Subject: Greenbrook, Block 2 Lot 4 – 13000 Von Scheben
Septic system design
Dear On-Site Services Engineer:
The owner of the above lot has a 4-bedroom home on the property, with an aging system. We are
submitting this permit application for the construction of a new well and septic system. The
attached site plan identifies the location of the home as well as the septic location. No conflicts
exist between this proposed system and any other well or septic system, whether on this lot or
adjacent lots.
The ground surface on the lot is mild. There are no slopes greater than 25% within 50 feet
downslope of either the primary or alternate site. Contours are shown on the site plan showing the
grade and direction of flow. Stormwater drainage will not impact this septic system. The new
trench will be constructed parallel to the slope as much as possible.
Wells on this and adjacent lots are shown. The new system will be a minimum of 100’ from all
wells and surface water, and more than 5’ away from the septic tank. Please refer to the attached
test hole log, plan and profile pages for the septic design. If this design is followed, there will be
no adverse impacts to adjacent properties.

An On-Site WaterAVastewater Permit, number SW010395, issued by this office for a single-family
system, will expire on September 26, 2002. This permit was valid for 365 calendar days.
If this was a xvell permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as-built inspection report must be sent to
this office for review, approval and documentation. This as-built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As-built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $100.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $400J00 for a wastewater permit and $150.00 for a xvell
permit.

To Whom it may concern:
This is a request for a new septic permit at the above referenced location. The pert test hole was
excavated to 14 feet with tight silts on top then sandy gravel starting at 5.5 to the bottom at 14
feet. A new 5 wide trench has been designed starting at 5.5 feet and ending at 7.5 feet, with 2.0
feet of effective depth. The perc rate was 2 minutes per inch but a slower rate will be used for
longevity of the system. A new tank will be installed if the old one is found to be rusted out.
The old perc test had water at 9 feet but this was on the lower elevation which is at least 3 feet
lower in the front of the house. Water in the new perc hole was measured at 12.5 feet thus the
new system can't be any lower than 7.5 feet. Other test holes were excavated but nothing was
found which had the clean sandy gravel.
Due/closeness of the north property line a waiver is requested of 5 feet to keep the system near
the test hole. The neighboring property will not be impacted by this new system due to the
community water supply.
This new system will not affect the future development of the neighboring properties due to the
community water supply. The general drainage is to the south and east at approximately two
percent, with slopes being less than 2 to 3 percent.

Dear Mr. Pace:
At the request of John Hardwick of your company a pressure test
of the Greenbrook Subdivision water system was accomplished at 14:00
hours on June 13, 1979.
This test was observed by Tryck, Nyman & Hayes and the results
are certified by TN&H as follows: 125 pounds of pressure was applied
to the distribution system for 30 minutes with no drop observed.
In addition, the well was pumped using the operational pump for
5-1/2 hours with rate of flow measured at 46.8 gallons per minute. Prior
to pump Static head was 8' above ground levet after 5-1/2 hours draw
down in the casing was 40' and holding following shut down of the pump
initial recovery at 1 minute was 20' with recovery to ground level in
6.5 minutes.
Compared to initial tests, see attachments, this shows considerable
improvement in the well since May 2, 1977 with ultimate production
in the 100 gallon per minute range possible.
