Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.

Ladies and Gentlemen:
I am writing to request a permit to upgrade the septic system for the above referenced property. The
proposed system is designed for a 4 -bedroom single-family residence. The existing system is no longer
functioning adequately and must be replaced. A soils log, design calculations, a site plan, design drawings
and construction specifications are enclosed for your review.
Design Cates: Groundwater was observed at a depth of 6 feet below ground surface (May 19, 2011)
Soil Rating. From Testhole (5/19/2011)
2.8 min/in = 1.2 gal per sq.ft./day
4.0 min/in = 1.2 gal per sq.ft./day

Required Area per Bedroom: 15011.2 =125 sq.ft.
Total area required: 125 x 4 = 500 sqft
System: The proposed system consists of a 1500 gallon STEP tank and a shallow trench absorption
field. The absorption field will consist of 2 trenches 50 feet long and 5 feet wide and contain 1 foot
(575sq.ft. effective) of sewer rock.
Soils: A test hole was excavated on May 19, 2011. See the attached soil logs. Ground water was
monitored for through June 3, 2011. In test hole #1, ground water was observed at 6 feet on the day of
excavation and at 7 feet below the ground surface on June 3. During the excavation of test hole #2 minor
seeps of water were observed between a depth of 4 and 7 feet. These appear to be small pockets of melt
water, as frost was still present in the soil. On May 20t', 2011 they had dried up and disappeared.
Surface Water: No surface waters were observed within one hundred feet of the proposed septic system
upgrade.
Topography: The lot slopes to the west at approximately 5%.

The installation of this septic system will not prevent wells and septic systems from being installed on the
adjacent lots. The proposed septic system will not change the general slope of the area. Ponding and/or
concentration of surface runoff will not result from this installation.

Construction and Material Specifications
Subject Property: Valli Vue Estates #2 Block 2 Lot 6
Date: June 6, 2011
General:
1. The owner shall contract with a contractor to perform the work outlined in these
specifications and plans in accordance with the attached MOA permit. There is not
contractual arrangement between the contractor and the Engineer. The Engineer shall
be the owner's representative and will inspect the work to document the contractors
activities. Final acceptance of the contractors work rests with the owner and the MOA.
2. The scope of the project includes the procurement and installation of a 4 -bedroom on-
site wastewater disposal system at the location depicted on the attached site plan
drawings. The scope of the work includes the installation of a 1500 gallon STEP tank
tank, a drain field comprised of 2 trenches, 5 -feet wide x 50 feet long, with a maximum
depth of 3 feet, with 1 foot of MOA filter sand and 1 foot of septic rock below the
distribution piping. All components of the proposed septic system must be constructed a
minimum of 100 feet from any surface water or water wells in the area. A minimum of 4
feet of earthen cover is required over the septic tank and a minimum of two feet of cover
is required over the drain field. In addition to the required earthen cover, 2 -inch thick,
40 psi, direct bury board foam shall be installed directly over all installed piping.
3. Construction shall be in accordance with the approved site plan and design drawings and
any special provisions noted on the Municipal well and/or septic permit. All materials,
components, workmanship and construction practices shall comply with the Municipality
of Anchorage's latest ordinances regarding wells and septic systems, as well as MOA,
Development Services Department, On-site Water and Wastewater Program (DSD)
policies.
4. The contractor is responsible for obtaining all underground utility locates, layout of the
proposed system, and verifying and maintaining all required separation distances to lot
lines, surface water, wells, steep slopes, etc.
S. The property owner shall be responsible for final grading any depressions formed from
settlement of backfill material. The property owner is also responsible for vegetation of
all areas disturbed by septic system installation activities.
6. Contractors installing wastewater disposal systems must be certified to do so by the MOA
DSD, On-site Water Wastewater Program. Property owners installing their own systems
must receive advanced approval from MOA DSD prior to system installation activities.
7. The contractor shall ensure that ALL materials being used are in compliance with the
most current guidelines established by the Municipality of Anchorage. Refer to
"Standards and Specifications for Component Parts and Materials Used in the

