b. Covered, sealed, and heated to prevent freezing

SPECIFICATIONS & DESIGN GUIDELINES
Wastewater System Sizing: This is an existing 3-bedroom, single family home. This is a
developed subdivision. A COSA inspection revealed that the 1500 gallon STEP tank
requires replacement. The existing tank to be decommissioned. No adverse impacts are
expected from STEP tank upgrade. No conflicts to the other lots will take place by this
septic system construction upgrade. The existing mounded absorption field was tested by
adding 1200 gallons of water over a two-day period; a field monitor tube was observed
and the tank drain-back was monitored. Easements depicted in the area, public water
serves these lots.
Specification Requirements: All components and work must comply with the
Municipality of Anchorage Specifications (AMC) & State of Alaska Drinking Water
Regulations and Wastewater Regulations.
New 1500 Gallon STEP tank. MOA-Approved. Steel tank.
Decommission old tank per UPC. MOA-Spec Material Under Tank.
10 minimum between the tank- trench, tank- house. 10 to property lines,
4 of cover or insulation is required for tank; 2 Minimum thickness for insulation can
substitute for 1 cover.
Tank & solid pipe must be set on well compacted, stable soil.
4 inch diameter cleanouts with airtight caps are required 1 to 4 from foundation
wall, prior to any 90 degree bend in 4 inch line, in each tank compartment, and two
adjacent opposing cleanouts between the tank and the absorption field, not more than
10 from the tank positioned to provide cleanout access towards the tank and towards
the absorption field.
All cleanouts must extend to at least ground level.
In solid pipe runs, ASTM D-3034 may be used in lieu of cast iron.
Force main to be 1.25 Schedule 40 PVC or equal.
The finish grade must be mounded to promote drainage away from excavation.
Insulation must be placed over any pipe installed under driveways or parking areas.
Approved pipe materials include cast iron, PVC ASTM D3034, PVC ASTM F789,
ABS ASTM D2661,
Sewer Service Line is minimum 2% slope.
Septic Tank to be pumped every two years or when required.
Insulation board to be extruded direct burial polystyrene (Dow Styrofoam HI/equal)

Request you issue a permit to replace the existing septic
tank and lift station serving the 3 bedroom home on the
referenced property with a 1250 Septic Tank Effluent Pumping
system.
The existing septic tank and lift station are of
questionable integrity. The existing septic tank and lift
station are 85 feet from a small stream. The new S.T.E.P.
system will be installed an appropriate distance from the
small stream. A site plan with the proposed location of the
S.T.E.P. system is attached.

GENERAL:
The scope of this project includes the installation of a
1250 gallon wastewater S.T.E.P. system (septic tank) to
serve the three bedroom residence located on the
referenced property. The existing septic tank is to be
excavated, pumped, crushed, and abandoned in place.
Construction shall be in accordance with the approved
site plan and design drawings; Municipal permit with any
special provisions or conditions; and all applicable
State and Municipal Wastewater Disposal Regulations.
The contractor shall be responsible for obtaining any
necessary underground utility locates.
Unless specifically agreed otherwise, the property owner
shall be responsible for final grading areas subsequently
depressed from soil settling. On all leachfield mound
systems, the property owner shall be responsible for
ensuring a satisfactory vegetation growth over the
mounded area.
Contractors installing wastewater disposal systems must
be certified by the Municipal Health Department for
system installations. Owners installing their own
systems must also receive prior approval from the
Municipal Health Department.
SEPTIC T~NK EFFLUENT PUMP SYSTEM INSTALLATION=
A septic tank is to be constructed by a certified septic
tank manufacturer. Construction shall include two 4"
c].eanouts for pumping access.
The septic 'tank shall be sufficiently bedded to prevent
settling or shifting of the tank.
All standpipes on the septic tank shall extend a minimum
of 12 inches above final grade.
4. Septic tanks installed with less than 4 ft. of cover
shall be insulated.
5. Final grading over the septic tank shall be such that a
positive slope exists away from the septic tank.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections
required during the installation of the wastewater disposal
system. These inspections will occur as follows:
The first inspection must be conducted after the
excavation of ditches, pits, trenches, or beds and
before the installation of any gravel. A septic
tank may be set in place, but may not be backfilled
before this inspection.
The second inspection must be conducted after the
placement of the silt barrier, gravel, distribution
lines, standpipes, cleanouts, and insulation, but
before the placement of any other backfill.
The final inspection is to occur upon final grading
of the property.
Often there will be more than these 3 inspections required.
Especially with the installation of multiple trenches, sand
filters, pressurized distribution systems, etc. Thus, the
inspecting engineer is to be contacted at least 24 hours prior
to the start of construction° If necessary, a pre-
construction meeting will take place on-site.

