Memo concerning converting CAT III AWWTS to CAT II AWWTS septic system
To whom it may concern:
The permitted drainfield was installed on the referenced property in 2010 and started up after the
installation. ARM Septic Services. LLC recently checked the monitoring tubes in the drainfields and
found them to be dry. In order to simplify the operation and maintenance of the system, we are
proposing that this unit be permanently configured as a CAT II system. Our justifications are as
follows:
The drainfield on property consist of three (3) shallow trenches with a total absorption of 410
sq.ft. The two (2) southwesternmost trenches (230 sq.ft.) are encompassed within the 30-foot
radius of GEG test hole #1 that had a percolation rate of 28.2 minutes per inch. There is another
test hole (Tobben Spurkland) that had a percolation rate of 40 minutes per inch for which the
30-foot radius encompasses most of the northwestern most trench. An application rate of 2.0
gpd/sq.ft. applies for the two (2) southwest drainfields; therefore, the two southwesternmost
trenches combined meet the absorption area required for CAT II effluent. The combined square
footage of all three trenches exceeds the Category 11 requirements; with a safety factor.
At this time, we request that your department approve the removal of the jandy valve actuator and
placement of the u,,�lve in the open position.

Ref: Proposed Septic Upgrade for The Landings Subdivision; Lot 2, Block 1,
To whom it may concern:
The existing 3 bedroom house is served by private well and septic system. The tank is a 1500 gallon
S.T.E.P. tank which pumps the effluent to a Lined ISF. The effluent from the ISF flows to a shallow
drainfield. The existing Lined ISF is failed/fouled and is allowing little to no effluent out to the drainfield.
We are proposing to modify the S.T,E.P. tank with Quanics controls/floats and install a 650 Aerocell
trickling filter to replace the Lined ISF. The existing S.T.E.P. tank will be exposed to verify integrity. If
the tank is found to be of good integrity, new floats and a new pump will be installed in the existing pump
basket. A new Quanics control panel will be installed on the outside of the house as well as a new outfall
line, bypassing the Lined ISF to the existing drainfield.
The existing drainfield is believed to be a bed type drianfield. On 10/6/1994, Toben Spurkland
documented the original drainfield which was supposed to be a 5 foot wide by 30 foot long drainfield
(150 sq.ft.). The inspection report stated that this was a temporary as-built and that a revised as-built
would be submitted in July of 1995. The property owner attested to the fact that the drainfield was later
upgraded; however, we are unable to find any as-built documentation on file at the MOA. In order to
verify what the drainfield consisted of, we had thee pipes inspected with a camera. We found that there
were three runs of perforated pipe that run approximately parallel with each other. The camera was ran all
the way back to the sample port after the Lined ISF. The comers of the drainfield were marked and all
pipes/comers were swing-fled into the house location. As can be seen on the attached design drawing, we
believe the drainfield was modified into a bed type system. During our numerous site visits to the
property, the monitoring tube and the cleanouts in the drainfield were always found to be dry.
In order to verify soil conditions, obtain accurate groundwater levels, and confirm the soil pemolation
rate, we had a test hole drilled, by Discovery Drilling, adjacent to the drainfield. As can be seen on the
soils log, we encountered 6" of organics and 9.5 feet of GM material (10 feet total depth). No
groundwater was encountered in the test hole at the time of drilling and 7 days later, the monitoring tube
was dry to 10 feet. A percolation test was performed between 2.5 to 3.5 feet and the rate was determined
to be 28.2 minute/inch. Elevation shots were taken on 2/5/2010, and based upon these elevations, there is
over 11 feet of separation distance between the bottom of the monitoring tube (drainfield) and the bottom
of the test hole. In short, groundwater could rise 7 feet in the test hole, and there would still be 4 feet of
separation to groundwater. According to Toben Spurkland, P.E. {2/I 7/I 994 test hole), there was a seep at
4 feet and on 5/16/1994, groundwater was at 2 feet. It should be noted that he commented that glaciation
and evidence of surface runoffwere present at the time of his groundwater readings. According to his soil
log, the soil had a percolation rate of 40 minutes/inch. It is our opinion that Mr. Spurkland's test hole was