Septic Tank Installation:
1. A new septic tank, of the size specified in the design drawings, shall be procured from a
qualified manufacture/supplier if the existing septic tank is found to be defective.
Construction shall include two 4" diameter cleanouts for pumping access.
2. The burial rating of the tank shall be adequate for the actual burial depth encountered.
3. The tank shall be level and sufficiently bedded to prevent settling of shifting of the tank.
4. All standpipes on the tank shall extend a minimum of 12inches above the final grade.
S. Tanks installed with less than 4 feet of cover shall be insulated.
6. The sewer service line from the house to the tank shall have a minimum slope of 2%
unless prior approval is obtained from the engineer.
7. The inlet and outlet lines from the tank shall be bedded in drain rock or the soil shall be
sufficiently compacted to prevent future settlement of the soil.
8. Septic tank inlets, outlets and cleanouts shall be fitted with water tight couplings.
9. A cleanout shall be installed 1 to 4 feet from the foundation.
10. Post tank cleanouts shall be installed per MOA requirements. Does not apply to
pressurized lines.
11. In areas with high ground water, the septic tank shall be sufficiently anchored to resist
buoyancy forces when there is not adequate soil cover to resist uplift force. The
contractor shall refer to the engineer for direction regarding this matter.
12. The tank backfill shall be graded sufficiently to ensure positive drainage away from the
tank.
13. All holding tank and lift stations shall have an audible and visible alarm inside the
residence being served.
14. All lift stations shall be wired in compliance with all MOA guidelines and the Uniform
Electrical Code. A municipal electrical permit and inspections must be obtained where
required by the MOA.
15. The contractor shall verify that the septic tank/lift station is a minimum of 100 feet from
any residential water well, 150' from any class 'C' well and 200 feet from any community
water wells. Additional separation distances must also be maintained.
16. The contractor shall remove or abandon the existing tank in compliance with the Uniform
Plumbing Code.
Drain field Construction:
1. The drain field shall be excavated to the dimensions outlined in the design drawings. If
any discrepancies are noted, contact the engineer immediately for clarification.
2. The drain field shall be installed parallel to slope contours.

3. The bottom of the drain field shall be within 2" of level. If the sidewalls or bottom of the
excavation become smeared, they must be raked or scarified prior to the placement of
filter sand or drain rock.
4. The drain field must be a minimum of 5 feet from the test hole excavation.
5. Distribution pipe is to be installed level with perforations facing downward, atop a level
bed of drain field rock. Septic rock shall then be placed atop the distribution piping to
provide a minimum of 2" of cover, level across the entire width and length of the trench,
above the piping.
6. A silt barrier must be installed at between the final drain rock and the native soil backfill.
The silt barrier shall cover the entire drain rock surface.
7. Monitor tubes must be 4" in diameter, installed in the locations depicted on the design
drawings, and extend a minimum of 12" above final grade. The portion of the
monitoring tube below the septic rock must be perforated and the portion above the
drain rock shall be solid.
8. Backfill over the final gravel layer must not be less than 2 feet. Insulation must be
installed when the backfill depth is less than 3 feet.
9. Drain field back fill shall be mounded to prevent depressions from forming due to
subsequent settlement of backfill material.
10. The contractor shall verify that the drain field is a minimum of 100 feet from any
residential water well, 150' from any class'C' well and 200 feet from any community
water wells. Additional separation distances must also be maintained.
Inspections:
1. A minimum of two inspections are required by the Municipality of Anchorage. These
inspections must be conducted under the supervision of a Professional Engineer licensed
to practice in the State of Alaska.
2. The first inspection must be conducted after the excavation of the drain field and before
the placement of any gravel. The septic tank may be set in place, but may not be
backfilled.
3. The second inspection must be conducted after the placement of the silt barrier, gravel,
distribution lines, stand pipes, cleanouts, and insulation, but before the placement of any
backfill.
4. The contractor shall provide a legible copy of all field notes relating to system layout and
construction to the engineer for the use in preparing the record drawings.
5. The contractor shall coordinate all inspections with the Engineer. A minimum of 24 hours
notice is required prior to the start of construction. If necessary, a preconstruction
meeting will be held on-site.