3. Insulation shall be at least 2" 'thick extruded direct
burial polystyrene (Dow Chemical Company Styrofoam HI or
equal).
4. Septic tank inlets and outlets shall be fitted with
watertight couplings (Caulder, Fernco, or equal).
A permeable nontoxic silt barrier (Typar 3401, Mirafi
140N, or equal) must be installed between the final
leachfield gravel layer and the native soil backfill.
Ail leachfield gravel (sewer rock) shall be 0.5"a2.5''
screened gravel with less than 3% passing the ~200 sieve.
7. When sand is being used as a filter material, it's
gradation specifications must conform to AMC 15.65.060D.

Re: Mound System Lots 13 and 14, Williamson Subdivision
Dear Mr. Kevrekian,
In response to your request, I have discussed the lack of an
as-built drawing of this mound system with Mr. John Smith of the
Municipality of Anchorage, Department of Health and Human Services,
(DHHS) and present this letter and the attached information to
satisfy their need for this information in processing the Health
Authority Approval.
History
In 1979, I designed an experimental mound waste water disposal
system :for this lot 'bo replace the existing system that had fa~.led
due to high ground water and avoid the need and ongoing expense of
a holding tank. The mounded disposal system used the existing
septic tank inspected and approved by the DHHS. The system was
designed to distribute the septic tank effluent through a dosing
tank by means of 2 inch perforated PVC pipe in trenches 2.5 feet
wide with 10 inches of gravel below the distribution pipe. Three to
five feet of sand was to separate the bottom of the distribution
trench from the native soils and the bottom of distribution trench
was to separated from the highest water table observed during our
testing for design by 5.3 feet.
Drawings dated June 13, 1980 were submitted to the DHHS and a
permit was issued September 10, 1980. The permit notes that the
system was to be inspected and as-built drawings were to be
prepared by J. M. Lambe. No as-bui].t drawings exist in the DHH$
files.
In May 1993, I was asked to research my files for such an as-
built drawing. My files have been moved several times in the
thirteen years since this system was built and are not complete,
but I found no as-builts in my files, nor any record of as-built
inspections nor any record of payment for inspecting the system
during construction.
I discussed this project with Messrs. Les Buoholz and Robert
Pratt formerly of the DHHS and now with the Soldotna Office of
ADEC. Mr. Robert Pratt remembers approving.the mound system.
I discussed the project with Mrs. Andrew Loyer, of D and S
Unlimited, who built this system and obtained the permit. She
indicated to me that while I came to the site several times during
its construction, it was more for a matter to satisfy my curiosity
for she specifically remembers that t was not retained by them to

