Your request for a waiver of the required 100 feet horizontal separation from the septic tank to the
private well has been approved. The approved separation distance is 55.0 feet. This is a reissuance
of WR890071 for the new tank installed in 2012.
This waiver approval applies to the existing septic tank only. Any future upgrade to the on-site
wastewater disposal system will require all separation distances be met or another approval from
this department.

The subject septic tank was installed in July 2012. The tank was sited at the location of the
previous septic tank, which was removed and decommissioned. The new tank replaced the old one.
The existing trench was also installed at the same time and "as -built" by others. The original septic
tank had an approved waiver at 55 -feet from the well, which was granted by MOA in 2004.
The conditions for the waiver are essentially the same, the owners request that the septic tank
installed in 2012 be allowed to remain at the 55 -foot approved waiver distance.
A COSA inspection recently conducted demonstrates the septic system to be working adequately.
Please reissue the waiver. The existing trench is over 100 -feet from the well.

appro~ 5" of fu~ r~ng in the bosom of th~ ~nk.
Excavation w~ p~rformed with a 580 Case Backhoe operated under
American Excava~t/ng. Excavation of the tank involved a (SW) w~ll
graded ~andy ~oll. The tank was approximately 64" in diameter, length
of 12' and a capacity of approximately 2,000 gallons.
Soil ~amples were taken from the excavation after the tank had been
removed. During and after excavation there were no petroleum odors
detected or visible sign6 of tank l~akage. The tank was i~pected and
v~ua~ly appeared to be in good condition. Soil 6~mples were taken at
random from the bottom of the hole and along the sidewaY. The
result6 of these tests indicated a TPH level of 172 ppm under EPA
method 418.1 in the hole, a diesel product was verified under modified
method 8015 and a TPH lev~ of 356 ppm under EPA method 418.1 from
random 6~mpling of the pile. There were no BTEX or aromatic volat~e
organic6 detected under EPA method 8020.
The contractor ouer-cxcavated the hole at your direction along the
6ide~6 and the bottom. New 6oil samples were ~ken and checked with
a PlO0 PID organic vapor meter and samples were 6ubm~tted to CHEMICAL
AND GEOLOGICAL LABORATORIES for analysis under EPA method 418.1 for
verification of .PID results. TPH 34.5 ppm were detected under
laboratory analysts.

Dear Mr. and Mrs. Hoffmann:
At the request of Mrs. Hoffmann, the department has reviewed all
the information on file regarding the issuance of waivers,
health authority application, and notice of violation on the
above property which you own. We have found a number of
inconsistencies and emissions which may have resulted in some
confusion in handling your health authority approval. As a
result, we will make the following adjustments to the record.
These adjustments supercede any previous correspondence which
you may have from our office.
Waiver request #WR890071, granting a separation
distance approval of 85 feet from your well to the
absorption trench on Lot 119, is reinstated.
Waiver request #WR890071, granting a separation
distance approval of 90 feet from your well to the
absorption trench on your lot, is reinstated.
A re-evaluation of the separation distance from your
well to your septic tank has shown that ADEC criteria
for protection of your well have been met; therefore,
you are granted a separation waiver to 55' You are
not required to move the septic tank unless you wish to
do so.
The currently revoked Health Authority Certificate will
remain disapproved until the fuel tank located
approximately 10 feet from your well has been removed.
After removal is accomplished, please have your
engineer confirm this action in a report to this
office. We will then be able to complete our review on
the Health Authority application.

The issuance of a Health Authority Approval for Lot 120 (HAA #
HA890517 dated February 28, 1990) was predicated on the
relocation of the septic tank to a site outside of the 100 foot
protective well radius, and the removal of an abandoned fuel
tank with the underlying soil checked for contamination. Since
neither of these conditions have been satisfied, the Health
Authority Approval (HA890517) and the attendant waiver
(WR890071) have been rescinded. A copy of the letter rescinding
the HAA and waiver is attached.
As a result, the attached Notice of violation #4384 has been
issued requiring that the encroachments of the drainfield and
septic tank to the well on Lot 120 be brought into compliance
with municipal code requirements by not later than September 20,
1990.

Dear Mr. Shafer:
On February 28, 1990, this office issued waiver WR890071
covering the following setback distance variances:
1. 90 feet from the drainfield on Lot 120 to the well on
Lot 120, and
2. 85 feet from the drainfield on Lot 119 to the well on
Lot 120.
This waiver was granted with the conditions that the septic tank
on Lot 120 be relocated outside the 100 foot well protective
radius, and the abandoned fuel tank on Lot 120 be removed with
the underlying soil tested for contamination. These conditions
were specified in your letter dated February 21, 1990. As of
this date the above conditions have not been satisfied and there
is no indication that they will be satisfied in the future.
Therefore, please be advised that waiver WR890071 is rescinded
effective this date. With the waiver being rescinded, the
drainfield on Lot 119 and the septic tank and drainfield on Lot
120 are not in compliance with municipal and state regulations.
Also please be advised that the Health Authority Approval (HAA #
HA890517) issued by this office on February 28, 1990 is
rescinded effective this date. This Health Authority Approval
was issued based on the conditions of the waiver being satisfied.