Dear Dr. Reid:
At your request, we are providing the following written comments
with respect to the subject permit application submitted by your
office. Our comments are based on design information which you
provided with the permit application as well as historical
information contained in our files. ,
Page 1 of 4 indicates that the invert piping of the drain
field will be located 5.0 feet below grade and that the
total thickness of the drain rock below pipe invert will be
4.5 feet. This conflicts -with the cross section detail on
page 3 of 4 which indicates that the drainfield invert will
be located 4.5 feet below grade and that the total thickness
of drain rock will be 5.0 feet. It is our understanding
that the existing system may be operating inadequately as a
result of the drainfield not being installed at the proper
grade. Therefore we consider it important that this
discrepancy be clarified prior to issuance of the permit.
The soils log, which serves as the basis for design of the
upgrade, indicates that a perc test was conducted between
1.1 and 1.6 feet below grade. However, the proposed
receiving soil strata is located between 5.0 and 9.5 feet.
Your design is based on the assumption that the soil strata
between 5.0 feet and 9.5 feet will have the same perc rate
(6 min./in.) as the soil strata between 1.1 feet and 1.6
feet. Given that the original system was designed using a
perc rate of 24 min./in. and adjacent lots 5 and 7 were
perced at 30 min./in. and 60 min./in. respectively, there is
some concern that the perc test conducted may not reflect
the perc rate of the receiving soil strata.

SPECIFICATIONS FOR A TRENCH -TYPE WASTEWATER DISPOSAL SYSTEM
LEGAL DESCRIPTION: LOT 6, BLOCK 2, VALLI VGE SUBDIVISION #2
1.0 GENERAL
1.1 The Drawings, sheets 1 thru 4, shall be a part of this
specification.
1.2 All materials and workmanship shall meet the
requirements of the Municipality of Anchorage,
Department of Health & Human Services (DHHS), the
conditions of the permit, and all applicable rules
and regulations currently in effect.
1.3 All elevations and depths are advisory, and are to
be verified or modified in the field by the
engineer or inspecting agency.
1.4 It is the responsibility of the property owner or
installer to adhere to approved designs for
installation, maintain the specified separation
distances, and have the appropriate inspections.
1.5 It is the responsibility of the property owner or
installer to report to the engineer any observed
conditions which would put the sytem in violation
of state or Municipal regulations.
1.6 If the installation is not inspected by an AECS
engineer, AECS will not be responsible for the
installed system. An engineer at AECS should be
consulted prior to construction to determine the
number of inspections that will be required and to
explain what these inspections will involve.
2.0 SEPTIC TANK
2.1 If there is an existing septic tank, it may be
used it if meets the capacity requirement for the
residence and the approval of the MCA.
2.2 .The septic tank shall be a UPC --approved
two-compartment tank, constructed of 12 gauge
steel with bi.tumastic coating and set level on
undisturbed soil. If the tank is buried at a
depth of 4 feet or less, it must be insulated with
an overlying layer of 2 inch burial type
polystyrene rigid board insulation.
2.3 The septic tank and trench shall be a minimum of