inspect the system because this system was to be the first
operating mound in Anchorage and an inspector from the DHHS was on
site nearly full time. I vaguely remember visiting the site during
construction. I also remember discussing the site with Mrs. Loyer
sometime after construction when the pump in the dosing tank was
required to be replaced. It was my experience that when working
with D and S Unlimited on other projects, Mr. Loyer was a
conscientious contractor who generally built sewer systems in
accordance with the design.
I asked Ms. Laura Ward of the DHH$ to pull the chronological
files to determine who from the DHHS was on site inspecting the
system, however I was told that all of these records have been
removed from DHHS files without authority.
Because I seem to be the only person in Anchorage with
knowledge of this system~ Mr. Smith indicated that on the basis of
the design drawings, an as-built survey of the lots prepared by
Lantech, and such information that is currently readily available~
that the system should be documented on one o£ the Department's
Built £orms, and that this documentation should be sufficient to
their immediate needs to supplement the "adequacy testing" being
performed on the site by S and S Engineering.
I have visited the site and discussed my findings with each of
Mr. Scott Swenor of S and S Engineering and Mr. Dan Roth of the
DHHS.
Observations
I have visited the site several times, reviewed the design
drawings, discussed the site at length with Mr. Scott Swenor of S
and S Engineering, obtained aerial photographs of the site during
constr~otion, and observed some of the work that S and S
Engineering has performed during the Health Authority Approval.
I observed that the top of mound ia approximately § feet above
the garage slab elevation. This height is consistent with the
design drawings. I measured the apparent top of mound and plotted
this on the as-built survey. The configuration of the top of the
mound is consistent with the first design, and nct the
alternative. I measured the location of the observation well
installed by S and S engineering reported to be installed in the
gravel section of a distribution ditch. This location agrees with
the middle trench location of the first design. I conclude that
the mound and distribution trenches were built in general
accordance with the three trench configuration of the first design
as presented in the drawings on file with the DHHS.
I note that the fence, indicated on the as-built survey as at
the toe of the mound, appears to have been placed on the cleared
slopes of the mound and that the toe extends beyond the fence. The
footprint of the mound then also appears consistent with the
design,

I observed the dosing tank and pump installation. I could not
identify the pump, and the pump controls appeared to be mercury
switches instead of float controls, however the installation
appeared to be appropriate for its purpose. I cycled the pump and
noted that the motor operated when the appropriate mercury switch
was activated. The electrical connection was an outdoor electrical
box into which the pump and pump control were plugged. While
consistent with early similar installations, this electrical
installation probably does not meet current code. If inundated,
this connection could present a hazard to health and safety, and
should be rewired in accordance with current code requirements.
I observed no check valve to be installed, however I remember
revising the design to include a goose neck at the mound system.
The goose neck is noted on the drawings that you possess and haw~
given to S and S Engineering.
I observed that the creek indicated on the Lantech "as-built"
survey appears to have recently moved from its original channel and
turns 'south and enters the property and now actually meanders
outside of the drainage easement. This channel of the creek is
cutting through a dense stand of alders (willows) less than 10 feet
from this turn, and the roots of a spruce tree downstream. I
interpret from the aerial photographs that this channel was not
apparent during September 1980. This is consistent with the notes
on the design that the mound was to be 100 feet from the creek. It
is also consistent with the fact that the existing septic tank was
approved by the DHHS at its current location presumably with the
prOPer separation from the creek. It is also consistent with the
observation that a 1976 "As-Built" survey of Lot 18 makes no
mention of a creek. It is also consistent with our telephone
oonver~ation of May 12, 199~, when you indicated that your
construction plans for a swimming pool were terminated due to the
fact that the creek had moved from it original alignment, south
towards the mound. None--the-less, the existing septic tank, dosing
tank and the edge of excavation of the mound system are now within
100 feet of the new alignment of the creek. I observe that even
with the recent realignment of the creek that the design locations
of the distribution ditches of the mound system are separated from
the creek by more than 100 feet.
I observed no observation pipes nor permanent markers at the
ends of the distribution trenches.
Conclusions
Based on the foregoing, I conclude that the Experimental Waste
Water Disposal System - Mound Design appears to have been built in
general accordance with the design and the drawings on file with