influenced by surface runoff into the test hole, causing erroneous groundwater readings. Since a drilled
test hole is the most accurate way of determining accurate groundwater levels, it is our opinion that our
test hole should be used for the actual groundwater readings.
Based upon this information we are proposing that the following work be completed as part of the
installation ofthe ~eptic system upgrade:
I. We are proposing that the southeast end of the drainfield be excavated to determine the width and
effective depth of the existing drainfield.
2. If the drainfield is found suitable, new monitoring tubes will be installed on the southeast comers
of the drainfield and the cleanouts on the drainfield will be made level.
3. If the drainfield is found suitable, our findings will be documented as part of the inspection report
for the entire upgrade and we will use the drainfild as the approved absorption system.
4. If the drainfield is not found to be suitable, we would propose rebuilding the existing drainfield in
the same area by removing all contaminated material, placing sand filter material at or higher than
the existing bed bottom elevation, and then install new drainrock, piping, filter fabric, insulation,
etc.
We believe the drainfield is in a trapezoid configuration and has over 600 sq.fl. By using an MOA
approved Catergory III advanced wastewater system, we are allowed to use an application rate of 3.0 for a
28 minute/inch percolation rate and a 2.0 application rate for a 40 minute/inch percolation rate. Even with
the higher application rate, the minimum drainfield size only needs to be 225 sq.fl. In short, if the existing
drainfield is found suitable, it is adequately sized and meets the square footage requirements.
Based upon this information, we request a permit be issued for this project. Due to legal matters, the
owner must complete the installation prior to May, 2010. We request the permit be issued on a timely
basis so that this timeline can be met.

Dear Mr. Spurkland:
This office is in receipt of your request for a Conditional Health Authority Approval
Certificate for the above property. We are also in receipt of the As-Built Inspection
Report which was submitted to this office on December 23, 1994.
Your inspection report indicates that the total depth from original grade of the
system is "0". Permit #SW940246, under which this system was constructed, allows
for a system with a total depth of 0 ft. and states as a special provision: "The
secondary drain field shall have a minimum of I foot of filter sand in the bottom
which passes the requirements of Engineer Bulletin 94-3".
During a phone conversation with Wayne Ellis on January 3, Mr. Ellis stated that
the method he used to construct the secondary drain field was to remove the top
organics which extended to a depth of between 18 inches and two feet. Mr. Ellis
than stated that he placed six inches of sewer rock in the bottom of the trench, over
which the distribution pipe was installed. When queried on the use ofm~y sand,
Mr. Ellis stated that he did not use any sand in the construction of the drain field.
The softs test data you submitted with the permit application indicated that groined
water was encountered at a depth of two feet. This information, along with the
construction methods described by Mr. Ellis indicates that there is no vertical
separation between the bottom of the disposal field and seasonally high ground
water.
As you are aware, AMC 15.65.060.A.2.b states that the minimum vertical
separation distance from seasonally high ground water and the bottom of a drain
field is 4 ft. As you are also aware, this requirement has been modified to a 2 ft.
vertical separation for effluent from a sand filter, as is the case with this system.

This department will not issue a conditional or unconditional Health Authority
Approval Certificate for this property until this drain field has been modified to
meet the requirement of a 2 ft. vertical separation distance to seasonally high
ground water.

We are requesting a conditional ItAA for this property. Mr. Elles installed an "owner installed "septic system doting
the fall of 1994. Unfortunately, due to the heavy snowfall this winter Mr. Elles was not able to complete the system.
Mr. Elles is committed to close the financing for his property on December 30, 1994. If this is not possible he stands
to accrue substantial additional financing costs.
Mr. Elles desires to escrow sufficient funds to complete the septic system installation not later than July 1, 1995.
The following item has not been completed.
I. Only30 feetofabsorptiontreuchhasbeeninstalled. Presently four persons reside at the house. Four
persons are equivalent to two bedrooms, and the present size trench will accommodate tile expected water flow. An
additioual 150 square feet of absorption trench will be iustalled next year.
2. The sand filter is presently backfllled with 16 inches of material over 2 inches of insulation.
Additional backfill will be placed to comply with the required 3 feet of effective ground cover.
3. A well flow test conducted on December 13, 1994 indicated that the well recovery was 1/3 gpm for
the fa'st three hours, with an average recovery rate of.25 gpm over a 18 bout period. The well is 350 feet deep, with
a static level of 8 feet. The pump is installed at 220 feet. By lowering the pump as much as 100 feet the recovery
should be more than 1/3 gpm for 8 hours, and probably closer to .5 gpm at the pump intake. A well flow test will be
performed to establish the rate after the pump has been lowered.

