Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).

shall be either:

b. Covered, sealed, and heated to prevent freezing

The owner of the 2 -bedroom home would like to replace the existing holding tanks with one (1) new
3,000 -gallon holding tank. The existing foundation cleanout is inside the house, so a double cleanout will
be installed ahead of the tank. The existing holding tanks do not have an alarm system. A new alarm
system will be installed along with the new holding tank.
There are no anticipated probable adverse impacts to adjacent properties if the holding tank upgrade is
constructed as designed.

2. All property lines and easements within 20 feet of the proposed
sewage holding tank shall be flagged by a licensed surveyor.
3. (E) Holding Tanks: Decommission in place or remove in
accordance with the code.
4. E Crib: The undocumented abandoned log crib was discovered
while digging soils test TH1. The extent and exact location is
unknown and may possibly reach into the proposed holding tank's
footprint. The contractor shall ensure the holding tank is not bedded
on any part of the old crib.
5. (P) Holding Tank: Epoxy coated steel 3,000 -gallon septic tank.
6. Buoyancy Anchors:

Subject: Jim Jackson, Percolation Test
Two test pits were excavated on the subject property. The first of these,
labeled #1 on the site plan, was abandoned due to water entering the pit from
near surface drainage, some two feet below existing grade, and wet soils
conditions. #2 test pit was excavated to the west, several feet. The #2 pit
was then filled in and an adjacent trench, some four feet in depth was excavated
for purposes of conducting percolation tests. The following day percolation
tests were run in three test holes excavated in the bottom of the trench. The
attached results show that no percolation was achieved.
Based on visual examination and test information, we suspect that the problem is
a combination of both tight moist to wet soils and near surface drainage.
Accordingly, the placement of a new system will not resolve the problem.
If it is decided to attempt to construct a workable system, we would first
recommend that the near surface drainage be channeled away from the leach field
area. One method to accomplish this would be by the construction of intercepter
ditches,upslope and on either side of the proposed system. These could either
be open ditches or covered, gravel filled trenches, with or without perforated
pipe or drain tile. The 'trenches should be at least six inches to one foot
low the base of the seepage, with adequate slope to drain toward Cox Drive.
Drainage along Cox Drive should also be improved, either toward the existing
CMP or to the west, providing that a culvert is placed beneath the existing
driveway.
Following improvement of the near surface drainage, we would then recommend
pumping and frequent monitoring of the existing system for 'improved performance,
prior to installation of a new drain field. As an alternative, a new drain
field could be constructed immediately, keeping in mind that performance cannot
be guaranteed.

We are requesting a conditional COSA approval be issued for the subject property, which will allow
completion of the home sale prior to completing the required repair/upgrade to the septic system.
Septic System: The two existing steel holding tanks were installed in the early 1980's and need to be
upgraded. The following items satisfy the conditional approval requirements.
1. Holding tank upgrade permit OSP211004.
2. At least two contractor's bids to complete the upgrade construction in accordance with the
approved permit have been obtained.
3. A letter stating there's an amount of money equal to 1.5 times the highest contractor's bid in an
escrow account that is for the specific purpose of completing the upgrade construction in
accordance with the approved permit.

balance of Escrowed Funds held has been escheated to the State, it may be necessary for the Parties to file a
claim with the State in order to receive any of the Escrowed Funds deposited with the applicable state agency or

harmless from any loss or damage arising from or pertaining to the Parties` conduct, requirements, action or
inaction regarding the matters addressed herein or for any other loss or damage that is not caused by Settlement
Agent, The Parties acknowledge and agree that Settlement Agent's sole responsibility arises under the
instructions given to Settlement Agent. The Parties further expressly waive any special, incidental, punitive or
consequential damages arising out of or In connection with these instructions to Settlement Agent and that
Settlement Agent's liability is limited to and cannot exceed the amount of Escrowed Funds held.
Close of Escrow: Except for the Parties',obligations under the preceding paragraph (which shalt survive), the
escrow and this Agreement shall autornatically.terminate upon Settlement Agent's disbursement of the Escrowed
Funds pursuant to the terms of this Agreement. If Settlement Agent has not received written mutual Instructions
from the Parties concerning disposition of the Escrowed Funds, or written notice of a dispute from one of the
Parties, by the date that is three (3) months, after receipt of the Escrowed Funds or the Estimated Completion
Date listed above, whichever is later, Settlement Agent has the right, but not the obligation, at any time
thereafter to dose this escrow by issuing one check In the amount of remaining Escrowed Funds to the Party that
delivered the Escrowed Funds to Settlement Agent without additional notice to or Instructions from the Parties.

