Reference: Septic Tank Upgrade and Holding Tank Documentation for T15N, R1 W,
Section 18; N2, N2, SE4, SW4, SE4
To whom it may concern:
The 3 bedroom house on the referenced property is served by a private well and septic system.
Per MOA records, the septic system serving the subject property was installed in 11/17/1986
and was sized for 3 bedrooms. The septic system consists of a 1250 gallon steel septic tank
and a bed type drainfield (50 feet long x 30 feet wide with 0.5 feet of effective). We recently
performed an adequacy test on drainfield. Based upon elevation shots taken down the
cleanouts of the four (4) corners of the bed, we did find the distribution lines in the drainfield to
have as much as 6 inches difference in elevation between inverts. In short, the drainfield
appears to be out of level. After the septic adequacy test, we came to your office and discussed
our testing results/findings with Tim Eklund and he stated he would accept our adequacy data
(passing adequacy test). The septic tank is over 32 years old. Instead of trying to verify the
integrity of the old septic tank, the owners would like to upgrade the septic tank. We are
proposing that a new 1060 gallon, 2-compartment Infiltrator plastic septic tank be installed, and
the old septic tank permanently decommissioned. We request a septic tank upgrade permit be
issued.
Undocumented Holding Tank: It is has come to our attention that there is a holding tank on
the property that serves a home office building. The owner would like to abandoned this holding
tank since the MOA will not allow a holding tank for this type of application. We propose the tank
be decommissioned per the UPC and the sewer line capped at the foundation. Per the owner,
there is no water service line to this structure
Other Issue or concerns:
The well that serves this property is located along with the pressure tank in a pit (see attached
photos). The pit consists of a 48" diameter vertical culvert and has an insulated wood lid above
grade. The top of the well head extends 18+ inches above grade on the outside of the wood

box. The well has a sanitary seal and the wires are in conduit to near the bottom of the pit. The
pit appears to have an open gravel bottom, so drainage or ponding water in the pit area should
not be an issue. We observed this well in June of 2015 and during a recent site visit (3/14/2019)
and have not seen any standing water, or seen any indication of water collecting, in the pit.
Regarding the well being on a separate property, per the owner, the properties are being sold
as a single parcel and has conveyed that the MOA has determined that the properties cannot be
sold separately. The owner has a well/water service line agreement that has been recorded
(see attached).

THIS WATER WELL AND MAINTENANCE AGREEMENT ("Agreement") is made and
entered into and effective this 1st day of June , 2019, by

(collectively hereinafter"McGoldrick"),whose address is

RECITALS
WHEREAS, McGoldrick is the owner of certain real property identified as

North, Range 1 West, Seward Meridian, according to the official Bureau of
Land Management Survey thereof, being located in the Anchorage Recording
District,Third Judicial District, State of Alaska,
("McGoldrick Property"); and
WHEREAS, McGoldrick is the owner of certain real property identified as
The North one-half of the Southeast one-quarter of the Southwest one-quarter
of the Southeast one-quarter(N'/2 SE'/ SW'/, SE'/a), Section 18, Township 15
North, Range 1 West, Seward Meridian, according to the official Bureau of
Land Management Survey thereof, being located in the Anchorage Recording
District,Third Judicial District, State of Alaska,
("McGoldrick Property");and
Both parcels are more particularly depicted in Exhibit "A" attached hereto and incorporated
herein by reference; and
WHEREAS, a water well, and its necessary pump, casing and pipes, which provides water to
both of the above described parcels,is located upon the McGoldrick property; and
WHEREAS, the parties desire to enter into a written agreement for the use and maintenance
of said well and all appurtenances thereto,as between the parties;

