I am pleased to report that both systems appear to be performing adequately at this time. The on-site systems were originally designed and approved by the State DEC to serve a 4 -bedroom duplex. Since this property has now been converted into a 4 -bedroom single-family residence it will be necessary to submit copies of all relevant documents and adequacy test data to the Municipality to obtain a Certificate of On-site Approval (COSA). I will do this shortly and request the Municipality to call you when the COSA has been approved and is ready for pick-up. The following is a report of my test procedure. I was unable to locate a driller's log for the well, however other documents on file with the State and the Municipality indicate that it was probably drilled in 1975 and that it has a total depth of 140 feet On April 20, 2009 the static water level in the well was at 60 -feet below the top of the casing. Steady pumping of 722 gallons of water from the well at the maximum pump rate of 7.6 gpm caused the water level inside the casing to be drawn down to a maximum of 104 feet. Based on this well test data I determined that the well yield is in excess of 7.6 gpm, which is more than adequate for any single-family residence. Water samples collected on 4/20/09 were satisfactory, showing 0 coliform or other bacteria per 100 ml., less than 0.1 mg/1 of nitrate -N and 5.03 micrograms per liter of arsenic. The wastewater disposal system was completely upgraded in October of 1995 and consists of a 1500 - gallon septic tank that discharges effluent into a 55 -foot long soil absorption trench that has an effective gravel depth of 9.6 feet. To assess the adequacy of the soil absorption system on 4/20/2009 I added 722 gallons of water into it while monitoring fluid levels in the tank and trench before, during and after the flow of water was stopped. Prior to the start of the test the fluid depth in the monitor tube at the end of the trench was 104 inches and after the addition of 722 gallons of water it was 115.6 inches. After the flow of water was stopped the fluid levels receded 5.5 inches over a two hour period, which corresponds to a more than adequate absorption rate for a 4 -bedroom residence. In conducting an adequacy test I attempt to provide a thorough, conscientious engineering analysis of the system. The test results describe the performance under the conditions encountered on the day of the test and do not guarantee future performance under different conditions. The well has long been recognized as a Class C well, but to the best of our knowledge has never been approved as such. A separation distance waiver was granted in 1986 for the existing sewage system distances from the well. There is a private well on Lot 4 100' from the existing septic tank and an approved Class C well on Lot 6B which is 125' from the existing septic tank, 155' froin the existing drainfield. Based on submitted information, no other wells are affected by the subject septic system. Submitted information included nitrate and coliform waver tests, drilling logs, soil tests, area map, map of affected system, and diagram of proposed trench soil absorption system. In addition, file data and verbal information show the ground surface to slope slightly to the north and west. We responded on July 27, 1995, suggesting that the sewage facilities be moved farther away from the wells and sent invoices for the waiver and plan review fees. Your reply dated August 4, 1995, was received on August 9, 1995. In it, you moved the new septic tank and the new drainfield further north and showed monitoring tubes on the new drainfield. You also paid the fees. WAIVER: We have carefully reviewed the submittals per 18 AAC 80.030 and grant a waiver for separation distance for the well on Lot 5 of 95' to the new septic tank and 130' to the new drainfield. This Byron Gajewski, CRW August 22, 1995 Page 2 waiver is only valid under existing conditions and those proposed in the application. Any change in building use from the 4 bedroom duplex or in the sewer system will require a new waiver review. This letter is your approval document. SEWER SYSTEM: We have carefully reviewed the submitted information on the sewer system as well as our file information. The soils appear acceptable, and groundwater does not appear to be a problem. In addition, the existing system does not appear to be affecting the well. Based on the requirements of 18 AAC 72.210, we therefore approve the new 1500 gallon septic tank and the Y x 55'x 14' deep trench drainfield as submitted and revised. This letter is your Certificate to Construct; a Certificate to Operate will be issued upon receipt of acc ptable as-builts from a registered engineer. These as-builts must indicate soils, watertable, and impermeable strata within appropriate distances and that the installation was monitored by a registered engineer. Additional detail concerning the installation of monitoring tubes should be included. We will also need at that time a filled out Domestic Wastewater System Owner's Statement and Data Sheet (copies enclosed). Per 18 AAC 72.210, this approval is valid only if construction is completed within two years. WELL: As previously mentioned, we have no information that the well on Lot 5 was ever approved as a Class C well. Pleas! either furnish such information or apply --or approval of the well. (The records the owner has may include an initial approval by the Anchorage Borough.) We are enclosing a check list for your use in such a submittal. Soils for the deep trench drainfield were uniform and identical to the soil boring originally taken within 10 feet of the trench. A new 1500 gallon septic tank was installed. The original tank was pumped, crushed and backfilled. The attached as-builts provide grades of the new piping and detailed locations.
