leave it at 25' long.
2.5' depth of rock is good because it maintains 2.4' of vertical separation from September
groundwater reading. Just include this info on the record drawings.

Sorry about that, but a reduced application rate of 5GPD/SF was used and a 6GPD/SF
application could be used and be at 20' total length if you want, we just proposed a 5'
longer tread and would still propose that to be installed. The 25' length is close to the 20',

but we can make the application 6 and shorten. It has been done before with just a bit
more length. The 1.5' total depth is ok, but we could make it 2.5' and that is why it was
proposed. 2.5' total depth - 5.9' September reading is 3.4' a current separation or 2.4' from
the assumed high seasonal. Since installation is currently in process, we cannot wait for
an October reading, but believe it will be about the same as monitored.
This system will be gravity fed. So we can shorten to 20' using the 6 rating or keep the 25' 5
rating AND we can keep it at -1.5' or -2.5'. Either way - 251 - 2.5' or 20' L - 1.5'. Kinda
spitting hairs here, but would think the 251 x -2.5' would be best. What would the MOA
want?

If you install bottom of rock at 2.5 below grade that would be 2.4' separation from
the adjusted groundwater reading in September. In order to have reduced vertical
separation you either need to use pressure distribution or segments not exceeding
20' in length. 15.65.355H. Which one will you do?
At time of inspection report submittal add new groundwater readings to your test
hole.

Per our permit letter dated 7/30/24, we ascertain that the early
groundwater readings on July 8 are conducive to a June reading
or 3' separation to observed groundwater and do not believe
these levels will fluctuate. We monitored ground water in
August on 8/17/24 at 5.8'& again today in September on
9/3/24 at 5.9'. Given this, we respectfully request that the
bottom of the proposed field be permitted to be installed at -
2.5' from grade and 3.4' from this recent September
groundwater level for this Advantex CAT III system. We
attached the permitted design for your use - please let us
know.

RE: SEPTIC SYSTEM UPGRADE PERMIT
LEGAL: RAINBOW CREEK LOT 1
The property owner has requested we obtain a permit to upgrade the
undocumented failed septic system (tank & field) of the above referenced lot. We
propose to install a shallow trench with an Advantex system to serve the existing 4 -
bedroom (3BR house & 1BR ADU) system per the attached design. The design is
based on the recent test hole conducted on June 27, 2024. Groundwater was
observed at 7’ during excavation and 5.6’ during monitoring of this test hole.
The absorption field is to be installed on slopes of 23-28% with no downhill slopes
greater than 46%. The downhill side will not be disturbed or developed and is the
Seward Highway ROW with heavy vegetation and a few hundred feet from the
actual highway. There was no evidence of any daylighting from the existing
conventional - undocumented trench. Additional cover will be placed over and
downhill of the proposed trench of the excavated / impacted area. The groundwater
was at 5.6' on July 8, 2024, and believe with the heavy and late snowpack melt of
2023-24 the readying is more conducive to a June reading.
As you know 15.65 code allows 2' separation to groundwater for CAT III and we
have maintained 4' separations and will with sand as needed. Although one would
wish to monitor groundwater in October or May, the groundwater levels will be
verified again. We maintain that site characteristics and current conditions that this
5.6' will NOT most likely rise and is the high seasonal groundwater level. If these
levels have not risen, we request that the proximity to a June reading be used with
an additional total depth of 1' or 3' to groundwater if so deemed.

The slopes are approximately 23-28% at the proposed upgrade location with 30-
40% down slopes. The lot and area are served by private water with no known
private wells within 50’ of the proposed septic system. Based on visual
observations and review of available information, the design will not impact any of
the neighboring properties. Please contact us if you have any questions.

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and
entered into as of this ti20� Day of of 20 2W , by and between

Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:

1. Advanced Wastewater Treatment Systems The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),
described as

2. Maintenance. Repairs and Alterations.
(Owner is required to read, understand and initial each section)
Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $600).
Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7 Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

Comments: This investigation was completed in compliance with MOA guidelines, regulations,
and best industry practices / methods. The assessment of the condition of the well and septic
applies only to the conditions as of the day tested. The flow and absorption rates may change
due to subsurface conditions that may not be observed from the surface, changes in land use,
local soil characteristics, groundwater levels that may fluctuate during the year, quality of
construction (workmanship & materials), the water usage of the family being served by the

ADVANCED WASTEWATER TREATMENT SYSTEM
MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and

Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems, The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AW WTS),
described as AN ADVANTEX SYSTEM
located at (legal description)

2, Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AW WTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AW WTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
a includes an annual maintenance fee (typically $400 to $600).
J Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an AW WTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.

Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 24-hour notice.

Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction, maintenance and repair of the Owner's AWWTS.
13 Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties render this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