Subject: Soil Investigation for Sanitary Sewer System, Lot 2, Block 1, The
Landings Subdivision~ Anchorage, Alaska
Dear Mr. Gaard:
At your request of June 8, 1979, we conducted a subsurface soils investi-
gation at the proposed location of the sanitary sewer system on the subject
lot. The investigation complied with those procedures required by the
Municipality of Anchorage Department of Health and Environmental Protec-
tion.
This investigation, which was accomplished on June 8, 1979, consisted o1' a
test hole excavated to a depth of 14.L~e_e.t. below the existing ground surface.
The 'test hole was sited according to your instructions and its location is
shown in aLtached Drawing A-01. Excavation was accomplished with a
backhoe and all material excavated was continuously monitored by an
experienced engineering geologist.
The topography at the excavation site is generally horizontal. At the time
of the investigation the site had some original vegetation consisting of 10
inch diameter birch and spruce trees. The top of the test hole was located
at original ground surface.

The soils encountered in the excavation are shown in the test hole log in
Drawing A-01. This log displays specific conditions encountered at the test
location. However, subsurface conditions may vary in other parts of the lot
without any apparent surficial evidence of the change. Groundwater' was not
encountered and bedrock was not encountered. At the time the hole was
excavated s e a s o n-~'F-'-~-~{ "r'-~ ~'~"-' r~ ~ present and permafrost was not
encountered.
A percolation test was performed in a 2 foot hole at the depth o1' 3__f_e.e_t~jD
an adjoining test pit, The data in Table 1 show average infiltration from the
depths indicated to the bottom of the hole. The measured p.~rc~ql:otiQ,D._ Ea~t?
was 15 minutes per inch.

We are submitting herewith the test boring results and our comments
regarding soil conditions encountered at the subject site. This
investigation was performed in accordance with your request of
August 30, 1973, and those procedures outlined in a letter dated
September 13, 1971 by Mr. Rolf Strickland of the Greater Anchorage
Area Borough Department of Environmental Quality.
A single test hole was put down within the Lot 2 area for the purpose
of defining general subsurface soil conditions for the proposed sanitary
system. Excavation was accomplished with an auger type drilling rig and
the test hole was extended to a total depth of 13 feet below ground sur-
face. The final log prepared for the test hole has been included in
Drm¢ing A-01.
Ground water was not encountered in the test hole.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may

