The septic tank and initial drainfield were installed in 1979. The trench was upgraded in 1983. The drainfield consists of three parallel trenches (which are likely spillover trenches). The house had been vacant since 4/2912017. During our initial investigation (5/19/2017) the drainfield/s failed to pass an adequacy test. There were functional MTs or Sumps in the 1979 trench and the westernmost 1983 trench. It took only 577 gallons to fill both trenches, and there was almost no recovery in 24 hours. No plumbing leak was noted by our field technician. The property owners authorized us to proceed with a test hole and a design. A test hole was excavated on 6/7/17. When returning to the property to perform 7-day groundwater monitoring we found the test hole monitoring tube and the perk test bench filled to a significant extent with overflowing wastewater from the adjacent 1983 drainfield. Given that the house was vacant, it was clear that there was a recent plumbing leak. It was discovered that there was a leaking toilet flapper that was introducing water continuously into the drainfield and grossly surcharging it to the ground level. The plumbing leak was fixed and arrangements were made to pump liquid out of the drainfields and test hole. The objective was to see if the liquid level in the test hole monitoring tube was going to subside and allow for obtaining an accurate reading of the groundwater level. Over the next several weeks the liquid depths were monitored in the trenches (the middle trench had no MT or CO in it) and the test hole monitoring tube. There was limited recovery in the trenches (again, the home is vacant). It was then discovered that the test hole monitoring tube was pinched-off or filled with silt. A new test hole GWM tube was installed on July 10, 2017, after it was discovered that it had become pinched-off or filled with silt. When the test hole was re-excavated, a monitoring tube and cleanout were installed in the middle trench segment (1983 system) so that we would have a functional MT in all of the "reserve trenches". . On 7/14/17 and 7/17/17 groundwater was found to be 9.8 feet below grade in the test hole GWM tube. It is possible that the test hole monitoring tube is providing false groundwater readings because of the nearby surcharged trenches. It should be noted that in Gary Player, P.E found groundwater at 10 feet below grade in one test hole on this property in March of 1979 (see MOA records). Due to time constraints on the sale of the house, the decision was made to proceed with the design, assuming groundwater levels in the recent test hole are accurate. We are proposing to install a new 1000 gallon septic tank and dual 5-wide trench type drainfields. Due to site constraints, the proposed east trench will be located over a portion of the existing 1983 western drainfield. We are proposing to excavate all drainrock and contaminated soil out of the abandoned portion of the 1983 trench and fill it with MOA approved filter sand. One testhole was excavated on the property and the drainfields are designed in the vicinity of its 30 foot radius. Comments regarding the design are summarized as follows: 1. SOILS: See the attached 2017 soil log which show the soil classifications, groundwater monitoring, and the percolation test results. Additionally, attached are the soil logs for Turnagain Park #2; Block 1, Lot 1 and Block 1, lot 3. Both showing a percolation rate less than 60 min/inch. Based upon this fact, we find it reasonable to believe that the application rate of 60 min/inch is conservative throughout the proposed trench and that additional percolation tests prior to construction are not necessary. 2. DRAINFIELD DESIGN: See attached design drawing for drainfields specifications. 3. SURFACE WATERS: There is a lined manmade pond (see attached photos) less than 100 feet from the proposed drainfields. The manmade pond is lined and does not have an outlet. 4. TOPOGRAPHY: As can be seen on the attached topography drawing, the average topography around the proposed drainfields is approximately 10% running northeast to southwest (see attached topo drawing). 5. LOT LINE WAIVERS: Due to site constraints, we are requesting a five (5) foot waiver to the north property line and a three (3) foot waiver to the west property line. Per MOA records the 1985 bed type drainfield for Turnagain Park #2; Block 1, Lot 1 is 5 feet from the property line. In short, the proposed drainfield would be approximately 10 feet from the existing bed on the adjacent property. (907) 2B4 4I 11 June 3, 1983 To: James Ethington Daniel Nix Alaska Mutual Bank, Attn: Carolyn Smith Subject: Intent to Revoke On-site Sewer Permit, Lot 2, Block 1, Turnagain Park Subdivision On March 10, 1983 this office received an application for Health Authority Approval relating to the sale and subsequent financing of the subject property. On March 10, 1983 this office sent a letter to James and Martha Ethington (copy attached) outlining certain steps to be taken prior to our approval being granted for the on-site sewer and water facilities. Commensurate with these instructions, an adequacy test was performed on the on-site sewer system by Alaska Environmental Control Services, Inc., the results of which indicated that the existing system was not acceptable for a 3 bedroom home. Subsequently, on April 18, 1983 this office conditionally approved the Health Authority