To whom it may concern:
The existing 3 bedroom house is served by a community well and private septic system. The existing
septic tank has collapsed and sewage is backing up into the house, we are requesting an emergency
review of the design and issuance of the permit. We are proposing to remove the existing 1000 gallon
septic tank and replace it with a new 1000 gallon septic tank.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title [except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my
investigation, based on procedures outlined in the Health Authority Appreval Guidelines for this application,-
shows that the on-site water supply and/or wastewater disposal system is(are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the .
information obtained from the Municipality of Anchorege files and from my investigation and inspection, the '
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

In conducting this evaluation, AKWWC, Inc. a~tempted to provide a thorough,
conscientious engineertng analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regu/ation[ The reported results desc#bed the performance cf the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of ali wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate durtng the year, and the water usage cf the family being served by the system.
These conditions are outside the control of the eva/uator cf the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. AKWWC, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the so/e benefit of the owner listed above. Any reliance upon or use of this report by any

Dear Mr. Luther:
Per your request of July 7, ]980, we conducted a test of the sanitary sewer
system on the above described property.
The septic tank was pumped prior to the performance of the test on the seep-
age pit. During the test the liquid level in the seepage pit was monitored
as 465 gallons were removed and 700 gallons were pumped back into the seepage
pit to the inlet level. All liquid levels were measured below the top of the
standpipe and are shown in the following table:

With the addition of 700 gallons of water the level in the seepage pit rose
6.7 feet indicating a capacity of 8.7] gallons per inch. After twenty four
hours the liquid level was measured again at 8.40 feet. It had dropped 3.65
feet or 43.8 inches. This indicates an average effluent acceptance rate of
380 gallons per day for the surrounding soils. If the 3 bedroom residence on
the property is to house 6 people, the average load on the system can be
expected to be 450 gallons per day. As this indicates a failing condition a
second test was conducted on July 16, 1980~ at which time the surge capacity
of the seepage pit was tested. 120 gallons of water were introduced into the
pit before backing up into the septic tank. The surge capacity for a three
bedroom house should be 400 gallons. We can therefore conclude that the
system is not disposing of effluent at an adequate rate for a 3 bedroom
residence, v-~ ,~
The tests was performed on July 9th and ]6th, 1980, and they indicate the
adequacy of the system at the time tested. Test results are subject to the
conditions of the septic tank, seepage pit, soil, and groundwater as affected
by weather and the extent of use of the system.