In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test

REPAIR AGREEMENT
MEMORANDUM OF UNDERSTANDING
BETWEEN MUNICIPALITY OF ANCHORAGE
THIS MEMORANDUM OF UNDERSTANDING made and entered into as of this
lc/ Day of 2011 ,by and between ,
herein the "OWNER," and the Municipality of Anchorage, herein the
"MUNICIPALITY." In consideration of the mutual covenants contained herein, the
parties to this Memorandum of Understanding agree as follows:
1. ADVANCED WASTEWATER TREATMENT SYSTEMS. Municipality
grants permission to Owner to utilize and operate an Advanced Wastewater
Treatment System (AWWTS), described as ~~ [ ~ S located
at~'-~e~ ~_O.~inO~ //~l Z- ~ ,Anchorage, Alaska.
Definitions.
A. Alteration. Any change to the design or function of an AWWTS that
Includes the installation or removal of any parts, components or pieces not
included in the original construction permit and design. Prior to
performing any alterations to an AWWTS the owner must obtain a
Wastewater Disposal System Construction Permit from the Municipality
pursuant to Anchorage Municipal Code (hereinafter, "AMC") 15.65.
B. Certificate of On-Site Systems Approval. An approval by the
Municipality of existing water and wastewater disposal systems given at
the time of property sale and title transfer in accordance with AMC 15.65.
These approvals certify that the systems are adequate for the homes that
they support and meet the codes that were in place at the time of system
construction.
C. Damage. Any man-made or natural change in a system that would inhibit
the system from performing as designed.
D. Maintenance and Repair. The scheduled and as needed replacement of
existing parts, components and pieces of an AWWTS that were included in
the original design which would allow the AWWTS to continue to
perform as designed.
E. Permit (Constrnetion) An On-Site Wastewater Disposal System
Construction Permit as defined by AMC 15.65.
F. Permit (Operating) An Advanced Wastewater Treatment System
Operating Permit. An annual permit, issued by the Municipality, that
allows the Owner to operate an AWWTS, upon meeting all the
requirements of this agreement, the conditions of Operating Permit, the
requirements of the On-Site Wastewater System Construction Permit and
all relevant provisions of AMC 15.65
3..Fee. Owner shall pay to Municipality an annual fee of
($ (_.D .00), payable on or before the issuance of the operating permit and annually
thereafter. The annual fee is due on or before the anniversary date of the approval by the
Municipality of installed system.
4. Ter._._~m. The term of this Memorandum of Understanding shall be for the life of the
AWWTS.
The term begins on the date of approval by the Municipality of the installed system and
shall continue while the AWWTS system is in use or operational or until the property is
sold or title is transferred by owner and a new certificate of On-Site approval is issued to
the new owner or transferee of the property.
5. Aiterations~ Installation and Removal of Additional Equipment. Owner agrees
not to make any alterations, removal of parts or additions to the AWWTS without a
Construction Permit from the Municipality.
6. Maintenance and Repairs.
A. Throughout the term of this Memorandum of Understanding, the Owner shall
maintain AWWTS in good repair. In addition, it shall be the responsibility of the
Owner during the term of this Memorandum of Understanding, and any extensions or
renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2)
maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs.
Further, Owner agrees to comply will all applicable ordinance, laws, regulations,
rules and orders for the AWWTS.
B. Owner agrees to provide the Municipality a written schedule of routine
maintenance and repairs which have been performed on the system pursuant to the
terms and conditions contained in the Owner's AWWTS Operating Permit. This
schedule shall be submitted to the Municipality annually upon the renewal of the
permit. The schedule of maintenance and repair contained in the Owner's AWWTS

c. Owner acknowledges that the fine schedule for failing to maintain and repair an
AWWTS are codified in AMC 14.60.
D. Owner agrees that only maintenance, repair personnel certified by the
Municipality will inspect and make any necessary maintenance, repairs or permitted
alterations to the system.
E. Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS upon 24 hours written notice.
F. Owner agrees that any sale or transfer of title of the property will not occur
without a new Certificate of On-Site Systems Approval.
G. Owner agrees that the relevant provisions of the standard specification guidebook
for AWWTS is the governing professional guidelines for the construction,
maintenance and repair of the Owner's AWWTS.
7. Nonwaiver. The failure of either party at any time to enforce a provision of this
Memorandum of Understanding shall in no way constitute a waiver of the provisions,
nor in any way effect the validity of the Memorandum of Understanding or any part
hereof, or the right of such party thereafter to enforce each and every provision
hereof.
8. Amendment.
A. This Memorandum of Understanding shall only be amended, modified or changed
by a writing, executed by authorized representatives of the parties, with the same
formality of this Memorandum of Understanding was executed and such writing shall
be attached to this Memorandum of Understanding as an amendment.
B. For the purposes of any amendment modification or change to the terms and
conditions of this contract, the only authorized representatives of the parties are:

C. Any attempt to amend, modify, or change this contract by either an
unauthorized representative or unauthorized means shall be void.
9. Jurisdiction: Choice of Law. Any civil action arising from this Memorandum of

