The subject permit, issued April 27, 1998 by this office for a single family well and/or on-
site xvastewater system, has expired as of April 27, 1999.
A new permit must be obtained fi'om this office for a well and/or on-site wastewater
system NOT installed by the expiration date.
If you have drilled the well, a well log must be sent to this office for documentation of the
installation and to close the permit.
If a licensed Professional Engineer has inspected the installation of the on-site wastexvater
system, the original as-built inspection report must be sent to this office for review,
approval and documentation. All inspection reports must be submitted within 30 days of
construction completion.
When applying for a new permit, the fees are: $320.00 for an on-site wastewater permit;
$120.00 for a well permit and $440.00 for a combined on-site wastewater and well permit.

We are submitting an applications for the installation of wells and septic systems for the five lots
of this subdivision. Originals of the soil investigation performed by DHI Consulting Engineers
are on file with the DHSS. Two additional soil logs are attached to this application. These two
testholes are identified with the initials TS. Groundwater was not observed in any of the
testholes.
The soil is fairly uniform throughout the subdivision, consisting of a fine sand, silty sand,
overlaying clean sand and gravel. The percolation rate of the fine sand was found to be less than
5 minutes per inch in several of the testholes. However a percolation rate of 6~15 was used for
design at all the lots. Tiffs approach resulted in the a trench length of 75 feet for lot 3, leaving a
portion of the proposed trench outside of the 30 foot radius around the testhole. If the observed
perc rate was used the length oftl',e trench would be 50 feet, placing the whole trench within the
30-foot circle.
To facilitate review I have shown all the proposed septic systems and the proposed well locations
on a sheet 1. For Lots 2,3,and 4, an alternative well location is shown. The specific design
assumptions are listed for each lot. The proposed well and se ~tic improvements will not impact
the adjacent properties or change the surface/subsurface drain,age of the lots.

The iostallation of this septic system will not prevent wells from being installed on the adjacent lots.
There are no developed or natural surface / sub surface drainage conrses on this or the adjacent lots.
The proposed septic system will not change the general slope of tile area. Ponding and/or concentration of surface
runoffwill not result from this installation. Surface ponding does occure during breakup. This ponding does not
affect tile proposed drainfield.