NOW, THEREFORE, in consideration of the mutual covenants and conditions contained
herein, and for other good and valuable consideration, the receipt, value and sufficiency of
which is hereby acknowledged,the parties hereby covenant and agree as follows:
AGREEMENT
1. Use and Maintenance.
The Parties and their successors in interest, heirs and assigns, shall be jointly responsible for the
maintenance of the well and its necessary pump, casing and pipes located at the well site.
McGoldrick grants to McGoldrick an access easement for the purpose of said maintenance.
During the existence of this Agreement, McGoldrick shall not utilize the well and/or the area
around the well in any manner which will prevent McGoldrick from having full, free and
uninterrupted use of said water well. Nothing herein contained shall be construed to imply any
warranty by McGoldrick as to the condition or status of the water well and/or the well's ability to
continue to furnish water,or liability for any damages in the event the well ceases to function.
During the existence of this Agreement, McGoldrick and their successors in interest, heirs
and assigns, shall be fully responsible for the maintenance of the appurtenances of the well
which run from the well to the McGoldrick Property. McGoldrick grants to McGoldrick an
access easement for the purpose of replacing, repairing and maintaining said appurtenances
located on the McGoldrick's Property.
During the existence of this Agreement, McGoldrick and their successors in interest, heirs
and assigns, shall be fully responsible for the maintenance of the appurtenances of the well
which run from the well to structures located on the McGoldrick Property.
2. Modification of Agreement. Any amendment to or modification of this Agreement shall be
binding only if evidenced in writing signed by each party or an authorized representative of each
party.
3. Term of Agreement. This Agreement shall automatically terminate in the event that the
well ceases to function and/or fails to meet minimum specifications for production of
sufficient water to accommodate both properties. In the event that the well ceases to function
and/or fails to meet minimum specifications for production of sufficient water to
accommodate both properties, each owner of their respective property shall drill a new well
on that owner's respective property. The benefits and burdens of this Agreement shall
constitute a covenant running with the properties herein described and shall be binding upon
the heirs, successors in title and assigns of the Parties hereto until termination.
4. Subject To. The rights granted under this Agreement are subject to all encumbrances, liens,
easements, conditions, covenants, reservations and iestiictions of record or that can be
ascertained by an inspection or survey of the property as of the effective date of this Ag'eement.

5. Successors and/or Assigns. Each covenant and condition contained in this Agreement
shall inure to the benefit of and be binding on the Parties to this Agreement and their
respective heirs, executors, administrators, personal representatives, successors and assigns,
except as otherwise provided herein.
6. Time of Essence. Time is of the essence for each covenant and condition of this
Agreement.
7. Governing Law. This Agreement shall be governed by and construed in accordance with
the laws of the State of Alaska.
8. Waiver. No waiver of any breach of any of the covenants and/or agreements herein
contained shall be construed as, or constitute, a waiver of any other breach or a waiver,
acquiescence in or consent to any further or succeeding breach of the same or any other
covenant and/or agreement.
9. Severability. If any term or provision of this Agreement shall, to any extent, be held
invalid or unenforceable, the remaining terms and provisions of this Agreement shall not be
affected thereby, but each remaining term and provision shall be valid and enforced to the
fullest extent permitted by law.

guidelines and regulations established by the Municipality of Anchorage and industry practices. The reported results describe the
condition of the systems on the date/s of the evaluation. Separation distances were measured to readily identifiable features.
Hidden defects or encroachments may exist that were not identified during the evaluation. The operational life of all wells and septic
systems depend on a variety of variables including, but not limited to, soil conditions, groundwater levels (that may fluctuate during
the year), quality of construction (materials and workmanship), and the water usage of the family utilizing the systems. These
conditions can vary, and are outside the control of GEG. Satisfactory test results do not guarantee future performance of the
systems; therefore, GEG makes no warranty (express or implied) regarding the future performance of the well or septic system.
GEG makes no representation whether an alternative well or septic system can be installed on the property in the event either of the
current systems fail. The content of this report is for the sole benefit of the person/party who retained GEG. Reliance upon the

