Ref: Proposed Septic Tank Upgrade for Greenbrook; Block 2, Lot 3
To whom it may concern:
The existing 4 bedroom house is served by a community well and a private septic
system.
Per MOA onsite employee Tim Ecklund, a site visit was made to the subject property by
himself on 3/22/2018, prompted by the age of the steel tank (35 years old per MOA
records) noted by GEG on the recently submitted COSA. Per Mr. Ecklund, the tank
liquid levels were approximately 3 inches lower than what was measured by GEG on
the day of the septic test (48" of liquid in the tank on 3/7/2018, 45" of liquid measured by
Mr. Ecklund on 3/22/2018). A 3 inch drop since the last GEG evaluation is an indication
that the tank is leaking due to age/corrosion. Per Mr. Ecklund, before a COSA can be
approved, the septic tank must upgraded.
We are proposing to replace the existing 1,250 gallon steel septic tank with a new 1 ,250
gallon steel tank. We are unaware of any adverse impacts this installation would have
on adjacent wells or septic systems. If you have any questions, please contact us at

the bottom of the excavation(in feet). There is no set width for trenches.
The gravel depth is the minim~ depth of gravel between the outfall pipe and

Backfilling of any system without final inspection and approval by this department
will be subject to prosecution.
Minimum distanCe between a well and any on-site sewage disposal system is 100 feet
for:a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minim~ distance from a private well to a private sewer line
is<25 fe~t and to a co--unity sewer line is 75 feet. Well logs are required
and.must be returned to this department within 30 days of the well completion.

available to insure proper installation.

Dear Mr. Curtis:
Transmitted herein are the results of laboratory tests run on
soil samples collected during excavation on the subject prop-
erty on July 12, 1983. A hole was excavated approximately 25
feet south of the new foundation to a depth of approximately
8 feet.
The excavation was conducted using a backhoe provided by the
client. Jan Mulder, engineeriog geologist with Alaska Test-
lab, logged the soil conditions encountered and collected
grab samples of the two major soil horizons. These samples
were visually classified in the laboratory, and a grain size
analysis was conducted on the gravelly sand horizon.
Soils encountered consist of a surface layer of brown organ-
ies to a depth of approximately 0.5 feet. This is underlain
by F2, brown silty gravelly sand (SM-SW) with some fibrous
organics to a depth of 3.0 feet. Soils encountered from 3.0
feet to the limit of the exploration consist of NFS, grey
gravelly sand (SP).
Ground water was encountered at approximately 8.0 feet. A
perforated RVC standpipe was placed and the hole backfilled
to approximately 4.0 feet. The water level was measured four
additional times during the following six days. ]he water
level remained constant throughout this period. No percola-
tion test was performed on the material of interest since it
was classified as NFS, qravelly sand (SR).