the DHHS. Observed exceptions are the deletion of the check valve
and substitution of the goose neck, and the modifications to the
pump and controls. Additionally, it appears that the observation
pipes and permanent markers either were not installed, or have been
removed subsequent to construction. I have filled out the DHHS as-
built form accordingly.
I am not an electrical engineer nor inspector, but I surmise
that the electrical connection in the manhole probably does not
meet current regulatory requirements and should be brought up to
these requirements.
I conclude that the creek to the north of the system probably
has meandered subsequent to construction of this system. I would
surmise that this is due in part to the increase in runoff
accompanying development of the Hillside. I would conclude that
the creek could be returned to its original channel, however in my
opinion to do so would not substantively reduce any risk to public
health associated with this system and may require concurrence from
numerous regulatory agencies and adjacent landowners. The
information indicates that the distance from the distribution
trenches in the mound is more than 100 feet from the surface water
and because the trenches are underlain by :filter sand between the
edge of excavation and the trench, in my opinion they should
present little risk to public health if the mound is operating
correctly. Evaluation of the operation of the mound is beyond the
scope of this letter.
The existing septic tank, inspected and approved by the DHHS,
and the dosing tank are also within 100 feet of the surface water.
The Corrugated Metal Pipe 'that serves as access to the manhole in
the dosing tank has separated from the septic tank and is leaking
a little water into the tank. While it does not appear that this
leak is~in sufficient quantity to affect the mound system, current
regulations require the dosing tank to be watertight. This work
and verification of the integrity of the existihg dosing tank and
septic tank would have to be performed prior to the issuance of any
waiver of separation distance to the creek.
I understand from discussion with Mr. Roth that the integrity
of the both tanks will need to be ascertained prior to the issuance
of any waiver. From these discussions, I understand that for
upgrades the tanks must be exposed and inspected for integrity, but
for Health Authority Approvals, the tanks can be inspected for
integrity by observation from the inside and/ or water level
measurements. Mr Roth related to me that most tanks older 'than 10
years are replaced during upgrades due to corrosion. Because the
tanks are located in plain sight near the driveway and the fact
that the dosing tank is equipped with a high water alarm, the
likelihood overflowing 'the tanks to an extent presenting a risk to
human health is, in my opinion, ].ow and you could probably obtain
a waiver in regard to the inadequate separation distance if the
tanks were determined to have no leaks and the manhole access was
brought up to current standards. Howew~r, due to the concern over

the age of the tank~ in regard to tank integrity and your need to
resolve this issue as quickly as possible, it would seem that the
quickest way to resolve this issue would be to replace the existing.
tanks with the Septic Tank Effluent Pumping system recommended by
S and S Engineering in an area adequately separated from the new
creek channel.
I trust the foregoing and attached are sufficient to your
immediate needs. As you directed, I discussed this letter with Mr.
Swenor prior to its final revision. Please call if I can be of
further service.

Replacement Alternatives to Existing Septic System
The department has reviewed the data on file, in our office, and
as presented by John Lamb, on this case.
It is our recommendation that a lift station be installed and
lift 'the septic effluent to a draintrench installed on the south
and southwest property lines. The exact details will be computed
when Mr. Lamb applies for a permit and after a soils test has been
conducted in this area. The option of an elevated absorption
field in the lower part of the lot was considered unsuitable because
it will discharge effluent into the water table. The final option,
of course, lsa holding tank.

Dear Mr. Swenor:
I have completed a review of this office's files concerning the monitoring status of the
above-referenced Class "A" Public Water System and found the following:
The last satisfactory Total Coliform Bacteria Sample results was submitted
to this Department on May 11, 1993, This does meet the provisions of 18
AAC 80.200(a) of the State Drinking Water Regulations.
The last inorganic Chemical Contaminants Sample results were submitted
to this Department on June 15, 1993. This does. meet the provisions of 18
AAC 80.200(a).
The last Radioactive Contaminants Sample results were submitted to the
Department on April 11, 1993. This does meet the provisions of 18 AAC
80.200(a), State Drinking Water Regulations.
The last Organic Chemical Contaminants/Volatile Organic Chemical (VOC)
were submitted to this Department on May 26, 1992. Based on analysis of
the previous VOC samples results have been satisfactory. This does meet
the provisions of 18 AAC 80.200(a), State Drinking Water Regulations.
Issuance of this letter does not imply that the above-referenced Class "_A" Public Water
System is in compliance with other provisions of the State Drinking Regulations. Unless
otherwise noted, this letter is valid for 30 days and is for the specified legal description
noted above only.
