We oriqinally proposed to replace the existing tank on the subject property with a 1000 gallon septic tank. During the
installation, the contractor (A+ Home Services) noted that existing sewer lines had insufficient drop between the house and

To whom it may concern:
The existing 3 bedroom house is served by a community well and a private septic system. The septic tank
was installed in 1973 and the homeowners would like to upgrade their tank. We are proposing to install a
new 1000 gallon septic tank off to the east side of the tank so that the new tank will have a 10 foot
separation from the water service line.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic systems.

conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the bTne of the test, and separation
distances measured to readily identifiable features. Tho operational life of all wells and
septic systems depend on the local soils condition, groundwater Iovels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of tho evaluator of the system. Satisfactory test

To whom it may concern:
The subject property is served by a Class A well (PWS#210605) and a private septic system.
During our inspection, we suspected the water line may be less than 10 feet from the septic tank.
The homeowner retained Water Systems Services, LLC to locate the water line. The tank
cleanout is less than I0 feet from the water line location, meaning the tank is even closer
underground. Attached is a drawing showing the location of the water line and septic system.
Also, the sewer service line from the house is less than 5 feet from the water line. We request
that your department issue a 5-foot waiver between the existing septic tank and existing water
line. We also request a 4' waiver between the water line and sewer line. The water line is made
of copper. The septic tank and water line have been in place for more than 35 years without any
apparent problems. I am unaware of any adverse impacts these waivers would have on adjacent

Tree Drive using a radio frequency type underground line Iocator. The separation distance between
the water line and the apparent sewer line location measured a minimum of 9 leer and a maximum of
13 feet. It must be noted that particularly on deep lines such as water lines, the accuracy of RF lype
line loc. atom is +/- 2 feet.