100 feet from any private well or body of water,
150 feet from Class "C" wells, and 200 feet from
Class "A" or "B" wells, unless otherwise
specified. Less than the required separation distance
must have prior approval. or waiver by ADEC or MOA.
2.4 The septic tank shall be a minimum of 5 feet from
the house foundation, and a minimum of 5 feet from
the absorption area.
2.5 Piping shall be fitted with a mechanical
watertight calder coupling on the outlet and inlet
of the septic tank. Piping shall be 4 inch solid
PVC ASTM D-3034 or cast iron, sloped a minimum of
1/4 inch per lineal foot. If the piping is buried
at a depth of 4 feet or less, it must be insulated
with an overlying layer of 2 inch burial type
polystyrene rigid board insulation.
2.6 Cleanouts shall be installed as designated and
capped with air -tight rain caps (Jim Caps or
equivalent), and extended a minimum of 1 foot
above ground level.
2.7 If a lift station is required it shall be a
combination lift station septic tank per Anchorage
Tank and Welding, Inc. design. Specifications and
design drawings are on file with the Municipality
and the engineer.
3.0 ADSORPTION AREA
3.1 The gravel for the trench shall be 0.5 to 2.5
inch, screened rock with less than 3 percent
passing the No. 200 sieve. All substitutes must
have prior DHHS approval.
3.2 The bottom and sides of the excavation shall be
raked with the backhoe blade to ensure that it has
not been compacted during excavation. The bottom
elevation shall be level.
3.3 Monitor standpipe(s) shall be placed as shown in
the drawings, and shall be 4 inch rigid PVC ASTM
D-3034, or cast iron. The section shown with
holes may be 0.5 inch holes drilled on 6 inch
centers on opposing sides of the pipe, or a
regular section of perforated sewer pipe clamped
to a solid section with either a no hub coupling
or a solvent joint. A rubber rain cap (Jim Cap or
equivalent) shall be installed over the top of the
pipe.
3.4 The distribution pipe shall be perforated 4 inch
PVC with a minimum crush strength of 1500 pounds
and shall meet the approval of DHHS for use as
drainfield pipe. All distribution pipes shall be laid
level.
3.5 Trenches may be paralleled, but must have a
minimum separation distance between the trenches
of 10 feet or 2 times the gravel depth (which ever
is greater). 75 feet is the maximum allowed
linear length of any trench.
3.6 If the final grade over the trench is less than 4
feet above gravel, insulation is required, using
burial type polystyrene rigid board insulation.
There shall be 1 inch of insulation for every foot
of soil less than the required 4 feet of cover,
but there must be at least 24 inches of soil cover
even though insulation is used. The solid pipe
extending from the septic tank to the drainfield
shall also have a minimum of 4 feet of cover or an
equivalent layer of insulation to prevent freezing
of the line.
3.7 If insulation is not necessary, the gravel shall
be covered with a layer of nonwoven Geotextile
fabric (such as Mirafai, Fibretex 200 grade, Poly
Filter X or equivalent).
4.0 INSPECTIONS
4.1 A minimum of two inspections are required for the
installation of the trench. The first inspection
will be of the open excavation to assure that the
system is installed in the proper soil strata,
correct depth and meet minimum specified design
parameters.
4.2 The second inspection will be after placement of
the gravel, monitor standpipe and distribution
pipe to verify proper installation and position
prior to backfill.
4.3 The inspection of the septic tank installation can
be incorporated with any one of the above listed
inspections.
4.4 The lift station will require either an MOA
electrical inspection or certification by a
licensed electrician, depending on whether the
building code applies to this part of the city.

Date of Test: October 22, 1995
Test Procedure: System was reinspected and measured. Tank was found with 5 feet of cover. Liquid
depth was measured to be 77 inches. Three clean -outs and a monitor tube were found. The post tank clean-out
was measured to be 71 inched deep and dry. Clean-out one was measured to be 97 inched deep and dry, but
showed signs of being wet in the near past. Clean-out two was 84 inches deep and dry. The monitor tube had a
total depth of 127.5 inches with 25 inches of fluid.
Water was added to the system at a rate of 3.8 g.p.m. The water levels in the tank and monitor tube was
monitored. Four hundred and seventy gallons of water was added over a 125 minute period. The level in the
monitor tube rose from 25 ' hes deep to 40.5 inches deep. No water was observed to be flowing back into the
tank. Twenty four hours rthe water level in the monitor tube was measured to be 25 inches.
TESTS RESULTS: This system meets the code requirements of the Municipality of Anchorage.
The operational life of all septic systems depend on the local soil condition, ground water levels that may
fluctuate during the year, and the water usage of the family being served by the system. These conditions are
outside the control of the evaluator of this system. We can therefore not give any estimate of how long the
system will continue to meet the operational requirements of the Municipality and State.

Alaska Drinking Water Regulations.
Please note that departmental records indicate that the public
water system was installed prior to the 1978 implementation of
the Alaska Drinking Water Plan Review regulations. No as—built
plans have been reviewed or approved by the department, nor are
any necessary. Since the system has submitted acceptable water
samples on a regular basis and received a satisfactory sanitary
survey evaluation by the department, the system is acceptable
under the standards in effect at the time of installation. An
official "Certificate to Operate" may be issued upon receiving a
complete set of as—built plans. Any expansion of the water
system after 1978 will require plan review and the issuance of a
"Certificate of Operation" permit.
