The owner has requested that we obtain a septic permit to upgrade the existing aged steel septic
tank on the above referenced lot. We propose to install a 1530-IM Infiltrator septic tank per the
attached design to serve the existing 2-bedroom duplex residence. The proposed 1500-gallon+
tank is to be installed for this 4-bedroom rated septic system, for current functionality and future
consideration or flexibility. No groundwater was noted, but if groundwater is encountered during
installation the tank may be required to be anchored. The lot and area are served by private water
and any encroaching wells, easements, … must be staked prior to construction. The design will
not impact any of the neighboring properties. Please contact us if you have any questions.

To whom it may concern:
ENSTAR Natural Gas Company, LLC (ENSTAR) has no objection to existing septic tank encroachment
within a ten feet (10 FT) wide ENSTAR Natural Gas Right-Of-Way Easement, which is in alignment with
the existing road and located within the N1/2, SW1/4, NW1/4, NE1/4, SE1/4 of Section 30, T12N, R2W,
Seward Meridian, filed in the records of the Anchorage Recording District, Third Judicial District, State
of Alaska.
***Be advised, there is a natural gas facility located in the area.***
Acceptance and use of this letter of non-objection by yourself, your heirs, your assigns, or your successors,
will constitute agreement to the following stipulations:
•Landowner/Contractor working near ENSTAR gas facilities shall contact the Alaska Digline,
Inc., (907) 278-3121 or 811 for line locating two (2) business days prior to any related excavation.
This service is free of charge.
•ENSTAR will be held harmless, now and forever for any damages or injury to any person or
property as a result of this encroachment.
•Any ENSTAR facility damaged or destroyed, as a result of this encroachment will be repaired at
no cost to ENSTAR.
•Any costs incurred by ENSTAR for special construction necessitated by this encroachment will
be borne by the land owner.
•This letter of non-objection will in no way preclude ENSTAR from full use and enjoyment of its
rights within any portion of its right-of-way.

Subject: Soil Investigation for Sanitary Sewer System, Lot 14, Section 30 N%Z
of S.W. 'a of N.W. 'q of N.E. 14 of S.E. '4 of T12N R2W Seward
Meridian.
Dear Mr. Schuster:
At your request of July 14, 1980, Pete Herstek of R&M Consultants con-
ducted a subsurface soils investigation at the proposed location of the
sanitary sewer system on the subject lot. The test proceedure complied with
those required by the Municipality of Anchorage Department of Health and
Environmental Protection.
Field activities were performed on July 14, 1980, and consisted of two test
holes. The first test hole drilled to a depth of 13 feet below the existing
ground surface. The second test hole was drilled to a depth of 7.5 feet.
This is the proposed effluent acceptance stratrum and a percolation test was
performed at this location. The test holes were sited according to your
instructions and their location is shown on the attached sheet. Drilling was
accomplished with a rotary drill rig using continuous flight solid -stem auger
with an outside diameter of 6 inches. Grab samples at the depths shown on
the soils log were taken to the laboratory and were visually classified. The
top of the test hole was located at original ground surface.
The topography at the drilling site is generally steeply -sloping to the north.
At the time of the investigation the site had original vegetation consisting of
Alpine plants and shrubs.

j The soil encountered in the first test hole is shown in the test hole log on
the attached sheet. This log displays specific conditions encountered at the
test location on the date of the investigation. Subsurface conditions may
vary in other parts of the lot without any apparent superficial evidence of
the change. Groundwater level may fluctuate both seasonally and from year
to year. Groundwater was not encountered. Groundwater level represents
the level at the time of the exploration only. Where ground water is
encountered the bottom of the absorption system must be at least four feet
above the water table.
Bedrock was encountered at a depth of 13 feet as indicated. At the time the
hole was drilled seasonal frost was not present and permafrost was not
encountered.
A percolation test was performed within the test hole at the depth shown in
the attached table. All depths were measured from the top of the test hole.
The data on the attached table shows average infiltration in the stratum
tested. The measured percolation rate was 4.44 minutes per inch.
However, because of the depth and size of the percolation test hole, it is
recommended that a absorbtion area factor of 150 square feet per bedroom be
used for the design of the system.

LEGAL: T12N R2W SEC 30 N2SW4NW4NE4SE4 – 13324 ALPWEG
Due to unprecedented events, seasonal conditions and site accessibility, the septic tank
installation or upgrade cannot occur at this time. The septic tank pump truck with chains could
not make it to the site and had to pulled out of the ditch by a wrecker. We are requesting a
Conditional COSA for the above referenced property. The pending permit OSP231406 of this
septic tank upgrade has been impeded due to these conditions with no specific known date on
when the conditions and other factors would allow the septic tank installation to occur safely.
The septic tank will be installed once prudent, and the conditions are favorable for installation.
The system has been working for the occupants, recently passed a septic adequacy test and
granting of this Conditional COSA (CCOSA) will not impact any of the neighboring properties
or pose a public health risk. Attached are three septic tank upgrade estimates and escrow letter
from title for the pending installation. Please contact us if you have any questions.

SCOPE OF WORK
We will pump, clean and decommission existing septic tank- Install new 1530 IM
tank. Tank will be bedded and compacted. We will install barricades to protect tank
Grade and clean-up on completion.
Unless specifically agreed otherwise, the property owner shall be responsible for
final grading areas subsequently depressed from soil settling. On all leach field
mound systems, property owner is responsible for ensuring a satisfactory
vegetation growth over the mounded area is in place.
Denali Excavating LLC proposes the above scope of work for $16,568. No additional
work will be done without a change order. This proposal is good for thirty days.

estimate. Acceptance of this estimate will be
acknowledged upon either receipt of a signed copy to
Northern Excavation and Construction (NEC) or an
email stating acceptance and notice to proceed by the
financially responsible personnel.
Changes that affect or increase specific tasks and
machine time shall be submitted and agreed upon in
writing. All agreements are contingent upon
circumstances beyond NEC's control, i.e. weather,
design changes, unforeseen site conditions, etc. NEC
is licensed, bonded, insured, and maintains safe
digging practice. All methods and material shall be in
accordance with MOA and SOA regulations. NEC shall
not be held liable for affects that may occur to the
property during or after install. Affects such as, trees
left in close proximity of where machine work takes
place, having to stock and later retrieve displaced
excavated material around trees, damage or structural
integrity of retaining walls or fences that may need to
be partially removed or traveled close too, disturbance
to driveways/landscaping, etc. Client is encouraged to
contact NEC prior to start of construction for
clarification of specific affects that may occur on the
property.
Payment in full is due upon receipt. Payments
received after 30 days of completion shall be charged
a 1.5% monthly fee on any unpaid balance.
