this department.

RE: T15N R 1 W SEC 18 LOT 123—WELL TO SEPTIC TANK WAIVER
Per previous discussions with MOA On-Site staff on the referenced non-permitted septic
tank that was installed in 2003 by the now-deceased owner, we respectfully request a
waiver of 68' from well to septic tank.
The well is up gradient and located on the opposite side and inside the house from the
tank. The nitrate sample results were ND. The plastic tank installed is a better tank than
the original steel tank that was allowed to be within 50' of the well at time of
construction. Water levels, the soil sorption and permeability of the area would indicate
low probability of any influence between the well and septic tank. Area well logs show
silty gravel and bedrock and test holes show slow percolating soils (20-40 min / inch).
Area nitrate results are also very low and mostly ND. Attached is a representative sample
of MOA record docs of the immediate area.
The existing well would be almost sure to be free from any form of sewage
contamination or influenced by the location of the septic tank. We do not expect there
to be any adverse effect on the subject or adjacent lots by the location of this existing
well and septic tank and request issuance of the waiver. Please contact Brent M.