State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and
obligations of the parties under this Memorandum of Understanding.
10. Severabili ,17/. Any provisions of this Memorandum of Understanding decreed
invalid by a court of competent jurisdiction shall not invalidate the remaining provisions
of the Memorandum of Understanding.

the existing drainfield. The closest neighboring drainfield (The Landings; Block 1, Lot--2')~has a
separation distance of over 10 feet from the lot line; therefore there is a minimum of 15 feet of
separation between the two drainfields. Both drainfields have 0.5 feet of effective depth, which

WATER WELL ADVISORY
HEALTH AUTHORITY APPROVAL NO.
During a recent Health Authority Approval on-site inspection
and test of the potable water supply well on Lot ~
Block i of ~ C/~DI~ Subdivision, the well's
productivity was determined to be 0,~ gallons per minute.
The minimum well productivity required by this Department
(AMC 15.55) for a ~ bedroom residence is O, ~ I gallons
per minute. Although the subject well currently exceeds this
minimum requirement, all parties concerned are advised that the
production capacity of the well may fluctuate. Restriction
of non-critical water uses such as washing cars and watering
lawns and gardens may be required.
This advisory must be attached to all copies of the subject
Health Authority Approval.
Health Authority Approval Certificate Conditions
Lot 2, Block 1, The Landings Subdivision
This listing of conditions is a part of and shall be attached to all copies of Health
Authority Approval Certificate HA940704 for Lot 2, Block 1, The Landings.
This is a conditional approval for three (3) bedrooms with the following
stipulations:
The pump in the existing well shall be lowered 100 ft. to a depth of 320 ft. A
well flow test shall be performed to establish the production rate after the
pump has been lowered, with the results submitted to DHHS.
Additional fill shall be placed over the soil absorption system to create three
(3) ft. of effective ground cover.
A test hole with a minimx~m depth of six (6) feet shall be installed
approximately 10 feet from the downstream end of the existing 5 ft. wide
trench. This test hole must be installed by February 15, 1995, and shall be
monitored for ground water levels through June 1, 1995 on a weekly basis. If
seasonally high ground water is found to be more than two (2) ft. below the
bottom of the existing trench, the trench shall be extended 30 ft. using the
same cross section as the existing trench. If seasonally high ground water is
found to be less than two (2) ft. below the bottom of the existing trench, a
new five (5) ft. wide by 60 ft. long drain field shall be constructed which
provides a minimum of two (2) ft. vertical separation to seasonally high
ground water.

Per your verbal communication on January 4, 1995, regarding the separation distance between the bottom of the
absorption field and the high groundwater elevation observed on May 16, 1994 1 obtained elevation shots of the existing
ground next to the installed drainfield aud ground shots at the approximate incation of the testhole. With the garage
slab as a reference elevation of assmned 100.00 feet, the grouod elevation at the testhole location was 97.1 feet,
undisturbed ground at the drainfield 95.4 feet and the bottom of the monitor tube, 95.3 feet.
These observations indicate that the drainfield is installed on top of the ground. I should point out that a permit for a
septic system was issued for this lot in 1981. At that time a building site was cleared and leveled, but no construction
took place. A testhole was dug to 14 feet in 1981 with no observatinn of grouud water.
The testhole dug in 1994 was witbin the previously cleared building site. No organic materials is shown ou the testhole
log. Ground water was observed two feet below top of ground on May 16, 1994. When Mr. Elles excavated for the
basement and the footings for the residence water was uot encountered. The excavation for the STEP tank was 13 feet
deep. Water was not observed itt this excavation.
Part of the drainfield installed by Mr. Elles was outside the previous cleared area. Mt'. Elles removed the organic
material where it was eucountered but did not excavate into the non-organic material. The field was installed as
intended, with a 2 foot separation to the observed ground water. All observations indicates that groundwater
encroachment is not very likely. However, observations during breakup call be made to ascertain tbat this is so. The
present drainfield must be tnodified to meet the required physical size. If ground water monitoring during breakup does
show that the groundwater are within 2 feet of tile bottom of the present drainfield, sand in the proper amount can be
placed to obtain the required separatinn.