Attached is a grid map (sc~e = I"-400') with the referenced property
outlined in red. On this map we have shown dots to r~present the
locations of several area w~lls for which w~ll logs are available.
Next to each dot is the depth to a clay layer as indicated on the w~l
logs. Also on the grid map we have transposed contour lines giving the
topography of the area. In two locations where w~ data most
available we have drawn a dashed lin~ and calculated the slope of the
clay layer taking the area topography into consideration. One dashed
line ~ to the south of the referenced property and the other is to the
north. It appears the clay layer slopes from the east to the west at
approximately 6.2% in the area south of the referenced property. It
~so appears the clay layer slop~ from the east to the west at
approximat~y 13% in the ar~a north of the r~ferenced property.
Therefore, within the referenced property, the confining clay layer
se~ms to trav~ from the east to the west at a slope between 6.2% and
13%. This shows that the my layer (water table gradient) has a slope
in excess of +39% away from th~ w~ll.
With this additional information we request that you grant the
referenced waivers from the w~ll on the property and the two
encroaching leachfields with the following ~onditions:

Dear Mr. Shafer:
Your request for waivers of separation distances between the
on-site well and septic tank of 55 feet, the on-site well to
absorption field to 90 feet and the on-site well to absorption
field on the adjoining lot of 85 feet are denied. We believe
that your request (reference your letter dated December 18,
1989) errantly transposed 10t number 120 and 119.
By using the absolute optimum point allocation under ADEC
guidelines, these requests fall far below the minimum required
16 points. Under no circumstance will a waiver be issued for
the 55 foot separation between the well and septic tank on Lot
120 The alternatives to the alver request you submitted are as
follows:
1. Relocation of the well on Lot 120 to a site outside al~l the
required 100 foot setback distances, or;
2. Furnish data which establishes that the water table has a
slope of at least +3% (away from the well) and relocate the
septic tank on Lot 120.

TEST PROCEDURE
~,~,~,~,~: The leachfield was filled with ~,~ater from an on-site well
at an average rate of 2.3 GPM for a total of 540 gallons in 231
minutes.
The septic tank and ]eachfield liquid levels were monitored
referencing a measurement below the top of the standpipes. During
the test, water was added through the trench cleanout tube and
the ]eachfield water level was monitored as water was added and then
absorbed into the surrounding soil,
The water level in the septic tank did not rise during the
addition of 540 gallons of water shewing that the leachffeld had
accepted the entire amount. A total rise in liquid level in the
leachffeld to the leach pipe level was recorded as 19". The monitor
tube level was maintained at full capacity for the duration of the
test establishing and average in flow rate of 2.3 GPM into the leach
area. An additional +3480 gallons of water was then added at 2 GPM
to confirm the inflow rate. Measurements were taken of the leachfield
liquid level, after the test period with satisfactory reduction in
liquid level. The monitoring indicates the septic system will accept
the required 150 gallons per day of effluent, per bedroom, which is
the required absorption rate for Municipal approval.

~.& The well was pumped et a steady flow rate of 11.5 GPM. Total
drawdown from a static water level of 7.2 feet was 30 feet after 30
minutes of pumping. At this point the water surface level in the well
was static et 38 feet below the top of well casing. With the use of
an electronic sonar, the yield rate of the well was monitored and
calculated at a maximum rate of 11.5 GPM. This yield rate is the
maximum rate that can be established by use o¢ the installed pump.
TEST RESULTS
The well yfeld or flow rate and septic system soil absorption
rate meets the requirements of the Municipality of Anchorage for a 3
bedroom single family residence es of the day the system was tested.
There are two separation distance waivers required for full Health
Approval.
Assessment of existing subsurface conditions by the inspecting
engineer is limited to information obtained from the available
monitoring tubes and Municipal record search. We do not guarantee
the valldity or quality of subsurface tests and inspections performed
by the original inspecting engineer or authority. This report is
limited to absorption rate testing and surface separation measurements
as currently required by the Municipality o¢ Anchorage and does not
verify the fntegrfty of the pfpfng for the water supply or water
quality other than the bacterial and nitrate content.
The operational life and the matter of compliance with
Mg.~,~,~R,,~,,,.,G~,~ for all water and septic systems depends on the local
soil conditions, groundwater levels that may not be observed from the
surface without additional testing, water usage of the homes being
served by the system, and the detail of required testing procedure.
Septic systems expire with use and future environmental concerns may
require more extensive testing, which could render the leechffield
unusable. This is true o¢ all septic systems.
There is no guarantee that the well and septic system tested will
meet the requirements for approval in the future. The test data and
investfgatfon of existing conditions frs provided to our client for
submittal to the Municipal Health Department for their review and
approval. Any concerns with this test report should be discussed with
the testing engineer. If it is requested we will submit the report
directly to the Municipality.