Test Hole LoS - Description Guide
The soil descriptions shown on the logs are the best estimate of the soil's
characteristics at the time of field examination and as such do not achieve the
precision of a laboratory testing procedure. If the log includes soils samples,
those samples receive an independent textural classification in the laboratory
to verify the field examination,
The logs often include the following items:
Depth Interval - usually shown to 0.1 foot, within that zone no
significant change in soil type was observed through drill action, direct
observation or sampling.
Frost Classification - NFS. Fl, F2, F3, F4, see "Soil Classification
Chart"
Texture of Soil - An engineering classification of the soils by particle
size and proportion, see "Soil Classification Chart", note the
proportions are approximate and ~nudifications to the soil group due to
stratification, inclusions arid changes m properties are included.
Moisture Content this is a qualitative measure:
d_~., m~ or little apparent surface moisture,
damp, moisture forms portion of color, less than plastic limit,
wet, no free water, ofien soft, if cohesive soil,
saturated, free water may be squeezed out, ifa free draining soil;
dilatent at natural moisture content, if a non-plastic silt or fine
sand. (The moisture content is further definedby reference to PI,
LW, NP, M%or dilatency.)
Density - refers to more-or-less non-cohesive soils, such as sand gravel
mixtures with or without a fine fraction, derived from drilling action
and/or sample data; usually described as: very loose, loose, medium
dense, very dense. General intent is to portray earthwork
characteristics.
Stiffness - refers to more-or-less cohesive soils and fine grained silts of
t-~ clay-silt groups. Derived from drill action and/or sample data. Very
soft, soft, stiff, very stiff and hard are commonly used terms.
Particle size -- The largest particle recovered by the split spoon is
1-3/8", Shelby tube Y', auger flights (minute-man) 2", Auger flights
(13-50 hollow stem) 6"-8". Larger particles are described indirectly by
action of the drilling and are referred to as cobbles, Y' to 8", or
boulders 8"+. Therefore when rcviewing the gradation sheets, if any,
the description on the hole log must be considered for an"indicafion of
larger particles.
Unified Soll Classification - This is a two letter code. See Unified
Classification sheet for further definition. In some cases AASHO and/or
FAA soil classifications may be shown as well as the unified.
Atterberg Limits - useful for fine grained and other plastic soils.
PI; natural moisture content believed to be less than plastic limit
PI+; natural moisture content believed to be between plastic and liquid
lTmits
L.__~; natural moisture content believed to be greater than liquid limit
NPi non-plastic, useful as a modifying description of some silty
mattrials.
Dilatency - is the ability of water to migrate to the surface of a
saturated or nearly saturated soil sample when vibrated or jolted - u~ed
as an aid to determine if a fine grained soil is a slightly or non-plastic
silt or a volcanic ash.
Rock flour - finely ground soil that is not plastic but otherwise appears
similar to a clayey silt.
Organic Content - usually described as Peat, PT, sometimes includes
discrete particles such as wood, coal, etc. as a modifier to an inorganic
soil. Quantity described as; trace, or an estimate of volume, or, in case
of all organic, - as Peat. This may include tundra, muskeg and bog
material.
Muck -- a modifier used to describe very soft, semi-organic deposits
usually occuring below a peat deposit.
Amorphus peat -- organic particles nearly or fully disintegrated.
Fibrous Peat - organic particles more-or-less intact.
Bottom of Testhole - includes last sample interval.
Frost Line - seasonal frost depth as described by drilling action and/or
samples at the time of drilling.
Frozen Ground - other than frost line, described by samples, usually
includes description of ice content, often will include modified Unified
Classification for frozen soils - this is a special case related to
permafrost studies.
Free Water Level - The free water level noted during drilling. This is
not necessarily the static water table at the time of drilling or at other
seasons. Static water table determination in other than very permeable
soils requires observation wells or piezometer installations, used only in
special cases.
Blow/6' - The number of blows of a 140 weight free falling 30" to
advance a 2" split spoon 6"; the number of blows for a 12" advance is.
by definition, the standard penetration.
M% - natural moisture content of the soil sample, usually not
p-~rformed on clean sands or gravels below the water table.
Type of Sample -
S__P, refers to 2" split spoon driven into the soil by 140 pound
weight, a disturbed sample,
S, thin wall tube, "Shelby" used to obtain undisturbed samples
of fine grained soil,
G, "grab" disturbed sample from auger flights or wail of trench,
~ cut sample, undisturbed sample from wall of trench.
Dry Strength - a useful indicator of a soil's clayey fraction, N=None.
L=Low, M=Medium, H=High
Group - The samples are placed into apparently similar groups based
on color and texture and are arbitrarily assigned a group letter. Further
disturbed tests including Atterberg Limits, grain size, moisture~lensity
relationship, etc. may be performed on the group and a.re assumed to
reflect the general distrubed characteristics of the soils assigned to the
group. This is an important phase of the soil analysis and is used to
standardize the various qualitative determinations and to reduce the
number of quantitative tests necessary to describe the soil mass.

TEST PROCEDURE: System was inspected and measured. Tank was
found with 8 feet of cover and wi'th a liquid level o.F 48.5 inch-

The operational life of all septic systems depends on the local
soil conditions, groundwater levels that may fluctuate during the
year, and the water usage of the family being served by the
system. These conditions are outside the control of the evaluator
of this septic system. We can therefore not give any estimate of
how long the system will continue to meet the operational requi-
rements of the Municipality and State.

To whom it may concern:
The subject property has a 4-bedroom home that is served by an onsite septic system consisting of a 1,250
gallon steel septic tank and bed type drainfield (50' x 25') that was installed 7/29/1983. The bed was
exposed on 3/6/2018 by ARM Services while replacing an existing sump at the southwest end of the
drainfield with a new monitoring tube and two cleanouts. Per ARM,the distribution line was uninsulated,
frozen with approximately 2-3"of sludge, and only 2'-4"of soil cover(see attached photo).The line was
then jetted,allowing for the system to be tested(see adequacy test results on COSA form).Soil cover could
not be added at this time.Per GEG elevation shots,the south 6 foot portion of the bed appears to be shy on
cover by approximately 4", however it is the responsibility of the contractor to ensure 3 feet of soil cover
over the entire drainfield. We are requesting that your department issue a conditional COSA with the
provision that additional soil cover be provided in spring of 2018 (see attached bids from contractors).
