ground surface. The test hole was sited according to your instructions and its location is shown in attached Drawing A-01. Drilling was accomplished with a rotary drill rig using continuous flight solid-stem auger with an outside diameter of 6 inches. A sample was taken at the depths shown on the soils log in Drawing A-01. The sampl~ will be held in storage at our lab for approxi- mately six months. In addition, all material brought to the surface by the augers was continuously monitored by an experienced engineering geologist. The topography at the drilling site is generally horizontal. At the time of the investigation the site had original vegetation consisting of birch and spruce. The top of the test hole was located at original ground surface. February 13, 1980 Robert $1eininger Page -2- The soils encountered in the bore hole are shown in the test hole log in Drawing A-O1. This log displays specific conditions encountered at the test location. However, subsurface conditions may vary in other parts of the lot without any apparent surficial evidence of the change. Groundwater was not' encountered. Bedrock was not encountered. At the time the hole was drilled seasonal frost was present from the surface to a depth of 1.5 feet and permafrost was not encountered. Based on the visual classification of the soil and the requirements set forth by the Muncipality of Anchorage, a percolation test was not necessary within the test hole on the subject lot. We appreciated this opportunity to be of service to you. Please contact us if you have any questions concerning this letter or if we can be of addi- tional service.