Approval subject to monies being escrowed to have the existing wastewater disposal system upgraded to meet the minimum requirements for a 3 bedroom home. On April 28, 1983 this office issued an "on-site Sewer Upgrade Permit (No. 830302) to James Ethington for the upgrading of the existing system. The dimensions of the required upgrade were based on a soils test performed by Gary Player for Jim Ethington on March 2, 1979. This test indicated that the "accepting" soils were rated at 125 square feet per bedroom. Based on this soils data (furnished by Mr. Ethington) the amount of system upgrade required was 21' of 6' deep trench with a gravel depth of 4' On May 3, 1983 an on-site inspection was performed by Alaska Environmental Control Services, Inc., of the upgrade system. During this inspection, it was observed that the accepting soils in the upgrade trench were not the SW soils (125 sq.ft, per bedroom) as indicated on the original soils test. A percolation test was then subsequently performed by Alaska Environmental James Ethington, et al June 3, 1983 Page 2 Control Services, Inc., and the soils were determined to be SM with a rating of 313 sq.ft./bedroom adjusted to 410 sq.ft./bedroom (see attached letter). Under this soil' condition, which definitely exists at the upgrade site, the amount of upgrade system must be 51' of trench 8' deep and 30" wide with 5' of gravel below the perforated distribution pipe. According to the attached letter dated May 24, 1983 the excavator has completed 21' of the upgrade, therefore, to comply with the total upgrade requirement, an additional 30' of trench must be completed. The total upgraded system (51') must be completed before the sub- ject 3 bedroom residence will have met the minimum requirements for on-site disposal of wastewater. We will allow fifteen days for the final upgrade to be completed, however, if the upgrade is not completed by Monday, June 20, 1983, we will then initiate action to revoke the On-site Sewer Permit. There are a number of inconsistan- cies and invalid design calculations in your calculations; namely, 1. There is a $50 delivery fee included in the price of gravel which will appreciably reduce the projected volume of gravel delivered. The gravel depth of the upgrade is not additive to depth of the original system because these are two systems. the gravel separate The theoretical area available for absorption in a deep trench utilizes only the sidewalls of the trench with no bot- tom or end effect. The effective areas of the original trench (49' long x 6' deep w/4 gravel) is 49x(4+4) or 392 sq. ft. Mr. James Ethington July 11, 1983 Page 2 The accepted procedure for determining the amount of upgrade is to calculate the additional square feet of drainage field necessary to handle the inadequacy of the existing absorption areas. In your particular case, this mathematical exercise is as follows: 3 bedroom wastewater generation = 450 GPD less capacity of existing facility -273 177 GPD underage 177 ~ 150 + 1.18 bedroom equivalent Soils rating at upgrade site = 431 sq ft/bedroom (adjusted) 1.18 x 431 = 509 sq. ft. of additional absorption area. Assuming that the original upgrade performed by Carl's Excavating during June of 1983 was 21' of~6' deep trench with 4' gravel depth, the additional absorption area was 21'x(4'+4') = 168 sq ft. This addition is still 341 sq ft less than the 509 sq ft total upgrade requirement. In order to meet the total upgrade requirement, there still must be an additional 34 feet of 8' deep trench with 5' of gravel. In one of our previous telephone conversations, you indicated that you would be back in the Anchorage area during the week of July 3; therefore, I had assumed that we could continue this case by personal contact. Assuming that you were going to be in the office as indicated, I deferred initiating any further action or correspondence. In the closing paragraph of your letter, you requested an additional 60 day extension of the permit which further indicates that your trip to Anchorage has been cancelled. This problem has already been extended since May 5 which is now Mr. James Ethington July tl, 1983 Page 3 65 days with no action yet in sight. The situation remains unchanged and the system upgrade must be completed. Allowing time for out-of-state mail service, and a realistic allowance for your negotiations with an excavating contractor, we will extend your permit to July 31, 1983. On July 28, 1983 we will have an inspector at the site (or sooner if the remedial work is completed at an earlier date) and if the upgrade has not been completed and approved, we will initiate action to revoke the on- site sewer permit. Subsequently upgrade permit # 830302 was issued to establish an adequate system for the 3 bedroom home. However, the original soils report rated the soils at 125 sq. ft. per bedroom, and was utilized for the upgrade design. (Total depth 6', gravel depth 4' and length 21'). During the May 3 1983 inspection, the excavator has installed a trench per specifications on the permit, however, the soils were found to be SM (per attached soils report). The soils were perced at 56 minutes per inch, which is 313 sq. ft./bedroom (per Municipality adjustment 410 sq. ft./bedroom). The soils were consistant to 11.5' with no observed ground water. The result is, the design per permit would also be inadequate due to the difference in soils observed.
