2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (24/7).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing

RE: STEP SEPTIC TANK UPGRADE PERMIT
LEGAL: VALLI VUE ESTATES #2 BLOCK 2, LOT 15
We have been requested to obtain a septic permit to upgrade the existing aged steel STEP septic
tank on the above referenced lot. We propose to install a 1500-gallon HDPE tank per the
attached design to serve the existing 4-bedroom residence. No groundwater was noted in the
subject and adjacent MOA on-site files, but if groundwater is encountered during installation an
epoxy coated steel septic tank may be required. The lot and area are served by public water. The
design will not impact any of the neighboring properties. Please contact us if you have any
questions.

I have located the water service line for the O'Malley fire station and painted marks on the ground. I have also used that
information to update our map and have attached that map to this email.
I also could not get any signal going to 6161 O'Malley, so I don't believe that property has a water service.

The subject permit, issued October 9, 1997 by this office for a
single family well and/or on-site wastewater system, has
expired as of October 9, 1998.
A new permit must be obtained from this office for a well
and/or on-site wastewater system NOT installed by the
expiration date.
If you have drilled the well, a well log must be sent to
this office for documentation of the installation and to
close the permit.
If a licensed Professional Engineer has inspected the
installation of the on-site wastewater system, the original
as-built inspection report must be sent to this office for
review, approval and documentation. Ail inspection reports
must be submitted within 30 days of construction completion.
When applying for a new permit, the fees are: $320.00 for an
on-site wastewater permit; $120.00 for a well permit and
$440.00 for a combined on-site wastewater and well permit.

Gentlemen:
My firm was contacted to investigate the possibility of installing of a replacement system at the above
referenced property. The existing trench type system has failed. The owners report slow draining of the
lower fixtures and minor backing-up of the drains. The system was installed in 1975. I was requested to
investigate the possibility of installing a new system adjacent to the existing system. A single test holes was
excavated on July 15, 1996 The soil report and percolation test results are attached. Additional soils will be
verified at the time of installation. No ground water was encountered during the seven day monitoring period,
but was measured to be 14 feet below ground level by MOA DHHS personnel on August 12, 1996.
The tot is approximately 1.25 acres in size and is served by a community water system, h slopes to the
southeast at a rate of approximately one to two percent. The proposed installation will be located greater
than 100 feet away from any well and 25 feet from the water service lines. The surrounding systems are
located greater than 100 feet from the proposed installation. The existing septic tank wilt be inspected and if
found competent, reused. A new tank will be installed if the existing tank is not found competent. The
existing soil absorption field will be reused. A diverter valve will be installed between the septic tank and the
two drain fields. The new drain field will be selected for use.

The Flow Divider automatically back flows t}om

depend on the homeowner ro change the sleeve
