The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional, civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer
of title (except between spouses)for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples.. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

As certified by my seal affixed hereto and as of the validation date shovvn belov4 1 'verify that my
investigation, based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
information obtained from the Municipality of Anchorage files and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

Engineer's Comments:
In conducting this evaluation, GEG, LtD. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of th(s report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

Subject: HAA for Lot 22, Tanaiaa Valley S/D.
To whom it may concern:
The subject lot has a 4 bedroom house on it which is served by a private well and community
water. The results of the adequacy test is summarized as follows:
SEPTIC SYSTEM ADEQUACY TEST: The drainfield is a 2 foot wide trench, which is 47
feet long, and has an effective depth of 8 feet. It is my understanding that the home had been
vacant for approximately 2 months (since Christmas). Based upon the trench dimensions, and
assuming a drainrock porosity of 33%, theoretically, it should have taken about 19.5 gallons to
raise the level in the trench by one inch. rFhe monitoring tube had 9.5 inches in it on 2/22/97.
Presoak (2/22/97): Water was introduced into the M.T. at a rate of 6.15 gpm for a total of 195.5
mitmtes (1203 gallons)~ The level in the M.T. rose 52.25 inches to 61.75 inches, which
corresponds to 23 gallons/inch. Twenty-four hours later the level had dropped 10.25 inches,
indicating only 236 gallons had been absorbed. This was considered at presoak.
Adequacy Test (2/24/97): Prior to starting the adequacy test the liquid level in the monitoring
tube was 46.5 inches. Water was added into the C/O at a rate of 4.69 gpm for a total of 246
minutes (1153 gallons). The liquid level rose 51.25 inches to a total depth of 97.75 inches
(slightly above the drainpipe invert), which corresponds to 22.5 gallons/inch. Sixteen hours and
39 minutes later (16.65 hours) the level had dropped a total of 22.5 inches, indicating an
absorption of 30.4 gallons/hour, or 730 gallons per day. Based upon this data, it was determined
that the absorption rate of the trench exceeds 600 gallons per day, as required for a 4 bedroom
house.
NOTE: The adequacy of a septic system is influenced by numerous factors, inchMing, but not
limited to, seasonal sm;face water it~ltration, groundwater variations, septic system
maintenance (frequency of septic tank pumping, usage of biological additives), condition of
drain pipe and pipe joints (which can be damaged by seismic activi(y and deteriorate with age),
type of substances deposited m septic system (cigarette butts, scmitary napkins, misc. objects),
and the amotmt of water being introduced on a continual basis. Consequently, the results of this
adequacy test are only valid for the specific day of the test. Furthermore, because of the limited
natm'e of this investigation, it is possible thai there are hidden defects which may trot have been
detected. No warrantee is made regarding the fitture performance of this well or septic system