Subject: Well Flow Test and Septic System Adequacy Test at T15N, R1 W, Section 18, N2,
N2, SE4, SW4, SE4. Serving the single-family home at 19561 Klondike Drive * Chugiak, AK.
Mr. Eklund:
We have performed a well flow test and a septic system adequacy test for the referenced property
for our client. On March 14, 2019, we went to the site and performed the well flow test and septic
adequacy test. The results are summarized as follows:
A. WELL FLOW AND WELL PIT ISSUE: Prior to starting the well flow test, the static water level
in the well was 14.6 feet below the top of casing (BTC). Over a period of 207 minutes, 721 gallons
were pumped from the well with the water level in the casing dropping to 28.8 feet BTC. Based
upon this data, it was determined that the well flow rate was 3.4 gallons per minute on the day of
our test and exceeds the MOA requirements for a 3 -bedroom residence (0.32 gallons per minute).
The well is located in a 48" diameter steel vertical culvert that is -11 feet deep with the pressure
tank in the bottom of the pit. The top of the casing extends 12+ inches above the existing grade
on the outside of the pit well. On 8/7/2019, GEG consultant, Jody Maus met with you onsite to
discuss this pit well. Due to the fact that the well pit does not have a drain line in the bottom, it
was determined that the well would need to be brought into compliance with the MOA Water Well
Ordinance. The owner is proposing to have the pressure tank removed from the pit and placed
inside the house. The culvert will be then be filled with soil up to the existing grade on the outside
of the 48" steel vertical culvert so that there is no depression around the well head. The well wires
above grade will need to be in conduit. The owner is currently obtaining bids from a
plumber/contractor to perform this work. The owner would like to obtain a conditional COSA prior
to this work being completed.
B. SEPTIC SYSTEM ADEQUACY TEST: Upon arrival, we shot elevations on the drainfield
cleanouts and found the bed out of level by approximately 6 inches. At the time of our test on
3/14/2019, there was only one (1) monitoring tube in the bed and the elevation at the bottom of
the monitoring tube was found to be at the same level as the inverts of the two (2) lower
cleanouts at the west end of the drainfield. The inverts elevations of the 2 higher cleanouts (at
the east end of the drainfield) are approximately 6" higher in elevation than the bottom of the

monitoring tube and were also found to be slightly lower in elevation than the liquid level in 2nd
compartment of the septic tank. Prior to the start of the test, all cleanouts and the monitoring
tube were found to be dry. We added 721 gallons to the two east cleanouts, which caused a
2.75 inch rise in the monitoring tube and water to appear in the west cleanouts, but not in the
east cleanoutf The first 262 gallons introduced caused a rise of 0.5 inches in the monitoring
tube. The next 459 gallons caused an additional rise of 2.25 inches. The water was then shut-off
and the drainfield was allowed to recover. The liquid level in the monitoring tube was checked
120 minutes later and found to have dropped 1.5 inches (1.25" of liquid in MT). We checked the
monitoring tube the following morning (almost 18 hours after water was shut off) and found the
water had dropped an additional 1 inch (a total drop of 2.5 inches). Based upon these readings,
it appears the septic system did pass the adequacy test; however, it did so with a portion of the
bed surcharged. When the new septic tank was installed on 8/6/2019, three (3) new monitoring
tubes were also installed in the drainfield. Per the contractor, theses three new monitoring tubes
(MT's) were installed 6" deeper than the invert of the distribution line where the new septic tank
outfall line enters the bed (on the west end). In short, the 3 new MT's were set deeper than the
existing MT. All MT's were checked on 8/6, 8/7, & 8/8/2019 and found them to be dry. On
8/7/2019, GEG consultant, Jody Maus met with you onsite to discuss the bed levelness and
septic adequacy test. During the site visit, you stated that you would accept the septic adequacy
test data, but an advisory letter addrerssing the deficient construction (distribution lines out of
level) and the fact that the drainfield may be nearing the end of its useful life. In short, you
stated a COSA would be issued, but an advisory letter for the drainfield would be issued.
