SPECIAL PROVISIONS:
ENGINEER MUST NOTIFY DHHS AT LEAST TWO HOURS PRIOR TO EACH
INSPECTION. ENGINEER MUST PROVIDE REVISED AS-BUILT
INSPECTION REPORT THAT DOCUMENTS SIZE OF CONCRETE TANK AND
ALL PIPING BETWEEN THE HOUSE AND THE BED. ENGINEER WILL
INSTALL TWO CLEANOUTS BETWEEN THE EXISTING TANK AND THE
PROPOSED NEW TANK.

Narrative
Dear Mr. Smith:
We are requesting a permit for a septic tank upgrade. The existing concrete tank was
inspected, and the baffle system has failed. A new tank is proposed after the concrete, retaining
the existing tank in line.
The proposed septic tank upgrade will have very limited impact on adjacent properties for the
following reasons:
1. The area has large lots allowing sufficient room for septic sites.
2. Immediate neighboring septic systems are all +30' distance.
3. Reserve space is adequate. There is an existing system in place that was tested
as adequate
4. Drainage will not be effected and is not a major consideration in our design.

Valley View Estates
A sewer system adequacy test was performed on the system
located on the referenced property, as you requested. The
septic tank was pumped amd verified to have a capacity of
1000 gallons. The seepage pit was full of water and approxi-
mately 800 gallons had to be removed. The system was
then recharged with 800 gallons of fresh water and after
a period of 24 hours approximately 44 gallons had percolated
out of the crib.
i~ can be concluded from the above test that the septic
tank is currently functioning adequately and has an adequate
capacity for the three bedroom residence. However, the
seepage pit has completely failed. I regret to inform
you that it will be necessary for you to have this system
upgraded.

'Fhe top of the well casing sealed with a sanitary seal so
that it is water tight.
Exposed electrical wires to the w~ll head are in violation
of the Municipality of Anchorage codes and must be encased
in conduit.
The water analysis report needs to be submitted to
office from the Chem Lab, 5633 B Street, for our review.
The septic tank pumped with a r~ceipt submitted to this
department.
An adequacy test needs to be performed on the existing
leaching area. This test will determine if the system is
adequate according to National Standards. A listing of
private firms performing the test is enclosed. This report
needs to be submitted to this office for our review.
Please notify this Department for a reinspection when the
noted discrepancies have been corrected. If there are any
further questions, please call this office at 264-4720.