Dear Sirs:
By means of this letter we are requesting a waiver down to 83 feet from the
required 100 foot separation distance between the well on Lot 1, Block 1, Terrace
Heights S/D and the septic tank on Lot 7, Block 1, Kimberley Manor S/D. We are
also requesting a waiver of the well-soil absorption trench distance down to 95
feet. A site plan and copies of relevant documents from your files are enclosed.
No driller's log exists for the well serving the subject lot, but it was probably
drilled in 1970 since the tax assessor's office reports the residence was constructed
in that year. I did find the enclosed well log for Lot 7, Block 1, Kimberley Manor
S/D which is located approximately 90 feet west of the subject well, and thus
should represent similar geologic conditions.
On August 7 the static water level in the subject well was 118 feet below
the top of the casing. I tested the well yield by pumping 572 gallons of water from
the well at the maximum pump output of 5.7 gpm. This caused the water level in
the well casing to be drawn down to 141 feet, but no further, indicating that the
well yield is at least 5.7 gpm. I suspect that the total depth of the well is only a
little more than 141 feet, because extended pumping at this level of drawdown
caused the water to become temporarily turbid. For this reason I believe that the
aquifer being tapped by this well is the one identified on the L7, BI, Kimberley
Manor S/D well log as being between 142 and 144 feet below ground level, and I
doubt that this well extends down to the second aquifer at 302 feet being utilized
by the Kimberley Manor well.
The L7, BI, Kimberley Manor well log indicates primarily clayey soils
between 3 and 48 feet, below which a three foot thick water bearing gravel seam
was encountered. Then another clay and gravel stratum was encountered between
51 and 135 feet in depth, before the aquifer in question was encountered. Based
on this log, it appears that the aquifer is well protected from contamination by the
presence of the overlying clay strata. Water samples taken on August 3 support
this hypothesis, showing 0 coliform bacteria and a non-detectabl(: level of nitrates.
The earliest record of the wastewater disposal system for L 1, B l, Terrace
Heights is an adequacy test report by R&M Consultants in 1980 stating that the
system was failing due to shallow groundwater. Accordingly, a holding tank was
installed in 1980, and this was subsequently upgraded in 1982 and 1983 to it's
present configuration of two 2000 gallon tanks. The cleanout pipe for the closest
holding tank is approximately 99 feet from the well, which exceeds the required 75
foot separation distance. At our request, Carl's Excavating is in the process of
installing a high level alarm to bring the holding tank system into compliance with
today's code requirement, so that it can receive Health Authority Approval.
In 1978 a wastewater disposal system was installed on Lot 7, Block 1,
Kimberley Manor S/D, consisting of a 1250 gallon septic tank followed by 43 lineal
feet of soil absorption trench containing 8 feet of gravel. The soils test prepared
prior to construction of this system showed no groundwater and a percolation rate
of 125 square feet per bedroom in the designated absorption stratum. On August 7
I measured the distance from the cleanout pipe on this septic tank to the subject
well to be 86 feet. The requested waiver distance of 83 feet is to allow for
underground projection of the tank. The nearest cleanout pipe in the soil
absorption trench is 112 feet from the subject well, but by extrapolation from the
other cleanout and 'the as-built record, I believe it is possible that the closest
portion of the trench may be within 95 feet of the well.
The surface topography in this area slopes to the south at approximately 5%
meaning that, should the septic system fail, any surfacing effluent would tend to
flow directly away from the subject well. One can hypothesize that the underlying
soil strata also parallel the surface topography, resulting in all subsurface effluent
will also tend to be carried away from the well. Since this septic system is on a
separately owned piece of private property, there is no way for me to assess it's
present adequacy. It is my professional opinion, however, that surface topography
coupled with the relatiYely impermeable subsurface strata overlying the aquifer,
ensure that the septic system in question cannot contaminate the subject well. I
have also enclosed my ,~nalysis of how the points might be alloted using the DEC
SCRO waiver analysis guidelines. Please give me a call if you have any questions.

Per your request of 1-28-80, we attempted to conduct a test of the sanitary
sewer system on the above described property which was vacated.
,/~ One 2-6-80 the septic tank was pumped prior to the performance of the test
, on the seepage pit and leach lines.
On 2-15-80 a technician from our office went to the subject site to complete
tile adequacy test on the system. It was noted that the septic tank and
leach lines were full of water indicating that the septic system was possibly
below the seasonal water table.
In order to confirm this, the septic tank was repumped on 2-15-80 and
monitored over a four day period. On 2-19-80, the water had risen in the
tank 24 inches. See page 2, Table I Summary of Water Level Measurements.
As 'the system is buried below the water table, an adequacy test cannot be
completed. The Municipality of Anchorage Specifications require that the
seasonal groundwater has to be a minimum of 4 feet below the system. This
test indicates that the system does not meet the Municipality of Anchorage
Specifications.
